25.01.2022 Accidents happen! One of our patients took a board to the face while surfing and suffered a deep laceration of the lip and gum down to the bone. He was referred to our office where we were able to carefully suture the wound and examine his teeth. 3 months later, his injuries have healed amazingly well and his teeth have maintained their health without requiring any endodontic treatment



24.01.2022 This patient was referred for endodontic treatment by a diligent dentist who extirpated and located 3 canals in this upper left second molar. The patient was concurrently being treated for a mobile implant-crown at the #26 site. Upon accessing the tooth I located a second mesial canal very close to the palatal canal and then took a CBCT scan after medicating the canals with calcium hydroxide. This confirmed the presence of an MB2 and mesio-palatal canal as well as pressure resorption of the disto-palatal root apex from the unerupted 28. The treatment was completed over two appointments with an MTA apical root-filling in the disto-palatal root. A review CBCT is planned to see whether the disto-palatal root resorption from the 28 is progressive or arrested.

20.01.2022 This album is a case-based illustration of how we approach orifice location. The patient was referred for endodontic treatment of tooth 36 due to challenges locating and negotiating the calcified canals.

18.01.2022 This patient was referred for treatment after a file fracture complication was encountered in the second distal root of a lower molar (radix entomolaris). The additional distal root can increase treatment complexity because it is uncommon, difficult to detect on radiographs and can follow a sharp or divergent curvature compared with the other roots. In this case, the file fragment was first bypassed with small hand files (6, 8, 10 ISO size) and then while establishing a glidepath, it came loose and was removed from the canal. Tooth 47 appears to have had some endodontic or vital pulp therapy performed before but whilst unresponsive to pulp testing, was asymptomatic and well restored, hence will be kept under review.

15.01.2022 This patient was referred for endodontic treatment of tooth 46 with a large swelling adjacent to the tooth. Treatment was performed over two visits with a calcium hydroxide medicament and 6-month review showed significant apical healing and an absence of symptoms.

11.01.2022 This 13 year-old patient was referred to me by a conscientious dentist who had identified a radiolucency at the DEJ of tooth 47 but could find no clinical evidence of caries/cavitation. Being uncertain of the aetiology, the dentist was reluctant to drill into a seemingly intact, virgin molar tooth and sought a second-opinion. The patient's presenting complaint was sensitivity to cold water and tooth 47 was partially erupted and responded normally to pulp testing. Careful exa...mination with the microscope, transillumination and radiographs confirmed the radiolucency but no sign of demineralised or cavitated enamel or resorptive entry points. Differential diagnoses of 1. "Hidden"/"Occult" caries or 2. Pre-eruptive Intra-coronal Resorption, were proposed. We decided to remove the undermined enamel and restore the defect rather than risk the undermined enamel collapsing and resulting in deep caries/pulp necrosis in an uncontrolled environment. Upon unroofing the defect, a necrotic, friable, connective-tissue-like structure was observed (consistent with intra-coronal resorptive tissue), but no sign of dental caries and no pulp exposure. The tooth has been restored and I am hopeful it will maintain pulpal vitality :) See more

04.01.2022 This patient was referred with a history of persistent, recurrent swellings adjacent to his lower right first molar which was endodontically treated some 3-4 years ago. Endodontic retreatment was performed over 2 appointments and at 12 month follow-up we were pleased to see a complete absence of symptoms and significant healing occurring :)

03.01.2022 Clinical Tip: GP Cone fit Anyone who performs root canal treatment would know the frustration of GP cones that won't seat at the working length. A number of factors play a role in this, including preparation technique and canal curvature but one factor outside the operator's control is irregularities in the GP cone size i.e. there is often some discrepancy in the purported size and the actual size of the GP cone due to the margin for error in the manufacturing process. A few years ago a friend got me onto DiaDent GP cones which are laser inspected during manufacturing and accept only a .029mm tolerance for diameter. Since using these GP cones, I've found a significant reduction in time wasted! Give them a try and let me know if you have any other hacks :)

03.01.2022 This patient had been suffering recurrent swellings associated with an upper left premolar which was endodontically treated and restored with a cast post-core and crown many years ago. Surgical retreatment was performed and one year review revealed an absence of symptoms and disease :)

02.01.2022 I love watching nature do its thing. This 10 year old patient developed an acute apical abscess after falling and suffering a crown fracture and luxation injury to her upper left central incisor. The referring dentist had extirpated the tooth and referred the patient approximately 10 months later. Upon preoperative examination, I observed a hard-tissue barrier was already forming in the apical third of the root because the referring dentist had done such a good job of disinfection already. I had the fortune of obturating the canal with MTA and watching the apical hard-tissue formation continue
