23.01.2022 For those of us waiting for lockdown to end and our favourite shoe stores to reopen here are some quick tips to help ensure the right fit. The key is checking not just the length, but the width and depth as well. Length: We all look at the length of our shoe when checking the fit, although sometimes we get a little caught up on the numbers. Just because we were a size 9 last time doesnt mean we’ll be a size 9 this time. We always recommend to use the numbers as a (very) rough... guide. Trust your instincts, go for the size that feels the most comfortable regardless of the number on the box, and ideally go for a half-thumb to full-thumb length of extra space at the end of the shoe. Some of us who are a little wider or narrower are used to checking width as well. If you can, pull the sock liner out of your shoes and stand on them without the shoe to get a quick visual cue if the width is appropriate for you. Always make sure that you’re standing up when checking the width as our foot spreads out when we stand up - this is your true width. Finally, don't forget depth. Make sure the top of the shoe doesn’t hug too tightly or rub against the tops of your toes. If your toe always bursts through the top of your runners, this might be why. Finding a good fit can be a bit of work and some trial and error, but it’s worth it. It’s about more than just going for your favourite brands or the priciest model. The best shoe in the world that doesn’t fit you is, by definition, a bad shoe for you. 19.01.2022 Ingrown toenails What are they? Ingrown nails occur when the nail edge impacts with the delicate skin around the nail sulcus. Initially painful due to the pressure of the nail against the skin, the nail edge often pushes further into the skin causing redness, swelling and infection. ... One in five people will have an ingrown nail at some point in their life, and they can be quite painful. Why are they? Ingrown nails are caused by a few things: - Tight shoes, especially in the toe box, forcing the nail to grow inwards. Don’t just think high heels, this happens in soccer or basketball shoes as well. - Poor toenail cutting technique - Genetics (blame mum or dad for this one) What can we do? A trained podiatrist can numb the toe (if you need), painlessly remove the nail spike and encourage the healthy growth of the nail. Sometimes, we can do this in a few minutes without you feeling a thing! In other cases a minor surgery can often correct the problem, permanently removing the nail edge all the way to its base, easing the pain immediately and preventing any future 'in growns'. Which, we can perform for you here once we have discussed it with you and tried the conservative stuff first. SOME PEOPLE WILL TALK ABOUT SOME OF THE MYTHS AROUND PREVENTING AND SELF MANAGING IN-GROWN TOENAILS. It might work sometimes, but most of our patients would tell you it's far easier to get it seen and sorted by a professional than to keep 'hacking away' yourself.
19.01.2022 Bunions!! Hallux Abducto-Valgus (HAV) deformity, more commonly known as Bunions, is where the big toe gradually points inwards and can form a prominent bony joint. The degree of severity, presentation and pain can vary greatly between people, emphasising the need for a professional opinion.... What Causes It? The large majority of bunions are multifactorial, meaning the accumulation of stress factors placed on the joint over time causes the deformity. The common theory is that poor footwear, such as high heels and narrow toe boxes, result in increased pressure at the joint when walking. However, other things such as your genetics may also play a role. Risk Factors: High heels Tight shoes Flat feet Family history Sex (Female) High BMI What can you do? The simplest way to prevent the progression or pain of bunions is to wear footwear with an accommodating width and stiff sole at the forefoot. The footwear feature we typically recommend is a Rocker-sole, which aims to rock you through propulsion thus alleviating the need for the big toe to push off. Yes not everyone has tickly feet, but they can still be some of the most sensitive. It no wonder that that sensitivity plays a role in so much of your foot discomfort or comfort. 18.01.2022 Case Study: BE CAREFUL IN BAREFEET! Name: John (not his real name, I promise... maybe) Age: 38 (ish)... Likes: Gardening, Golf, Reading History: Type 1 Diabetes Issue: Walked around barefoot and kicked the side of the table. Ouch! Lesson: Dont walk around the house barefoot, wear a comfortable pair of shoes instead!! Plan: Clean up the area. And apply betadine and a sterile bandage. Which he can do himself at this point. As we are in COVID-19 monitor by video and photos to check on the status of pain, redness, swelling or and goop that might come out. Salt bath the foot after his shower to clean the area daily. then re-apply bandage and anti-septic i.e. betadine. What a silly boy! ;)
16.01.2022 We’re seeing a lot of sore toes this year. An achy toe sounds like a small thing until you have one. A painful toe makes us dread the walking the dog, they make us limp, they stop us wearing our favourite shoes. They get in the way of life. So why would our toes hurt? All of our toes have a special role to play in our feet and sometimes they can get overworked or overwhelmed trying to do fulfil their role. ... We often hear people complain about their big toe hurting. It’s a hard working part of our foot and so it takes a beating. It stabilises our arch, helps us push forward when we run and supports over half our body weight with every step we take. A painful big toe could be any number of things: arthritis, turf toe, synovitis, capsulitis, sesamoiditis, insertional fasciopathy, bunions, ligament tears, in-grown toenails... you name it. 16.01.2022 How to tell if your feet are flat.... Is it if your feet have low arches? What about if they roll in too much? I am sure my kids have flat feet... So... my opinion on flat feet. They are just shaped like that. Just like if you have blue eyes or green eyes that is the way you are. Just as you wont be given glasses on their colour, but you might if you are having problems with them. You shouldnt be given foot orthoses on shape alone. ... 