23.01.2022 It depends..... The most important answer to whether any particular intervention is useful in rehab. Everything that is applied must have purpose and must have reasoning behind it. Very rarely are there good and bad exercises, however the most money is made in rehab where your particular intervention of choice is targeting a specific quantifiable outcome. There are too many factors to count that need to be considered. The number 1 principle is to tailor everything to the ...individual presenting in front of you. 21.01.2022 Sometimes I find it crazy how the world works and will put you in just the right places at just the right time. In 2018 I reached out to Karl and Lachy to take me under their wing as I was a young physio who was hungry to learn more about the strength and conditioning world. In 2019 I finished my mentorship and wanted more, so I signed up as part of the Athlete a Development Program which not only improved my sporting performance but through some of the best minds in the indu...stry took my knowledge to another level. At the beginning of 2020, Karl approached me and offered me some part time employment to help fill a current void in their Physiotherapy program. Naturally I accepted... Now it is the place that I call home. AA has one of the most incredible communities that the private sector has to offer. 20.01.2022 Non-operative ACL rehabilitation pathways are becoming increasingly popular, with emerging evidence surrounding the efficacy of this particular pathway of practice it is easy to shift the pendulum dial too far down this lane and forget that context is critical. - Potential non-operative copers have to meet a strict set of guidelines and the issue with being a coper is that you are a coper until you’re not. We only know in retrospect whether you failed this method and the con...sequences of secondary speed wobbles can be significant on the knee. - I encourage all clinicians to be open minded regarding their preferred choice of management. Educate your patients into making empowered decisions. High quality intense rehab is the cornerstone of all ACL rehab regardless of whether surgical intervention is included or not. When it is excluded you can make an argument that the quality and intensity of your rehab needs to be even greater. - The Physiotherapy community is currently up in arms debating about whether surgery is required or not, we do this despite an overarching issue that as a profession, we do ACL rehab incredibly poorly. Rehabilitation post ACL injury is primarily advanced strength and conditioning focused to which majority of physios have poor knowledge on. - If you injure your ACL, make sure you take the appropriate steps. 19.01.2022 FEMALE TRIAD - Whilst levels of organised sport are declining due to the current COVID laws, self training appears to be increasing as a method of keeping both sane and on track. ... - The formula is simple right? I’ve got heaps more downtime to train so the more I do, the better shape I will be in when normality resumes? Not necessarily. Females this, in particular is the time where you should be discussing your training and nutrition plans with accredited specialists in the field. - The female athlete triad is a spectrum of pathology characterised by one or more of the traits detailed in the image. - The crux of the condition is that low energy availability -> menstrual dysfunction -> reduced oestrogen -> poor outcomes for bone and endothelial health. - One common sign / symptom when this is present is the absence of menses (your period) however it is important to note that this is NOT always the case. Just because your cycle is functioning regularly does not mean this condition is absent. - Other personal items to look out for include: 1. Slower recovery times from training 2. Increased and persisting muscle soreness from training 3. Bone sensitivity (history of bone stress injuries is a sign to watch out for). - Don’t try and go it alone over the next 6 months. Hormone health is so critical and it a very specific and individualised approach needs to be taken for female athlete training. - If you think calorie restriction and high volumes of cardio are the way to ‘keep the weight off’ or ‘not get bulky’ over the next 6 months im telling you now it won’t end well. - Coaches - if you don’t consider this pyramid when programming for your female athletes, you have a lot of time on your hands now so start self educating!! - If you need help with getting hooked up with specialists in the field to get your plan set up. 18.01.2022 YOUTH PHYSICAL DEVELOPMENT - Recently after watching ‘The Last Dance’ documentary it had me thinking about the competitiveness of Michael Jordan, his will to win and how might this attitude trickle down and have an effect on the youth athletes of today. ... - Current models of youth physical development assist in us understanding how youth athletes best physically respond to training stimuli however I feel there is far too little an emphasis on the behavioural patterns