15.01.2022 Derangement syndrome Achieving partial reduction in lumbar derangement syndrome is a common challenge. Before going lateral or combining planes do you routinely ensure you... Achieve end range - this can be further than you initially think and may require some alternatives (elevated hand or leg position) to achieve. ... Ensure enough reps were completed - sometimes high volume is required to achieve reduction. Doing 5 - 10 - 20 reps every 1 - 2 - 3 hours? Depends on the patient in front of you. Sometimes more is more. Apply enough force - using the force progressions appropriately can make a big difference. If force is required in the clinic to effect symptomatic change do you teach self generated or partner force progressions for home? Manage postural loads - sometimes the exercise (technique/dosage/force) is right but the aggravating factors are not being addressed thoroughly enough. Changing sitting/standing/lying posture or reducing an aggravating activity may be what’s needed. Happy reducing! #McKenzieMDT #derangementsyndrome #reduce

The history can give some clues to the potential presence of derangement syndrome. This variability in the history could be expressed in terms of:... - ‘Good’ and ‘bad’ days - The use of the term ‘sometimes’ in relation to aggravating or easing factors - Change in location of symptoms - Change in intensity of symptoms - Varied levels of stiffness with motion (eg. sometimes when I stand up from sitting it feels stiff - but not always) - Relieving postures/stretches/movements being inconsistently effective Does symptomatic or mechanical variability in the history change your approach to the physical examination? #McKenzieMDT #derangementsyndrome

All patients received MDT intervention and those patients with ongoing cognitive drivers also had additional graded activity and / or graded exposure. There was no clinically relevant benefit from the addition of graded activity/exposure.

05.10.2021 Meet a member(s): Mary and Peter Knapman Tell us a bit about your career and how you ended up pursuing training in MDT. We both did our first McKenzie Course in Lismore in 1987 with Robin McKenzie, and loved it from the start. He was such an entertaining Educator, and we both found we could go back to work on Monday and use the assessment and treatment immediately. That inspired us to continue and follow up with Part B also with Robin, and then on to the Credentialling exa...m. What does your current work look like? We currently run a private practice in Parramatta, with 6 other MDT inspired Physio's. We treat a mix of patients, and use the MDT for all our patients. We find it provides a great system for assessing and treating. It's also a nice way for younger Physio's to learn great Clinical reasoning. As older Physios it has given us great longevity in our careers, and introduced us to other like minded Physios from all over the world. What's impacting your clinical practice most at the moment? Currently, more posture related problems as a result of people working from home with Covid, and more psychosocial issues- as we are currently in lockdown again in Sydney. What's unique about being an MDT trained practice owner? Having a group systematic approach to the way we assess so that we ask our clinician's "what is the classification of that patient". It helps then to work out the treatment pathway, once that's clarified. What advice would you give to physios starting out on their career? Study the MDT method! As far as courses go we think it's still the best investment we made in our Professional learning. #McKenzieMDT
