22.01.2022 MELBOURNE FIRST KEYHOLE TREATMENT FOR BENIGN BREAST LUMPS An innovative new procedure that removes benign breast lumps without the need for open surgery or general anaesthetic has been introduced at Mitcham Private Hospital. The procedure, a first in Victoria, is called vacuum assisted excision (VAE). It uses a small needle under ultrasound guidance to ‘suck’ the breast lesion out through a tiny stab incision.... It can be performed in an out-patient setting and is suitable for the removal of non-cancerous lumps such as fibroadenoma and recurrent cysts up to 2.5cm. It is also useful in establishing diagnosis of indeterminant breast lumps by combining needle biopsy and excision into a one-step keyhole procedure. Traditionally breast lumps are removed under general anaesthetic with an ‘open’ incision large enough to remove the mass. VAE is performed using local anaesthetic and light sedation if required. Breast and endocrine surgeon, Mr Michael Law has introduced the procedure at Ramsay Health Care’s Mitcham Private Hospital, after a formal clinical trial lasting 12 months. VAE is commonly used in Asia and America but hasn’t been widely used in Australia, until now. Vacuum assisted breast biopsy is a one step process that means patients don’t need to have a biopsy and then return to have a lesion removed in theatre, he said. The procedure results in a 3-4 mm ‘stab’ incision scar rather than an open incision that requires multiple stitches. This may result in a faster recovery time, less scaring and a reduced need for pain killers. The tissue is still taken for analysis like with a normal needle biopsy, but the difference is the entire lesion is removed, increasing the accuracy of diagnosis. The VAE keyhole excision takes about 15 minutes. This is an excellent alternative to surgery for the removal of small, benign breast lumps, said Mr Law. No one wants to have a general anaesthetic in theatre if they don’t have to, and it’s reassuring to patients this can be done in an out-patient setting or with minimal sedation. Patient Maria Micalizzi had a non-cancerous tumour removed using the VAE keyhole excision. It was the second lump Mrs Micalizzi has had removed from her breast, and she said felt good knowing the biopsy and procedure were performed in one sitting. I felt a lot of anxiety prior to the procedure, but it was great that everything was done on the day. It was all gone in one hit, and for me the recovery time was minimal, she said. Mollie McDonald is an endorsed enrolled nurse at Mitcham Private Hospital’s perinatal mental health unit. Here is her story: My mother was a nurse, so it was something I was always interested in doing. One thing I discovered very early on is that, as a nurse, you are constantly continuing your education. I completed 18 months study at Swinburne University and... I have started studying again to become a Registered Nurse. When I heard about the opportunity to apply for a scholarship for the Mental Health First Aid Course, it was something I was really keen to put my hand up for. The course appealed to me as I work in the Perinatal Mental Health ward and I wanted to not only improve my knowledge of mental health issues but also my confidence with patients. The 12-hour course was run over a number of days and I really got a lot out of it; I’ve already started to implement some of the strategies I learned. It has reaffirmed how important it is to take a holistic approach to dealing with mental health issues. My skills have definitely improved and I am much more confident in my decision making around risk assessment. Doing the course as a group helped me build relationships with people in other parts of the hospital and encouraged us to feel more united as a team. It is definitely something I would recommend to my colleagues. It was great to be given the opportunity to do this course, particularly for those who are fairly new to nursing. I’ll be on the lookout for other Ramsay scholarship opportunities to further improve my skills.

04.01.2022 Our philosophy is ‘people caring for people’ and it’s always wonderful to see examples of our team members looking out for both their patients and each other. Rachel Gale has been a staff development coordinator at Mitcham Private Hospital for four years and is always searching for opportunities for staff to improve their qualifications and knowledge. Here is her story: I successfully applied for a scholarship for 12 staff to take part in a mental health first aid course, wh...ich is one of the many scholarship opportunities Ramsay offers every year. The scholarships offer fantastic opportunities for clinical and non-clinical staff to develop their skills in areas of interest. I really felt the Mental Health First Aid Course would be useful, as it’s important we care for our patients’ mental health as well as their physical health. While we have a mental health unit at Mitcham Private Hospital, both an adult and perinatal unit, In addition to patients in the mental health unit, we discovered that a lot of our patients in the medical department, operating theatres and maternity wards also had some mental health concerns. Completing this course enabled our staff to better deal with this and understand patients from a holistic point of view. The feedback was great; staff felt empowered to better understand our patients and look out for their mental health. They were able to use the new skills in all aspects of general care - from After Hours Managers liaising with patients outside normal hours to those working with perinatal mental health patients and also our surgical staff. I feel very lucky to work at Ramsay, with so many opportunities to develop our skills to enable us to do our job to the best of our ability.

02.01.2022 The Ramsay community has grown stronger by our shared experiences this year. As we approach the end of 2020 and look forward to 2021, our staff are providing personal stories about working on the frontline in Victoria. Their words are inspirational: It was when I was visiting a sick grandparent in hospital that I first realised I wanted to be a nurse. I was in Year 11 at the time and growing up in Benalla in north-east Victoria. I remember watching the nurses go about their ...work and feeling the pull to care for people in the same way. I’ve been a registered nurse for 29 years now, and I love it. I love the people. My first job out of university was in an aged care facility and that instilled a great deal of compassion in me. There’s a lot of stories to be told amongst the elderly and, from a skills perspective, I learned so much from taking care of them. These days, I work mostly in the orthopaedic ward but, during quieter times, we all work in different parts of the hospital. I don’t think there’s a ward I haven’t worked on. As nurses, we are in the trade of caring. If we can help, we want to lend our skills and knowledge. Everyone wants to be part of the team that makes the community as healthy and safe as possible. It’s silly but I still get emotional about how the people we touch really rely on us. We are in such a fortunate position where we get to have a big impact on how well their hospital stay goes. It can be something as simple as a warm smile that makes their worries disappear. We always want to make sure we meet the expectations of our patients, so we measure patient satisfaction through Net Promoter Score surveys, which are conducted independently by a third party. Scores above 50 are considered ‘excellent’, and all Ramsay Health Care hospitals across Victoria were ranked in this tier throughout 2020. This really makes us happy to know that our patients are scoring us highly on the care they receive. 2020 has been challenging for everyone, but we are proud to have answered the call for help when other hospitals and aged care facilities needed help. It was a terrific team spirit. I think the secret to being a good nurse is to come in every shift and try to make someone’s just that little bit brighter.