that are developed in these stages of life. - For the vast majority of athletes that go through schooling systems or sporting academy / squad structures the sad reality is they won’t make the top level and become a professional athlete. For many of these kids once they leave these realms there exists a dissociation between themselves and their ‘why’ behind their training and thus fall away from sport and physical activity. - If all we associate to these kids is the link between training hard and playing sport professionally we won’t set the foundations up for a lifetime of healthy living. - Rather we should be creating positive environments where the athlete can independently drive their own vehicle, whilst we sit in the passenger seat and guide them along the way. Creating an atmosphere where adolescents are empowered rather than forced, inspired rather than pressured, filled with enjoyment rather than purely competitiveness. We have been challenged on our belief system in recent times about what the effects of it are and whether it is low value care or not. - My personal perspective is I liken it the incorporation of ‘junk’ or ‘low nutrient’ food in our lifestyles. If all you consumed was junk food, you wouldn’t be particularly healthy and similarly if you prioritise manual therapy over the core foundations of a healthy lifestyl...e such as exercise, a good diet and sleep routines you will have suboptimal health. 11.01.2022 My physio was a bit old school which was fine initially I guess... but I needed something that was more sport-specific if I was going to have the confidence to... go back to sport. When Jordy came to us six months into an ACL rupture, he was low on confidence. Any person regardless of qualification can throw together a mish-mash of movement on an aimless 45s/15s time span. 07.01.2022 Pyramid of athlete identity - Health -> Connections -> Sport -> Athletic Properties... - When training somebody in athletic development, it is important to understand that whilst we are aiming for improved physical preparation performance markers - that there are far more important factors at play that create a successful athlete. These must all be considered (not necessarily always addressed) by a coach to ensure they are pushing the athlete on the path of success. - Let’s explore each section of the pyramid: - HUMAN: Bottom and the biggest foundation of the pyramid is being human. This relates to the health of an individual. Without a solid foundation of health amongst different pillars (physical, mental, spiritual - just to name a few), all other aspects of the pyramid become obsolete. If you cannot win the battle against yourself you stand no chance against opposition. - PERSON: Whilst being human forms the foundation of identity being a ‘person’ identifies that each individual carries a story - with that story comes individual traits, likes, dislikes and a uniqueness that creates connection. Athletes need to connect with their fellow athletes as well as their coaches to ensure that a successful career can be reached. We must never forget the social aspect that sport and training brings for people. - SPORTSMAN: Athletes love their sport, realising this allows us to create an understanding that the training they are doing will translate to sports performance. 03.01.2022 If you’re an athlete, you don’t have access to your gym anymore and you’re thinking your chance to improve yourself is over, I’ll counter that thought with the argument that this is the greatest opportunity you’ll ever get to hit the reset button. - The fundamentals of human physiology take time to change, hell they take a lot of time to change. Whether that be Vo2 max; maximal sprint speed; aerobic efficiency; maximal strength; force-velocity profiling - the list goes on fo...r days. The ultimate conundrum in team sport environment which hinders this development is the fact that your sport has to take priority. Being the best athlete won’t make you better at the fundamental skills that are required in your sport. This is where concurrent training models arise from - how can we best jam in as much from as many different training variables as possible so you can be the best athlete you can, whilst also being the best sportsman you can. - This is where the opportunity lies. You no longer are bound by the commitments of your sport - you have time on your side for the first time in your career. Now is the time to be maximising the profits which can be made from improving your physiology to reach its actual potential, not the capped ceiling that the life of team sport limits you to. The potential that only people who’s sport actually is to be the physically best athlete ever properly achieve. - But don’t I still need to be focusing on my sport? Here’s where opportunity number 2 arises. The chance to have the time to work on the fundamental skills that you’ve always wanted to improve on but never had the time to dedicate to the motor learning that it requires. Always wanted to be a better passer or shooter?