1) Flat feet is not a diagnosed condition. 2) It may provide a contributing factor to your diagnosed condition, but so might a foot with high arches or a funny big toe joint, or a foot that looks completely normal. When a podiatrist assesses your foot type it is often done statically. We are often moving more than we are standing and research has shown that static posture does not dictate your movement pattern. So, while flat feet maybe your foots shape. What is most important is how you are moving, feeling and getting on with what you are doing. If you have pain or problems with your feet consult your podiatrist and let them guide you to a rehabilitation and symptom minimisation program. Alternatively, if you have been given orthoses (orthotics) for your flet feet. Maybe you can go through a plan to assess how they are helping your feet function and how much you still do or dont need them anymore based on your actual diagnosis and not a shape! 13.01.2022 So, how often should you review your foot orthoses? There is no one size fits all approach here (almost a good foot pun). However, this is how we try and do it. Issued... 2 weeks later: make sure they are doing what they are designed to do (take away discomfort, improve the positive snowball) 3 months later: double check wear and tear, follow up on progress of reason who have them. 6 months later: consider long term usage or not usage of the device 12 months thereon: check quality and condition of devices, check they are doing what you need and meeting the needs of your activity (ies). As I said, this is not a one size fits all approach. But, they should be checked regularly. They are not a set and forget. They are prescribed as part of improving your overall health. So until that is resolved, then your review appointments should not be. If you wear them for comfort, and some people do. You should check them each year for wear and tear and also that they are meeting your new demands each year. You may have gained or lost weight, taken up chess or marathon running. All of these things contiribute to how they are used. If you were given them as a kid. Get them checked NOW. You are no longer growing and developing, you are now an adult. If you are still a kid, you and your body changes a lot in a short space of time, so your needs and demands for them will change with it. #orthotics #podiatry #physiotips #shoes #running

Do the maths, it is likely you will be turning them over about every 10 months if you cover 70 km a month, or 17 km a week or 5 km a day (for 5 days). Check these bad boys out from a patient this week... what do your shoes look like? Be great to see some running shoe pics on here and how long you have had them. Bonus points if you can guess the age of these shoes! (p.s this guy had serious heel pain, which was sorted once he updated his shoes, so simple!)
13.01.2022 Patient story of the month: Meet Jack. Jack has diabetes, loves his loafers and reading the paper.... Jack came to us for a general check up of his feet some months back. In that process found a wound/hole/big cut on his toe. Fortunately, he arrived before Stage 4 lockdowns! However, we managed to clear that up by stopping the aggravating factors of nails rubbing against the skin and shoes that are too tight and healed him up just as Stage 4 was being put into place. Then he turns up with a new wound under the bottom of his foot from too much new walking in his very worn-out loafers! Way to ISO! We cleared that up too with some offloading, a check-in with his GP for any extras and some adequate bandages (none of this is technical speak). I am pleased to say, we got his foot intact and clean and we are hoping when we catch-up face to face he has kept it intact and clean! Stay safe all! Well done Jack! Time for new shoes Jack! ;)
12.01.2022 Huge congratulations to John for being the FIRST IN AUSTRALIA to obtain this qualification (CERTIFIED SPORTS PODIATRIST) outside of the committee that developed it. This has been a huge win for John and the clinic all while he completes a PhD, works and has his little family at home. John has been able to demonstrate across a series of case studies, exams and practical exams his capacity to manage sporting injuries in podiatry. All patients that have seen John should feel pr...etty proud that they know they are getting looked after by one of Australias best podiatrists, which we are all pretty excited about here. So, congratulations John on passing this last night and we look forward to seeing the extra bits of paper that go with it. 12.01.2022 Aaaargh! BUNIONS!! That is the common response people give about these change to the structure of your foot. So I thought I would address a couple of common question about them. Do bunions cause arthritis?... In short, No. However, that is very short and the question needs more unpacking than that. Big toe joint arthritis is the change in joint space at that place. However, arthritis is very, very common. In the young footballers we manage, I would say about 70% these 17 year olds all have OA in the big toe joint, but no bunion. I think the statistics suggest prevelance of arthritis (OA) in the joint is over 50% of the population will get arthritic change in that joint. The big one is Rheumatoid Arthritis. In 90% of cases the forefoot is involved! That is huge. Overall, the main concern is pain and function. If it is sore or it is reducing your function SEEK HELP, we can provide strategies for helping you manage them. 09.01.2022 Cricket Season is on its way... And pre-season is beginning. Now is the time to update your old equipment. And shoes should be a priority! A note on cricket shoes. Find a pair that fits properly. This is a challenge for the womens game as the brands are yet to release a pair specifically made for women. However, we are lucky to have an inside word that Asics are developing one for the women (yay!) so we might be able to tell you before they are released (watch this space)!... Try and avoid re-soling/re-tooling a pair of runners (see picture). It makes them much HEAVIER, more RIGID and takes away so many of the benefits the runners were once providing. Not least of all, your toes are still only protected by a mesh upper. There are plenty of brands and styles out there and if you get stuck we are happy to help guide you around it. So update your shoes, but get a genuine pair of spikes, dont retool if you dont need to. #cricket #cricketshoes #gothevics

