Many women think of testosterone as a male hormone but you will be surprised to know that women produce three to four times more testosterone then oestrogen in the ovaries! Testosterone levels reach their peak in women during their 20s and decline with age. During menopause women have only one quarter of the testosterone compared to their peak decades. ... Testosterone increases LIBIDO, SEXUAL AROUSAL and ORGASM increasing DOPAMINE levels in the brain. Menopausal and post menopausal women may experience low libido and pleasure due to decline in testosterone. Women who have their ovaries removed (surgical menopause) notice a significant change in their libido due to sudden deficiency of hormones. Many studies show that testosterone alone may not be effective in the absence of oestrogen. The synergistic effect of oestrogen plus testosterone boosts sexual desire and pleasure. It’s important to be aware of any conflicting people, situations and surroundings that may be contributing to chronic stress, emptiness, unhappiness or worry. Once you become aware of these triggers, you can identify and take appropriate st...eps to address the root cause of the problems. Try to surround yourself with positive people, people who you connect with and who bring joy to your life. Follow your passions and try new things. Declutter your home, bedroom and work spaces to optimise mental clarity. Play your favourite music. Sing in the shower. Buy fresh flowers. Decorate your home with objects that lift you up. Create an environment which is uplifting and pleasant to your visual, auditory and olfactory senses. Make a conscious effort to address the environmental triggers in your life and you will feel more balanced. It’s also important to avoid ENDOCRINE DISTRUPTERS chemicals which interfere with hormone function by altering their levels. 23.01.2022 LOW LIBIDO is the reduced desire for sexual activity. It is one of the most commonest symptom of the perimenopause/menopause but also the one symptom women rarely complain about until asked. Unfortunately, the majority of doctors will not ask about low libido, hence, women continue to suffer in silence. Women find that low libido has a detrimental impact on their relationship. There is also a great deal of shame associated with talking about sexual desire or be it the ...lack of. Many women during or post menopause, are at the time in their lives, where they maybe looking for new relationships low libido is a significant issue which needs to be addressed for them to continue enjoying healthy relationships. Clinically, low libido is also part of the Hypoactive Sexual Desire Disorder. 20.01.2022 GSM - Genitourinary Syndrome of Menopause is a term used to describe the changes in vagina and vulva, bladder and urethra, and pelvic floor tissues due to the decline in oestrogen levels. This was previously called VVAvulvovaginal atrophy. Oestrogen receptors are found in the entire pelvic floor and it’s organs; and play an important role to maintain healthy tissue, lubrication and sexual function. Menopausal symptoms such as hot flushes will settle with time but the GS...M persist or even worsen with time. Studies have shown that more than 50% of women do not report vaginal dryness to their healthcare professionals and less then 4% of affected women use proven vaginal treatments for relief. The decline in oestrogen causes genital tissues to become fragile, dry and thin due to loss of elastin, collagen and hyaluronic acid. This causes vaginal dryness, burning, itching and bleeding. Vaginal discharge and recurrent thrush also becomes common due to changes in vaginal pH and microflora. Bladder symptoms such as going to the toilet more often, urgent need to go and recurrent urinary tract infections also become common. Dyspareunia is painful or uncomfortable sexual intercourse which is commonly experienced due to vaginal dryness and tissue changes in the genital tract. This makes women avoid sexual intercourse as it’s not pleasurable. . . . What can be done about it? Hormonal treatments: Vaginal oestrogens-creams, tablets or ring Vaginal androgens such as DHEA suppository Non-hormonal treatments: Vaginal laser therapy Vaginal moisturisers Vaginal lubricants Vitamin E orally or locally Pelvic floor exercises NOTE: Contraceptive pill causes vaginal dryness. 19.01.2022 OESTROGEN is the hormone that makes us LOOK like women and FEEL feminine. It’s the hormone that peaks just before ovulation and makes us feel sexually attractive and increases our sexual desire towards our partners. . The decline in oestrogen during the hormone transition results in low sexual desire and motivation for sex. This is particularly dramatic, if you have had your ovaries removed. .... Post menopausal women have almost non-existent oestrogen levels and can have distressing lack of sexual desire. . Low libido coupled with painful sexual intercourse due to vaginal dryness make the experience unpleasureble and leads to fear of intimacy. . 