10.01.2022 I am a hospital pharmacist in Australia and I review medications of patients that are admitted to hospital to ensure we don’t miss anything. As you all are awar...e COVID-19 is in the forefront of our mind every hour and every day. If you have to be admitted to hospital for COVID-19 or another reason, you will be helped by my colleagues, fellow Doctors, Nurses and Pharmacists where they will take an allergy and medication history on top of their assessment of your health. I am asking you all to contact your family members while we are at home social distancing, to ensure they have an up to date medication list in the event they might have to come to hospital. Every day I speak to patients, that might normally remember their medications, but forget when asked in hospital. Sometimes people forget the name of the medicines or how much they take. It’s hard to remember when put on the spot or when you are unwell and sick. It is okay. But, what can you do to help us? Please, now, spend time to write out an up to date medication list. It can be on a piece of paper or you can use an app such as: NPS Medicinewise, or download and print a hard copy. You can also ask your GP or your pharmacist for a copy or if you need help. A good medication list includes: - Allergies - The medication name (brand or drug name) - The dose - How often and when - Why you take the medicine Eg. Pantoprazole (Somac)20mg daily in the morning for reflux - Which pharmacy you attend or GP you go to? (to make sure we don’t miss anything) - Don’t forget to include herbs and vitamins (some cause drug interactions!) - Is there anybody we can call to ask about your medications if you are unable to let us know for some reason?
