23.01.2022 There is a time and place for MRI scans, in particular to rule out serious conditions. However when scans show similar degeneration for painful and pain free joints then they are of limited value.

Think deeply about your own beliefs as a healthcare professional. -Are you afraid of pain? -Do you think pain me...ans: there has to be something damaged! -Bending the back is something dangerous and can damage the discs -You have to tense your core muscles to protect the back Be honest with yourself and try to change your mindset about the back. The back is a very strong and robust structure, that can be trusted. Movement is good, just like every other joint in the body. Lets try to reduce fear among us, healthcare professionals, because when we are fearful of certain movements or think the back is vulnerable, then your patients will become fearful too. And what do people do when they are afraid? Tense their muscles, guarding, slow movements. This is not good for our tissues in the long term. Of course, when you are injured, then its good to protect the injured tissues for a few days, but after that its best to gradually load them. What do you think? Let us know! 15.01.2022 ARTHRITIS . I often get asked by patients why have I got arthritis, or how does arthritis happen? . Well lets just say its bloody complicated and still not fu...lly understood, but this is a very simplistic way I tend explain arthritis. . Its not perfect, or 100% accurate so bear with me, but its pretty close. . Our joints cartilage is both a-neural and a-vascular meaning it doesnt have a nutrition or waste removal service. . Instead it relies on the movement of water in an out of it to feed it and keep it clean! . Water is constantly being drawn into cartilage by high amounts of molecules called proteoglycans within it, and its then squeezed out by the forces it experiences during movement and loading! . When there is a break down in this transportation of water in and out of cartilage things tend to go down hill quickly . One of the most common misunderstandings about early arthritis is its often caused by increased levels of water in the cartilage. . This is more often than not due to not ENOUGH water being squeezed out of cartilage meaning waste products are not removed and the cartilages environment becomes toxic. . This then begins a chemical cascade and the shit show of cartilage degeneration begins. . Our joint cartilage needs tonnes of force everyday to squeeze water out of it and remove these waste products. . However, the GOOD NEWS is early OA is often reversible, yes reversible by INCREASING joint loading to force more water and waste products out of and improve cartilages cellular environment. . This is why exercise and joint loading is so IMPORTANT and ESSENTIAL for arthritis! . Often is not too much but too little loading that is the culprit! . 14.01.2022 In todays presentation at #IASP2018, there was a huge talk on exercise and pain modulation, and as the current evidence indicates, it doesnt really matter wha...t type of exercise a chronic pain person does. What matters more is ADHERENCE. This is one of the stories and interviews that was addressed at today. "What the reviews DO NOT say is that therapists and trainers should discard MCE or similar exercise modalities. Almost any type of movement or exercise can benefit those with acute or chronic low back pain. The non-MCE exercises included a wide variety of exercises (i.e., general exercises, aerobic exercises, stretching, general strengthening). It is already known that exercise is effective for low back pain irrespectively for the type of exercise, Saragiotto explained. If you have a previous training or experience on MCE, you should use because it seems to be effective just like other exercises, the same if you are trained in another type of exercise (i.e., Pilates), they seem to be equally effective. 10.01.2022 My thinking over the years has changed regarding the feeling of stiffness in the body in that "feeling tight" is more of a sensation rather than a genuine biome...chanical restriction. Probably the original trigger for that was an article on "Better Movement" which Ill link to in the comments. An article published in Nature this week provides further evidence to support this thinking. ...from the conclusion "Here we show that a conscious experience of feeling stiff does not reflect true biomechanical back stiffness, but may rather represent a protective perceptual inference that may serve to reduce movement and re-injury. By showing that feelings of stiffness do not relate to biomechanical stiffness measures and that biomechanical meas- ures of stiffness do not differ between those with and without feelings of stiffness, this suggests that information other than actual joint stiffness is influencing this perceptual inference. 09.01.2022 A year ago I never could have posted this. No way. No how. This was my shame. This was my horror. But today, I have no hesitation. No nerves. No oh my gosh wh...at will they say?. Just peace. Because I understand now. I understand that this is just the human body. This is a body that has carried three amazing children. This is a body that has been a range of weights swinging 110lbs in difference. This is a body that used to feel worthless and now feels strong. This is a body that needs to be fed with love, attention, kindness and good food. This is a body, and through PRACTICE and EFFORT and EXERCISE in self care, I have grown to love. This is a body that I will use to help show others that they are not alone in their bodies. My friends, this is your life. This is your body. Whether youre tall or small, curvy or striped, this is you. And you are beautiful. And it takes time to grasp these things. Its not something that you SUDDENLY magically feel ok with, but the effort in hate is more draining that the effort in love. So, practice. Because it took years of words, pictures of perfection and endless comparisons to create the voice inside your head to where it is today, and now its time to redirect. Now its time to fill it with new words, new pictures, and valuable connections. Now its time to live. Really live. With peace, and understanding. And a love that will not just fill you, but pour into others. Spreading the truths for more to know. That self love, its so worth it. So, practice.

03.01.2022 MRIs There is a common belief by many patients and healthcare professionals that an MRI scan will definitively tell them what is causing their pain. However,... its just not this easy and sometimes MRI scans can lead to more problems than they solve. The big problem with MRIs and all other scans is the high variability in the interpretation of what is seen on them This study highlights this with ONE patient with stable but chronic low back pain having 10 different scans at 10 different centres within 3 weeks. The results of these 10 scans highlighted 49 different problems, yet NOT ONE of these findings was reported on all 10 scans . Whats one radiologists facet joint arthropathy is anothers normal age related changes. Whats one radiologists disc herniation is anothers degenerative disc disease... etc etc! This is not to say MRIs or other scans are useless or should never be used. In the presence of serious or sinister signs and symptoms MRIs and other radiology tests are invaluable to help make fast accurate diagnosis and quick treatment. However without any of these serious or sinister signs or symptoms MRIs can and do lead to over and misdiagnosis.