06.01.2022 Here are the feet of the fastest man in the world! Back on the flat feet for a moment... Or even bunions... have a look at these beauties! Notice how the fastest feet in the world are not short of imperfections. However, I am not sure some more perfect feet can run as fast as these little (very big) treasures can. There was a whole lot of talk around his biomechanics (fancy word for how you move) and how they contribute to his speed. However, breaking that down it is i...nteresting to look at his feet. My favourite topic, foot strength, plays a role here. Bunions have been shown to make your ability to flex your big toe weaker. Leading to increased falls and shortened step length among others. It would be interesting if he a) didnt have the bunion or b) worked on his digital (toe) strength and if that may have made him even faster! I guess we will never know. For the rest of us speedsters out there, perhaps try working on your big toe pushing strength to see if it can not only improve your balance but your overall push off and stride control... #podiatry #sprinting #isoruns #physiotherapy #100m #usainbolt
05.01.2022 Can you change your shape by doing different exercises? Why? Well first, I would like to ask you that question myself. Why do you want to change the shape of your foot? I feel like it feeds the narrative that foot shape matters. Consider for a moment, the way your foot looks while standing still is not = how it will move. If you want more on that I have a few posts earlier check them out.... 1) There are too many joints you are trying to modify. Squatting your way to oblivion does not all of a sudden mean you walk around with your knee permanently bent or straightened beyond straight, the same is at the foot. Plus there are 33 joints to try and do that to to influence the shape of your foot, with very small muscles producing on average about 60 N of force (the calf can produce a whole lot more than that). 2) The foot needs to bend and generate force. If you were to try and stiffen the foot (assuming you are trying to make more of an arch) by exercising it, then you are potentially taking away its incredible capacity to absorb your body weight. In the interests of keeping this post quicker... WHAT IT DOES DO... Exercises will make your foot stronger. Thats it. Boring hey! However, if that strength means you can do more and increase your capacity that is a massive WIN! If it gives you confident to do more, WIN! If it rehabs your tendon, WIN! So consider what you are. trying to achieve before going down the long and tiresome road of exercising to change your foot shape. And anyone telling you otherwise I selling #snakeoil. So... **Spoiler Alert** The answer is simply... NO. Exercises do not change your foot shape. They do other wonderful things instead. 05.01.2022 One simple exercise to increase your foot strength... Reduced foot strength is implicated in increased falls risk. It can also be associated with pathologies such as heel pain, stress reactions and shin pain. Furthermore, increasing foot strength as part of your rehab protocol has been shown to improve outcomes for those with knee pain and knee pathology too. So, the take home, dont ignore your foot strength. Obviously, you want to make sure you are addressing the deficit y...ou need to. However, try this simple exercise while you brush your teeth to improve your foot strength. Getting you back on your feet is more than one magic bullet though... foot strength is one part of the whole picture. It would be really cool to see if people (YOU) could add your record or how long you can do this for... I got to 5 mins and could do 10 repetitions only.

04.01.2022 An inch, as we know it today, would be the total length of three barleycorns. During the reign of King Edward of England, he signed a royal decree in 1324 making barleycorn as a unit of measurement in shoe sizes. He proclaimed that three barleycorns were, in fact, equivalent to one inch! From then on, in England at least, sizes of shoes were derived from the length of barleycorns. If you’re one of the many Melbournians who’ve taken up running this year, here are a few simple tips to keep you injury free. One: Build up slowly! We all know by now that going too hard too soon is a good way to get hurt.. And while that might be good for your podiatrist it isn’t good for you. Gradual, incremental improvements over time are how we get fitter and healthier. So with that in mind, we recom...mend increasing your weekly running volume by about 10% each week to avoid over-taxing your feet and legs. Two: Cadence! The smallest big thing you can do to avoid a running injury is to take smaller steps. A higher cadence means less force going through your foot with every step and less chance of bruising or breaking something. Three: Speed isn't everything! Resist the urge to max out on every run. Sometimes taking a moment to slow your pace and take it easy can help you run for a little bit longer, and those extra minutes can add up to hours by the end of the year. 01.01.2022 Aaaargh! BUNIONS!! That is the common 'response' people give about these change to the structure of your foot. So I thought I would address a couple of common question about them. Do bunions cause arthritis?... In short, No. However, that is very short and the question needs more unpacking than that. Big toe joint arthritis is the change in joint space at that place. However, arthritis is very, very common. In the young footballers we manage, I would say about 70% these 17 year olds all have OA in the big toe joint, but no bunion. I think the statistics suggest prevelance of arthritis (OA) in the joint is over 50% of the population will get arthritic change in that joint. The big one is Rheumatoid Arthritis. In 90% of cases the forefoot is involved! That is huge. Overall, the main concern is pain and function.
