22.01.2022 [LOW BACK PAIN SERIES PART 8 - COMMON QUESTIONS IN THE CLINIC] . . . Four common questions I get asked frequently in the clinic pertaining to back pain:... . IS MY SPINE OUT OF ALIGNMENT? In short: NO!! Most people have zero concept of how strong the human spine actually is. Newsflash: its crazy strong. As in, can handle thousands of pounds before it breaks, strong. So dont you dare for a second let anyone scare you into thinking your spine is fragile. And while Im on the topic: when people crack your back are they moving bones from a misaligned position back into place? NO!! . SHOULD I GET AN MRI? In most cases, no. If you go back to Part 4 of this series, I list a series of signs/symptoms for when getting an MRI for back pain is most indicated. In absence of these symptoms, an MRI is almost always unhelpful. Why? Because so called abnormal findings are bound to show up, and these may have little if anything to do with your pain. We know this because these same abnormal findings are seen in pain free populations. What happens however is people worry, become fearful and attach themselves to these diagnoses which makes recovery more challenging. Last rant: MRI scanners can never see pain. Pain is not a thing you see, its an experience. Case closed. . IS BACK PAIN MORE COMMON WITH AGEING? Generally no. We actually see less prevalence of back pain as people get older than 50. . DO I NEED SURGERY? With the exception of medical emergencies, generally speaking most surgical interventions should be considered as a last resort: following a committed course of conservative care that fails to achieve desired outcomes. Sometimes, surgery is warranted. That decision ultimately should be based on detailed surgeon-patient discussion, in light of proper surgical indications, duration of symptoms, risk-benefit analysis and patient wishes. . In addition he has indicated that they are now developing full allied health telehealth options within the week for allied health along with all other health and medical services. This will be part of a staged plan. Face to face services for many remain vital so where possible, we encourage you to continue. I thank you for your courage, dedication and care, the Minister said. 07.01.2022 Do marathons increase knee damage? Nope. Actually looks like it improves certain MRI changes. The paper I will link to shows that approx 35% of people traini...ng for a marathon had meniscus tears and early cartilage changes under their kneecap. Those tears did not worsen or progress during the marathon. We know that many of us are walking around with meniscus tears and other changes in our knee yet we have no pain. The incidence of these asymptomatic tears appears to be the same as in the general population. This paper might comfort those who are training for races, or those who simply like to run. Remember... osteoarthritis is typically lower in incidence in runners vs an age-matched control group :-). We will be doing these at a reduced rates for private patients given the lack of support from the private health funds. See the link attached for the less than ideal support. However, a proper exercise program has ...to involve resistance exercise. Thats the only way you will minimize the risk of losing muscle mass and strength as we age. Frailty is a tragic process.