16.01.2022 SEX & DRUGS I am starting the low libido series with MEDICATION because almost every woman I see in my clinic for menopause is on some form of antidepressant. This is primarily because mood disturbance is a predominant symptom of the perimenopause and most doctors feel comfortable prescribing antidepressants rather then hormone replacement therapy. Antidepressants: The most commonly prescribed drugs for depression, anxiety and menopause symptoms is a Selective Serotonin Re...uptake Inhibitor (SSRIs) such as fluoxetine, sertraline and paroxetine. Venlafaxine and mirtazapine are other commonly prescribed antidepressants. All of these affect female sexual function including low libido, arousal and orgasm. Anti-hypertensives: Blood pressure lowering medication such as diuretics can cause low libido. Beta blockers like propranolol given commonly to treat anxiety, panic attacks and palpitations also cause low libido. Anti-epileptics: Gabapentin and topiramate are some of the drugs to treat epilepsy but also used for chronic nerve pain and migraine prevention respectively. Both are associated with orgasmic dysfunction and reduce libido in women. Anti-psychotics: Certain mental health illnesses like Bipolar disorder and schizophrenia are prescribed drugs like olanzapine which decrease sexual desire and impairs arousal & orgasm. Contraceptives: Oral contraceptives reduce circulating free testosterone and suppress ovulation, which can result in low libido. Depot contraceptive injections can cause low libido, vaginal dryness and painful sex. . . . This is not an exhaustive list of drugs causing low libido. I encourage you to review your medication with your doctor and try to switch to an alternative drug or reduce dose. . . . . However, with today’s fast-paced lives, this is not always possible. Sometimes, a boost is needed to achieve optimal physical, emotinal and mental wellbeing. I normally, recommend the following supplements and if necessary, refer to a nutritionist for a personalised nutrient-dense meal plan: Magnesium B-vitamin...s Omega-3 Vitamin D Optional: Reversetrol, CoQ10, Calcium D-Glucorate etc The above supplements help with liver detoxification, reduce inflammation, balance hormones and neurotransmitters, which support heart health, maintain healthy cholesterol ratio, strengthen muscle & bone, improve immune system, mood and brain function. HERBS particularly Adaptogens such as Ashwaganda, Rodiola, Ginseng, Holy Basil etc help with stress modulation and improve sleep, mood, energy and hormone balance. Garden Sage, Chaste Berry, Black Cohosh, Red clover , Traditional Chinese Medicine etc are commonly found in over the counter Menopause supplements. These have some evidence in short-term menopausal symptom relief but do not provide long-term prevention against osteoporosis, heart disease, dementia, diabetes or depression. 12.01.2022 FOOD PILLAR of HORMONE WELLNESS ~ The food we consume communicates directly with the cells and organs of our body. A nutrient-poor diet with an excess of sugar/transfats/refined carbs creates energy imbalances leading to excess weight and disease promotion such as diabetes, dementia, heart disease and mental health disorders. Nutrient-dense food full of vitamins, minerals and anti-oxidants supports optimal bodily functions and prevent againts chronic disease. A WHOLE FOOD..., PLANT BASED diet which is similar to the Mediterranean diet, has shown to protect against chronic illnesses including Alzheimer’s disease and Heart disease. GREEN superfood such as cruciferous vegetables ( brocolli, kale, bok choy, brussel spouts and similar green leafy vegetables) helps Oestrogen metabolism and Liver detoxification. Eat good quality meat in moderation including grass fed beef, organic chicken and plenty wild caught fish. Avoid processed meats. Don't forget eggs! Spices and herbs such as ginger, garlic, turmeric, cayenne pepper, cinnamon, oregano, basil, sage, rosemary and parsley add flavour and have anti-inflammatory effects. Plenty healthy FATS such as avocado, olive oil, nuts, olives and seeds. The movement does not have to be long, strenuous gym sessions the purpose is to keep active by doing something you love and enjoy. The goal of exercise is not just weight loss. During peri-menopause, it’s important to adapt exercise regimes to rehabilatative styles, such as Yoga, Pilates, low intensity interval training and/or resistance training; ideally combining a ...variety of styles during the week. Simple brisk walking is equally effective. High intensity training workouts and strenuous aerobic/cardio exercises such as long distance running may exacerbate symptoms of fatigue, stress and mood disturbance as these exercises stimulate cortisol and put extra burden on your already burnt out adrenals. If you are feeling exhausted and want to sleep after your workout that’s a sign you need to change to alternative workouts. Try to sync your exercise regime with your menstrual cycle more rehabilitative exercise regimes like yoga, Pilates etc 7-10 days prior to their period to avoid/exacerbating symptoms of Premenstrual tension. If cycle is irregular, then keep movement low impact. Remember to be kind to yourself and take a gentle approach to exercise.
