The extensions to our suite have started, which will include the new Lisle office For patients this means entering through our private Hollywood Orthopaedic Group doors that are on the left hand side of the Medical Centre, just for this week. The builders were kind enough to let us all go in and write our names in the cement before it set too.

14.01.2022 Well done Perth! Never have I felt so privileged to live in the most isolated capital city in the World. With our isolation, lack of density and aggressive approach to social distancing our Coronavirus infection rates are impressively low. That being said, my sympathies are extended to the families affected by this tragic pandemic. Personally, this enforced slow down has been used to have some well overdue family time and recharge the batteries! It certainly has made me a...ppreciate how truly lucky I am. With elective surgery due to recommence in limited capacity from Tuesday April 28, I am really excited to get back into Theatre. I would like to extend my thanks to all my patients for their understanding and patience over this difficult period. 14.01.2022 PATELLOFEMORAL INSTABILITY. . . Patellofemoral instability (recurrent kneecap dislocation) is generally due to a range of factors, including bony and soft tissue issues. Bony issues include trochlea dysplasia (or a failure of development of the normal groove in the distal femur that the kneecap runs in), alignment or rotational abnormalities. Soft tissues include generalised ligament laxity, muscular weakness, rupture of medial restraints (MPFL rupture) and an enlongated pate...lla tendon (patella alta). . . After failure of conservative management for recurrent patella (kneecap) dislocations, surgical stabilisation becomes an option. Treatment should be focused on addressing individual abnormalities. . . This particular patient had a combination of trochlea dysplasia, patella alta and MPFL rupture. They underwent lateral release, tibial tuberosity transfer and MPFL reconstruction. From the pre and post-operative images you can see that the patella has been distalised (lowered) 2cm and medialised 1cm. The distal 2cm from the tibial tuberosity osteotomy has been excised and transposed proximally to further aid stability. The gracilis tendon was harvested for MPFL reconstruction, and tunnels for fixation can be seen in the medial aspect of both the patella and distant femur. . . . . . *Legal disclaimer: All patients featured in the Images (photographs and/or videos) have consented in writing to their Images being shared. 12.01.2022 DDH - Developmental dysplasia of the hip. The hip joint is a ball and socket joint, with the femoral head (ball) moving freely within the acetabulum (socket). DDH is a condition of abnormal development of the immature hip, incorporating varying degrees of hip instability and acetabular immaturity. It encompasses a spectrum of disease from mild delay in acetabulum development to frank hip dislocation. In Western Australia, we have a selective screening program, with all b...abies undergoing clinical examination at birth, 6 weeks and 3 months. Babies with an abnormal hip examination, or those with significant risk factors, undergo further investigation (usually an ultrasound, ideally around 6 weeks postpartum). Risk factors for hip dysplasia include: First born children; Females; Breech presentation; Positive family history. 12.01.2022 The incidence of developmental dysplasia of the hip (DDH) appears to be increasing. This may be due to better screening and diagnosis, or reflect a rise in the number of children affected. Currently in WA we can expect approximately 200 children to be diagnosed with hip dysphasia every year. The significance of hip dysplasia is that it is associated with the early development of osteoarthritis. Early recognition and treatment has been shown to decrease the chance of surgical ...intervention and improve outcomes. Treatment options depend on the age of diagnosis and the severity of dysplasia. Some mild dysplasia can be treated with observation alone, while others require a period of abduction bracing. The pictures shown are different bracing options including a Pavlik Harness, Correctio brace and Rhino brace. Children that do not respond to bracing, or those that presents late with severe DDH (or even a dislocated hip) may require surgery. Surgical options range from reducing the hip in theatre (either open or closed) to pelvic osteotomies, where bone cuts are made in the pelvis to improve the coverage of the hip.

