01.01.2022 The HIV Outreach Team (HOT) provides outreach services to people living with HIV with complex health and social needs in SESLHD/ISLHD. The multidisciplinary team uses a case management approach to deliver services in partnership with clients and service providers including primary care, hospitals, community services and NGOs. A culture of promoting innovation amongst staff to deliver seamless, compassionate and efficient integrated care for clients/families is a key focus of the team which has adopted an asset-based approach to health and wellbeing.
