25.01.2022 Changing practice in shoulder dystocia Shoulder dystocia occurs when the anterior, or less commonly posterior, fetal shoulder becomes stuck behind the mo...thers pubic bone (RCOG). Although in the majority of cases the baby will be born promptly and safely with a little help, shoulder dystocia is a life-threatening childbirth emergency recognised by both midwives and obstetricians. In Septembers issue of #ThePractisingMidwife we have an innovative online-only feature documenting how Lesley Ansell, a midwife working in Central Auckland, pursued a change in practice. Following a particularly traumatic birth, involving severe shoulder dystocia and subsequent fracture of the fetal humerus, Lesley undertook a journey to discuss, investigate and research the notion of axillary traction as a method of resolving shoulder dystocia. As a result of Lesleys work, the method of axillary traction for managing shoulder dystocia was reviewed by the Obstetric Quality Forum at Counties Manukau Health (CMH) (Auckland New Zealand) in 2009 and a new guideline including axillary traction was approved. As a result of that guideline, axillary traction was also included in the Practical Obstetrical Multi-Professional Training (PROMPT) emergency skills and drills training courses. In our article, Lesley documents her journey in fascinating detail, from the initial research stages through to implementing change and spreading the word. 25.01.2022 We are loving using our Peanut balls at the Family Birth Centre. Using a peanut ball can help bub navigate the pelvic outlet during labour. There is good resear...ch around women using a peanut ball during labour and having a reduced length of first and second second stage labour, and reduced incidence of caesarean sections. Most women who use the peanut ball during labour have found it helpful to rotate a posterior baby. Our midwives have made an educational poster showing different positions that might be helpful. 23.01.2022 A reminder that, earlier this year, expert researchers at Oxford Brookes University brought together research on childbirth, birth setting and the use of water ...immersion in labour and shared key points which highlighted the safety and benefits of the use of water for labour and birth, during and beyond the coronavirus pandemic. Their points included that: Water immersion for healthy women is associated with a number of beneficial maternal outcomes with no known adverse risks to the neonate. COVID-19 is not a waterborne virus, therefore, the water environment dilutes respiratory droplet and faecal contamination potential. In the Coronavirus context, water immersion presents a lower risk of contamination risk for midwives compared with bed birth because it promotes the use of social distancing without interrupting normal midwifery care. Burns et al (2020) The authors also noted that: "The birthing pool environment presents a natural barrier between the woman and her midwife. Supporting women in the water reduces droplet, aerosol and faecal contamination, presenting a low-risk transmission activity for the Coronavirus. Water immersion for healthy women is associated with a number of beneficial maternal outcomes with no known adverse risks to the neonate. For primiparous women, birthing in midwifery-led settings (AMU/FMU or home) water immersion reduces transfer rate with the greatest benefit seen at FMU. Water immersion for labour and/or birth should be supported and encouraged as an effective method of analgesia." Burns E, Feeley C, Venderlaan J et al (2020). 21.01.2022 The last 48 hours have been wild, to say the least. This sweet client of mine, who also happens to be my best friend was scheduled for a C-Section at 8:30 yest...erday morning. I had a client go into labor the night before who happened to be delivering at the same hospital. I was still there with her when morning rolled around, and she delivered her baby 68 minutes (literally! & I'll be sharing more from hers later) before the other mama delivered her baby via Cesarean. Talk about a whirlwind. 2.5 years ago, this friend had an extremely traumatic twin birth at this very hospital. One twin born vaginally, one by C-Section, and she was left with PTSD from that and a NICU stay plus an inverted T incision that she knew would prevent her from safely laboring ever again. When she got pregnant with this fourth baby, she knew that she had to make this a healing experience for herself and she did just that. Aside from a few little nuances, things went just as she wanted. I watched OB's, nurses, anesthesiologists, etc., all treat her as every mother should be treated. She was met with gentle words, respect, and kindness that should always be present in a provider - patient relationship. Her mother and I sat by her side as she welcomed her sweet boy, Adler Henry into the world at 37 weeks. Don't be afraid to speak up and advocate for what you want your birth to look like, and be. Even with a Cesarean, this mama was able to have: -Baby gently being brought out while she watched (so cool!) -Delayed cord clamping, weight, and measurement -Immediate Skin to Skin -Free arms, and EKG leads on her back/sides/shoulders -Nursing while still on the table being closed up -Pictures (though I had to shoot through the window in the drape) of baby being born. The trauma from her previous experience will probably never fully disappear, but to see her have such a healing and peaceful birth this time around was so fulfilling. 