Last night was the second education session Mr Ryan Lisle, Mr Nicholas Frost and Mr Andrew Wallis presented to some wonderful Physiotherapists of Perth. 10.01.2022 Knee replacement surgery is major surgery. However, it has proven itself as a safe and reliable way of dealing with end-stage, activity limiting osteoarthritis of the knee. Developed in the late 1960s, total knee arthroplasty (replacement) involves replacing all the worn cartilage surfaces with metal on plastic (polyethylene) bearing surface. In 2016, there were over 52,000 knee replacements performed in Australia. With our aging population, this figure is steadily rising.... The National Joint Replacement Registry (NJRR) shows that for those knee replacements implanted for osteoarthritis, only 8% can be predicted to fail (i.e need revision) after 16 years (as shown on the graph). Put another way, 92% of knee replacements can be expected to last 16 years! Although survivorship is not the sole determinant of a successful outcome, it is a testament to the safety and durability of this procedure. The attached X-ray shows the pre-operative and post-operative appearance of a patient having undergone knee replacement. 06.01.2022 NOT ALL MENISCAL TEARS ARE CREATED EQUALLY. . . . The above arthroscopic images are from a 48 year old patient with a medial meniscal tear and some low grade chondral changes. The pre-resection image (1st image) shows a complex meniscal tear with an unstable flap. The post-resection image (2nd image) shows the torn meniscus has been removed to a stable meniscal base.... . . The menisci are semi-circular fibrocartilaginous structures within the knee. There are two in your knee, one medial and one lateral. They serve 3 primary roles: 1. Shock absorber - distribute weight from the articulate cartilage 2. Distribute lubrication - help to move synovial fluid around the joint 3. Stability - provide secondary stability to the movement of the femur on the tibia. . They have peripheral blood supply, but are essentially avascular centrally. This means that the chance of healing following meniscal tear is relatively low. As we age, the meniscus undergoes a normal process of degeneration and becomes more brittle. This makes them more susceptible to tearing. . There is now good evidence that arthroscopic meniscectomy of degenerative meniscal tear in the setting of establish osteoarthritis doesnt provide predictable long term relief of symptoms. As such, first line treatment of these tears is non-operative management of the underlying arthritis - a low impact exercise program, weight loss, anti-inflammatories, corticosteroid or viscosupplement injections. . Tears with a more predictable response to surgery include meniscal injuries in younger patients, older patients with meniscal tears after injury with limited osteoarthritis, and meniscal tears eliciting mechanical symptoms (e.g locking). . . . . *Legal disclaimer: All patients featured in the Images (photographs and/or videos) have consented in writing to their Images being shared. From your initial appointment booking until aftercare of your operation and everything in between, I am here to make your patient experience as smooth and stress free as possible! . . I have a Medical Secretary background and joined Hollywood Orthopaedic Group early January 2017 as a Relief Secretary, which enabled me... to do different jobs throughout the group. In May there was an opening to work with Mr Lisle as his Secretary and Personal Assistant, and I jumped at the opportunity. I am absolutely loving working for Mr Lisle, his humour and friendly nature certainly makes my job easier and very enjoyable! . . The health and well-being of our patients is top priority and we are doing everything we can at our rooms to uphold that. Hollywood Orthopaedic Group have made some important changes to minimise the risk of COVID-19 to patients and the ...team as a whole. APPOINTMENTS: Every patient has been receiving information included in their reminder text message that states if they or a family member has travelled overseas within the last 14 days, has flu like symptoms or is generally unwell to call our rooms immediately. We are also in the middle of transitioning our high risk patients over to Skype reviews or Phone call review appointments. WAITING ROOM: We have strict signs at every entry advising not to enter if unwell (listing symptoms) and that our space is not to be used as a thoroughfare (minimising the amount of patients traveling through our rooms). We have hand sanitiser present at our reception desk, which all patients are welcome to use. We have also separated the chairs in our waiting room to practice social distancing while you wait for your appointment. If necessary to have someone accompany you to your consultation, please keep this to a 1 person limit only. This is important to help limit the amount of patients in our waiting room at one time. SURGERIES: *EDITED WITH UPDATE* All non-urgent Public surgeries at St John of God Hospital (Midland) have been cancelled. 05.01.2022 Ride for Youth This year Hollywood Private Hospital is one of 32 teams involved in the Hawaiian Ride for Youth event. Mr McCormick and Mr Spencer, who are a part of our Hollywood Orthopaedic Group, are participating in the event and will complete over 700km across 4.5 days. Last year Ride for Youth raised an incredible 2.5 million dollars for Youth Focus, which is an independent organisation that supports vulnerable young people throughout WA. They offer a range of... mental health services aimed at supporting young people and their families overcome issues associated with suicide, depression, anxiety and self-harm, and focus on early intervention and prevention. 02.01.2022 Joint replacement surgery was one of the most significant and effective medical developments of the twentieth century. Current techniques and prosthesis continue to evolve in order to improve safety and long-term outcomes. In Australia, we are fortunate enough to have the National Joint Replacement Registry (NJRR), which records information for hip, knee and shoulder replacement procedures. Data for hip and knee replacements started being collected in 1999, and became nationa...l by 2002. Shoulder replacement information has been collected since 2009. The 2017 report includes information from over 1.2 million joint replacements performed in Australia as of December 31, 2016. Using revision as the primary end point, the NJRR allows Orthopaedic Surgeons to identify key points that may increase failure rates, ensuring best possible practice.
