25.01.2022 TENDINOPATHY - REACTIVE This is an image that I have seen plenty throughout my studies and I now use it to educate my clients about tendinopathy and its stages. Different stages of tendinopathy require different levels of activity for rehabilitation. Today well talk about the reactive tendon. A good example of a reactive tendinopathy is me returning to Muay Thai classes. After years of no professional training and returning, I presented with bilateral triceps reactive tendin...opathy the next day. This was likely due to the pyramid style push-ups during the warm up (starting at 30 and reducing by 5 until we reached 0). At first I thought it was just the usual muscle soreness but my triceps became insanely stiff and I noticed my distal tricep become swollen. This was when I realised my symptoms were more in line with tendinopathy being: - Thickened tendon - Stiffness with range - Worse with prolonged rest - Minimal improvement with heat It was also in line with my history: - Spontaneous return to high level activity - Repeatitive compressive overload (push ups) TREATMENT - Rest: reduce the load to allow for the tendon to recover. The reason for the tendon thickening is to reduce the stress on the tendon. - Isometric muscle contractions - Pain medication if required A reactive tendon, if managed properly will return to being a normal tendon in a week. Note that tendinopathy is a non-inflammatory condition. 24.01.2022 KNEE OSTEOARTHRITIS. The most common lower limb joint to have OA is the knee. OA is a joint degenerative condition, more commonly known as joint "wear and tear" or "bone on bone". This can cause symptoms such as pain, stiffness, swelling and/or reduced joint range/flexibility. Degenerative joint changes are quite normal and is part of ageing. I will likely have some OA changes if you took an X-ray of me. It really only becomes an issue once it becomes painful and limits your ...function. There are 4 pillars to OA treatment: 1. Improve lower limb strength 2. Maintain/optimize joint mobility 3. Maintain/optimize muscle length 4. Weight management to reduce overall force travelling through the joint. These are my goals as a physio. I also encourage and explore exercise modalities with my client. I want to ensure that daily exercise becomes part of their normal day and not a chore. 23.01.2022 CULTURE AND PERSPECTIVE Being of Vietnamese background, I have noticed many cultural differences in terms of what is expected from health care. In Vietnam, people do not usually see a physiotherapist as their first point of contact. They will see a doctor - nothing wrong with that at all, but the expect a magic pill to save them for a musculoskeletal problem. This could also be due to a lack of knowledge of physiotherapy as a profession.... I am sure this is not only in Vietnam and this magic pill perspective exists everywhere. As a physiotherapist, I like to thoroughly explain and make sure the client/patient understand that medication will not solve this musculoskeletal dominant presentation and that they commit to the exercises because that is the only way to get them better. Of course, it is all dependent on what the issue is. I will definitely refer on if the presentation was out of my scope of practice. Sometimes you need to put in the work to get better. This could be from your desk setup (ergonomics) which in turn affects your neck position or the amount of support your chair provides which could create back pain. ... 13.01.2022 FLEXIBILITY. . . People tend to forgo flexibility for strength or have flexibility lower on their priority. .... I was like this too but ever since I started stretching regularly and improving my flexibility, I just feel great. It helped me with a lower back pain, improved my kick height in muay thai and jumps/sprints in basketball. . . 10.01.2022 PROGRESS POST . . . An elite teenage swimmer presents with an insidious shoulder flicking without any triggers. She has no pain with movement or resisted muscle tests. She tested negative for shoulder ligament instability and rotator cuff specific tests.... . . . This was tricky as it was hard to identify what exactly was causing the problem. After a long assessment, we determined the flicking (subluxation) was due to inadequate motor control of the rotator cuff - the biggest indicator was she had NO shoulder flicking at all in sitting. . . . These videos show the initial presentation and our re-assessment 5 weeks after! Safe to say we were all excited to see this progress . 09.01.2022 Having proper footwear can reduce risk of lower limb non-contact injuries. John Wannops study in 2012 found a relationship between roational and translational traction and rate of injury. Increased rotational and translational traction increase ankle and knee joint loading in both frontal and transverse planes. The take home message being - ensure you have shoes with proper traction and grip in all directions. ... The clip shows the types of movements require in most sports. The cutting motion and quick acceleration/deceleration. Freuqent jumping is more attributed to basketball than other sports. It is obvious to see that without proper footwear traction, this movement can be detrimental to the ankle and knee. 08.01.2022 New evidence with swelling management - PEACE and LOVE Recent evidence suggests NOT to ice a swollen area due to it limiting inflammatory mediators which in turn causes sub-optimal healing during the mid and late stages of recovery. Although this sounds good and well - there are some limitations to the article. The biggest one being no description of the population that was observed. Missing essential data includes but is not limited to:... - age group - area of injury - severity of swelling - follow-up sessions and re-assessment I have tried this method on myself with minor injuries from basketball - jarred fingers (minor ligament sprains). I have not found any difference between icing and not icing in terms of: - time to recover - strength deficiency - pain levels during the acute and late stage of recovery However, for larger injuries where swelling is causing intense pain and restricts normal movement/function of the joint - I will continue to use ice as prolonged swelling can cause compensatory strategies and cause further issues. 08.01.2022 Continuing on the notion of my previous post - it is a shame that people do not push their bodies and do not find what they are actually capable of. When we think of fitness our first thought is the gym. Buff and toned with muscles that pop out. Or you may think of running - sprints and long distance marathons. However, I also relate fitness to mobility, movement and dexterity. It is far more important to find an actvity that you enjoy and will commit to. ... This is my first session at this gymnastics gym learning how to do a back flip. I was quite surprised at how fast I was able to progress. Although, I do not have anything specific I am training towards, I am just interested in what my body can do. I encourage you to find an activity you enjoy and push yourself. 04.01.2022 TENDINOPATHY - DYSREPAIR/DEGENERATION. Continuing with the tendon spectrum, we will move on to the tendon dysrepair and degeneration. Sticking to the same case study as previously with my bilateral reactive triceps tendinopathies. Tendon dysrepair occurs when you do not adequately offload the reactive tendon. This can cause further pain and stiffness in the tendon. Mainly being - if I pushed through the pain and continued training or did not rest enough (~1 week) this would h...ave progressed to tendon dysrepair. The degenerative tendon is long term repetitive loading without adequate rest. At this stage the tendon changes is irreversible BUT we can improve tendon function! TREATMENT for dysrepair and degenerative tendons include progressive resistive exercises. Namely, eccentric exercises to load the tendon and progressing to improve client function and reach their individual goals. Biggest takeaway from this is that even though it may be termed degenerative you can still improve tendon function!

Common in sports that involve a lot of running, stop-start motions or high impact activities. TREATMENT... - Rest: now knowing that it is an overuse injury, rest is essential. However, this is active rest. 02.01.2022 Shoulder Special Tests A very recent article released in this month questions the validity of shoulder orthopaedic tests. These tests were supposedly created to isolate individual muscles in the shoulder to identify the source of pain. Salamh and Lewis article highlights very interesting points. The biggest being that these shoulder special tests cannot isolate certain structures and can only be considered a pain provocation test.... It was confirmed that there were up to 9 other muscles active when testing empty and full can tests. This test was meant to isolate supraspinatus. Personally, I have been using these tests as a general pain provocation test rather than using it to identify a specific structure. What are your thoughts?