21.01.2022 I've been rocking and cuddling my baby to sleep since that day he was born. I mentioned this to a nurse at my clinic recently. She said that she did the same th...ing with her son, and everyone warned her that she would still have a teenager sleeping in her bed. And then she told me that, a few weeks ago, her teenage son came home from school very upset. He didn't want to talk, and just went to his room and listened to his music, typed on his phone and cried. The mother gave her son space, and night time came and she went to bed. Just as she was about to turn off her side light, the door opened, and her 15 year old padded into the room. He climbed into the bed with her, laid his head on her shoulder and cried. He told her all about the girl that broke his heart, all about the friends who laughed, all about the stresses of being him. She told him about her first broken heart, about friends who'd been cruel, and told him she understands. They talked in the dark for hours, until he feel asleep in her bed. Still sad but relieved after their talk. 'So you see' she said to me, when she finished the story. 'I was so scared that I wound wind up with a teenager who would 'need me' at night, that I never stopped to consider how beautiful that would really be.' 18.01.2022 RESPECTING THE PHYSIOLOGY of childbirth should be a priority for all midwives, student midwives, doulas, obstetricians - ALL birth workers. Whatever the mode... of birth - its as important before, during and after a Caesarean birth as it is with an assisted birth or normal, vaginal birth. Look at this image - it represents a wonderful humanised birth experience where the baby meets her parents and the first union is special. A private loving space. Think about oxytocin! The midwives and doctors, anaesthetists, nurses have facilitated this moment - does this happen where you work? A POSITIVE childbirth experience matters to the mother, her partner and her baby both in the short and long-term. Weve just finished #birthtraumaawarenessweek so we know the psychological reasons. But there are physical and physiological reasons too. So, things to consider to optimise physiological processes: Share any knowledge you have (and do the research if you dont know it) with those in your care, about the benefits of maximising physiology. Listen to what the woman/birthing person wants.. Help her to write a birth plan documenting the things she would like - there are some fabulous freely downloadable stickers available in @milli.hill #ThePositivBirthBook available from @pinterandmartin website. Check out the clinical guidelines in your maternity service - do they mention the importance of optimal cord clamping, skin to skin in theatre when possible, early breastfeeding and zero separation of mother and baby? Are the teams involved in performing CS birth respectful of the couples privacy and dignity? This includes asking non-necessary personnel present in the room to step outside (kindly), promoting quietness and if the birthing couple want music played - helping that to happen. Any other tips? Moaning and making lower toned noises is very effective in reliving pain in labor, this keeps the jaw loose and relaxed, which is direct relation to a loose and relaxed perineum. Making noises in labor is no different than making sounds while participating in sports or sex (the same noises that often gets baby in gets baby out), so surround yourself with people that allow you to be primal and make you feel safe and comfortable. @lela_partohumanizado
15.01.2022 LEARNING THE STAGES OF LABOUR DURING A CALMBIRTH WORKSHOP Have you thought about the stages of labour and how you can work through each stage to your advantag...e? The first stage of labour begins when your baby has their head laying on the pelvic brim, ready to be born. The cervix can be tightly closed at this point and sealed with the mucus plug. The sac or membranes surrounding your baby are intact, filled with amniotic fluid, a warm pool for the baby to float in. Leading up to the end of your pregnancy your body secretes hormones to soften and ripen the cervix, preparing it to open for labour to begin. During this stage it is important to bring awareness to your breathing, focusing on the inhale and exhale, relaxing your body as baby moves down and dilation occurs. Moving through labour can be like climbing a mountain, each part takes time, with breaks, rehydration, refuelling with food and reassurance. When moving into what is called Transition there can be a peak, change of mood and some can feel the struggle to go on. It is here the breath becomes important along with acupressure, touch and visualisation. It is the last bit of the mountain that requires all your stamina and courage to move onto the summit to birth. After this stage, you move into the second stage when your cervix is fully dilated and your babys head is moving through the birth canal. Here the baby will rotate, placing pressure on the vagina to open, and your baby will begin to be born. At this stage, the baby is still receiving oxygen from the placenta via the umbilical cord. Ensuring your breathing is utilised throughout each stage of labour, holding your breath for great lengths of time as the sensations change will slow or stop the oxygen going to you and your baby, this is a crucial time to ensure they have enough air. This home schooling mom had two previous cesarea...n sections when she found out she was pregnant with Di-Di Twins. Her Orange County midwife @bornwithanangell_midwife referred her to @birthinginstincts as pretty much her only option for a trial of labor. She really didnt want another cesarean and needless to say there was not much support for her coming from the hospital based world. Early this morning, at 37 weeks, she and her husband and two sons welcomed these twins at home just before dawn. Baby boy A was breech. Baby girl B came head first 35 minutes later. Such an honor. Don't forget a large table...spoon of cod liver oil as a great source of omega 3 fat. It is also loaded with vitamin a and vitamin d. Fats should be consumed with protein. If you are having difficulty digesting fats you may need some digestive supports. Remember we don't get fat from eating fat. We get fat from the overconsumption of carbohydrates.
