The seatbelt is a bit loose & the top tether is not anchored. All forward facing car seats are designed with a top tether strap that attaches to an anchor point in the car. This vastly improves performance but is often forgotten by parents. Some older vehicles are not equipped with an anchor point, and all car seats must pass testing without the tether in use. Even with the top tether anchored, a toddler's head can still be thrown forward violently enough to permanently damage the spinal column when forward facing.
