04.01.2022 Osteo-arthritis Case story A 65-year-old lady with chronic Knee pain and difficulty in walking due to arthritis. Seen a few doctors and took the medications as prescribed but in long term, there was no relief. She was referred for exercise and some physiotherapy. Gradually she had put on more weight and her physical fitness was deteriorating. After starting Physiotherapy, she reported some improvement in her walking and ability to move better but when she stops going to the ...physiotherapy clinic the knee pain increases. Due to the Coronavirus - pandemic, she refrained from going to the clinic which has led to increase in her pain and difficulty walking, going up/downstairs. After suffering for two months someone suggested her MTPC Virtual care, initially she said No, how can a Physiotherapist treat her being in Australia, that too being in front of a computer.With someone’s advice and persuasions, she agreed to see the physio - online. The session was conducted on Zoom and a detailed assessment with the use of webcam assisted the Physio to understand her issues. The exercises were demonstrated on the camera and she was asked to perform the same in front of the computer. The exercises and instructions were also emailed to her husband for future references. The review appointment was booked in a week and the session was conducted on Zoom again. The Physio noticed the patient was engaging better and showed some improvement. She was encouraged to continue her exercises, the progression of exercises, and pain care strategies were implemented by the Physio during the Virtual care session. Regular online follow-ups sessions were successfully conducted which were getting easier every session and she enjoyed consulting the therapist from the comfort of her home. In six weeks, the patient was walking better and reported less pain on walking and going up/down the stairs. She had a better understanding of her knee arthritis and developed self-confidence in managing her pain. She realized if she continued her exercises and followed her Physio’s advice she would not need any knee replacement in near future. She was discharged after 12 weeks of movement therapy with advice to re-connect if needed. To date, she is going very well and has no plans for total knee replacement. Happy days.
