VITAMINS B and STROKE PREVENTION Vitamin therapy with folic acid (B9), B6, and B12 can lower levels of total homocysteine (tHcy) which is a strong risk factor for vascular disease tHcy. This is particularly true for cardioembolic stroke; high levels of tHcy quadruple the risk of stroke in atrial fibrillation now an epidemic in those over 65. Folate fortification exists in most civilised countries and Vitamin B12 is then what lowers the homocysteine. Metabolic B12 deficiency was present in 30% of Canadian stroke or transient ischemic attack patients over age 71 in 2006. It is much more common in other countries with a higher prevalence of vegetarianism. But, it has just been found the usual B12, cyanocobalamin is not the best form of B12. We should be using B vitamins to prevent stroke, but we should probably be using methylcobalamin or hydroxocobalamin, not cyanocobalamin. DIETARY SUPPLEMENTS STILL TAINTED Dietary supplements continue to contain potentially dangerous ingredients that are rarely declared on the label, according to an analysis in JAMA Network Open. Most of the products were marketed for sexual enhancement, followed by weight loss and muscle building. (Well no surprises there). The effect was substantial: in men, a standard deviation decrease in IQ was roughly equivalent to a 30% increase in risk of death from all causes and, in women, about half that, but, without information on health-related behaviours such as smoking, exercise, and diet, the underlying mechanisms can’t be investigated any further. SICKER AT THE WEEKEND People admitted to hospital as an emergency at the weekend have a higher mortality than weekday emergency admissions. Low levels of staffing and lack of senior doctors are sometimes blamed. But an analysis of data collected over four years from a UK university teaching hospital suggests that the explanation is simply that patients admitted at the weekend are sicker. The authors suggest the underlying cause of the weekend effect is more likely to lie in the absence of community services than in substandard hospital care. See more
