22.01.2022 The piriformis muscle is a fan-shaped muscle found deep in the buttock. It attaches to the sacrum at one end and the upper portion of the femur at the other end. The piriformis muscle helps us turn our leg outwards when our legs are down and straight, like when we stand. However when we bring our knees up towards the ceiling such that our thigh is at 90 degrees to our body, the piriformis becomes a weak abductor (sideways elevator of the leg) and even an internal rotator. Du...e to the intimate relationship between the piriformis and the sciatic nerve, when the muscle gets tight or injured, it may exert pressure on the sciatic nerve leading to pain and even tingling or numbness in the buttock and down the back of the thigh - a condition called piriformis syndrome. It has been suggested that various anatomical variations may predispose an individual to piriformis syndrome- nerve splitting; piriformis splitting and nerve travelling entirely above or below the muscle. Though this remains a point of conjecture. And it is worth pointing out that not all nerve-type symptoms extending down the lower limb come about through piriformis syndrome. 19.01.2022 Ahhhhh.. where would civilised society be without stilts!? Consider the unadulterated joy of being 3-4 metres tall whilst you stroll along the beach. At least thats what Katie, Darlene, Fredericka and Lola thought one bright summers morning. Laughter filled the air. Yet moments later all were carted off to their nearest Accident and Emergency Department for an assortment of injuries that included cruel levels of splintering, nasty knee ligament tears and shoulder dislocatio...ns and subluxations. 15.01.2022 Spiral fractures are complete fractures that can occur to long bones such as the tibia, fibula, femur and humerus and even metacarpals (hand) and metatarsals (foot) that result from a rotational/ twisting force applied to the axis of the bone. Spiral fractures are usually the result of high energy trauma and are likely to be associated with displacement (the fracture fragments do not line up) though not always. These sort of fractures are known to occur in high velocity and i...mpact sports (footy, skiing, basketball etc). They often need orthopaedic intervention- operation to align and use hardware to maintain alignment (known as open reduction and internal fixation-ORIF); a period of immobilisation followed by rehabilitation with physiotherapy and graduated return to sport and/ or normal life. Early physiotherapy rehab usually involves regaining weight bearing, settling pain & swelling and returning to normal ranges of movement (such as regaining full range of ankle mobility with a spiral fracture of the lower portion of the tibia). And later stage physio rehab is preparing the patient for return to normal day to day activities and eventually sport (again with the tibial fracture example- gradual increase speed of running; changes in direction at speed; balance drills; strength and endurance and so on). 13.01.2022 Hamstring injuries are one of the most common injuries to occur in the community. From professional athletes to every day sport orientated adults and kids. very common. What is probably most concerning is the recurrence rate of hamstring injury. Stats suggest that re-injury occurs in over 30 percent of people in the first two weeks after return to sport. The hamstrings are made up of three muscles- biceps femoris, semimembranosus and semitendinosus. A large portion of the h...amstring muscle group cross over two joints (the hip and the knee). Therefore they contribute to movement at the hip (hip extension) and knee (knee flexion). There are a few two joint muscles like this throughout the body- the biceps brachii (upper arm); gastrocnemius (upper calf) and others but it isn’t overly common. Hamstring injuries commonly occur when the hip is driving forward into flexion whilst the knee is moving into extension. This phase of running occurs when the leg is swinging forward and the foot (though still in the air) is just about to plant down onto the ground. To make things worse, if there has been a previous injury to the hamstring and you are attempting to return to running/ sport there are a few things which can contribute to recurrence after even a modest lay-off: inadequate strength (eccentric is often cited though both eccentric and concentric strength maybe inadequate); reduced muscle length and reduced muscle flexibility which can be the result of residual scar tissue post initial injury. There are plenty of other factors that need consideration too- neural mobility; opposite leg flexiblity etc etc. The take home message is- get your hamstring injury treated properly the first time round by a professional. No short cuts. Recurrence of injuries can drive you nuts and result in far longer off sport than is necessary. 08.01.2022 The hip is a ball and socket joint comprised of the head of the femur (the ball) and the pelvic acetabulum (the socket). The labrum is the cartilage the surrounds the outside lip of the socket which helps add depth to the joint, improve joint stability, cushions and facilitates smooth, normal movement. Injuries to the labrum can occur through traumatic injury, sport or take place gradually over time- this can happen to younger patients and older patients alike. A high percent...age of people with confirmed hip labral tears, cannot identify a preceding injury. Whilst symptoms vary, pain often presents in the hip or groin region and/or a catching or locking sensation may occur intermittently at the hip and/or significant reduction in hip range or movement. There are specific clinical tests that can help define the origin of the symptoms- including the labrum. However an MRI or even an MRI using a contrast medium is a far more reliable tool in reaching a definitive diagnosis of labral tear. Treatment for labral injury with physiotherapy usually follows three phases- Phase 1: settle pain and unload labrum/ Phase 2: restore mobility and strength and Phase 3: graduated return to sport or activity. Unfortunately in some cases surgery will be required. Using an arthroscopic technique the damaged labrum is either repaired or excised. Physio is often required post-surgery to restore strength and flexibility and supervise a graduated return to normal activity. 07.01.2022 From time to time people dislocate or sublux a joint- most commonly a shoulder or a kneecap or even a finger. Dislocations are usually pretty obvious. The person is in a lot of pain, the joint itself looks odd/ disfigured and usually it physically has to be relocated. Some repeat offenders can relocate their joint with relative ease whereas first time felons can often have great trouble returning the joint to its natural position (sideline physio, ambulance or hospital A&E a...re usually needed). Subluxations are sometimes a little less obvious. A subluxation is where a joint partially dislocates- that is the joint has moved out of its normal location but not completely out of position. And subluxations usually relocate themselves immediately/ spontaneously. Often a person will describe a brief moment of instability at a joint- "I felt the knee go out just for a second" or "my shoulder felt like it bobbled out of position for a moment". Despite the immediate relocation of the joint- the damage can be significant. Pain (moderately severe-severe), swelling and restricted mobility are common. 01.01.2022 The piriformis muscle is found deep in your buttock. It runs from the sacrum (at the base of the spine) to the top of the femur (thigh bone). Like many muscles it can be prone to tightness and that in itself can produce localised pain and restriction. The piriformis muscle may in some cases even exert pressure on the sciatic nerve which is in close anatomical proximity. Try this stretch for size for the right piriformis: 1. Lie on your back with the left knee flexed 2. Cross... the opposite right lower leg to rest on the left knee/ lower thigh 3. Reach through with your hands to pull the left thigh towards your chest. In doing so youll pull the crossed right leg toward your chest also. 4. Feel a comfortable stretch or strain deep in your buttock only. No pain. 5. Hold the stretch for 20-30 seconds 6. Relax and repeat 3-5 times, making sure to do both sides #physio #physiotherapy #piriformisstretch #piriformis
