25.01.2022 We are all familiar with the saying "prevention is key", but when prevention fails, seeking treatment early will be your next best bet. And, here's why To prevent the pain from becoming the new normal We often see people who have had some kind of pain in their feet and legs for 10 years, or even more sometimes. Pain is there to tell us something is not quite right, but if we put up with it enough, we become desensitised to the pain, accepting that living with pain is ...somehow normal. . To prevent the "it can't be treated" mindset The longer you have had a problem, the harder it is to un-do the problem. Along the way, you may feel frustrated and hopeless, and those are the perfect ingredients for a "it can't be treated" mindset. The problem with this mindset is that you will always doubt whether a treatment will work even before trying it. Someone with an optimistic view on the outcome of the treatment will ALWAYS have a better outcome at the end. Essentially, if you truly think your condition can't be treated, or you will always live with pain, this thought would affect your actions, and your actions would further reinforce your presumption. . To minimise negative impact on your wellbeing The funny thing about pain is that it often draws our full attention. It can be a real distraction that affects our performance and productivity. While our feet are cosy and warm in our socks, there are a few things we should be aware of: . Choose socks that are thicker for better heat insulation and sweat absorption. YES, our feet still sweat even in the cold! If our socks are too thin, they get damp from our sweat quicker. The end result is your feet ac...tually end up feeling colder! This is especially important if you are prone to developing chilblains (yikes ). . Change your socks everyday. Nothing fancy about that. Just basic foot hygiene worth reiterating. . Wash your socks at 60 degrees celsius water (together with your laundry powder/liquid of course) to ensure all the microorganisms are killed. You can also add Canestan (an antifungal laundry liquid) to your laundry if you are prone to getting Athlete's Foot (fungal infection of the skin), or sharing the laundry with someone to avoid cross-contamination. . With dermatological conditions, treatment will take place at the initial consultation after the podiatrist has taken enough history regarding your problem. . Biomechanical conditions, on the other ...hand, include FOOT PAIN (HEEL, ARCH, BALL OF THE FOOT, TOES), ANKLE PAIN, SHIN SPLINTS, KNEE PAIN & LOW BACK PAIN. As these conditions are complex in nature, the podiatrist will have to spend more time on history taking in order to gain a clear clinical picture. 16.01.2022 Thickened toenails are indeed more common in the older population, so its a fair question to ask. What contributes to this phenomenon? Lets have a look at a couple of things that can make your toenails thick! . 1. Repetitive trauma from footwear This form of nail damage takes place over years. It is markedly different from direct trauma, like dropping a heavy object on a toe, kicking a table leg, etc. Nail trauma related to footwear generally occurs as a result of ill-f...itting shoes (too narrow, too small, too short, too wide) and the nature of the shoes (e.g. steel-capped boots, high heels). It can also be caused by a hyperextended big toe, where the big toe bends upward. When there is too much rubbing and stubbing of the toenail, subtle changes start to occur in the nail matrix, affecting how the new nail looks. Because of this, pressure-damaged nails tend to grow thicker over time. . 2. Fungal infection of the toenails As toenail fungal infection progresses, the integrity of the affected nail(s) starts to change. The nails become thicker and crumblier while they slowly become detached from the nail bed. Yellow crumbly debris will also start to collect under the nail. As toenail fungal infection is more prevalent in the elderly, thick toenails also become a commonly seen phenomenon in this population. . So, to answer the question YES, toenails do get thicker with age when there has been: long-term trauma to the nails; and/or the presence of a fungal infection. . 15.01.2022 MUSCLE CAPACITY While the term "muscle capacity" is rather self-explanatory, it is not necessarily something we take into account when we launch ourselves into our fitness plan. . As the name suggests, muscle capacity refers to what our muscles are capable of doing effectively. If an activity falls within the range of your muscle capacity, you can do this activity easily without exhaustion. The risk of sustaining an injury is also very low. .... Let's take walking as an example. Walking on level ground requires your muscles to work at 30% capacity. However, walking on a slope needs your muscles to work at 40% capacity. If most of your physical activities involve only walking, your muscle capacity is somewhere between 30 and 40%. . However, if one day you decide to go for a 20-minute run, and running requires your muscles to work at 60% capacity, you have a deficit of a minimum of 20%. . The discrepancy between your muscle capacity and activity requirement is what often causes injuries, aches and pains. . What if you keep pushing on with the running? Will you eventually reach the 60% capacity that running requires? Yes and no. Your previous fitness level, age and weigh all influence on how much damage the discrepancy will cause, and hence, the end result of this "keep-pushing-approach". . The thing is, when it comes to increasing our fitness level, a one-dimensional approach (i.e. just doing the sport you want to do e.g. running, playing basketball etc) is never enough. 08.01.2022 "How many follow-up appointments would I need?" . This entirely depends on what you are presenting with and the severity of it. . For example, if you have a corn or ingrown toenail, you may not need more than one appointment. HOWEVER, sometimes a corn can be very deep because it's been there for a long time, or that it is at a bony location (the toe). Combined with lifestyle factors (shoes, how much time you spend on your feet), the corn would become more difficult to treat.... Then, a few follow-up appointments may be essential for you to get complete relief. . Similarly, a straight-forward ingrown toenail can often be treated in one appointment. But, if the ingrown toenail is deep, or trapped in a tricky spot, it is also normal that you would need a few more follow-up appointments. We have seen many ingrown toenails as an once-off occurrence while some are a recurring problem. . Fungal toenails and warts ALMOST ALWAYS would require ongoing treatment due to the nature of these two conditions. . Like we mentioned in our previous post, your condition may be Biomechanical in nature (i.e. you have pain somewhere in your lower limbs). 08.01.2022 If you run your finger across your big toenails and feel some ridges running across your big toenails, you must be wondering "WHAT ARE THESE?" These horizontal ridges are called BEAU'S LINES. They are a good indication that your big toenails have sustained some direct trauma. Beau's lines form when nail growth is temporarily disrupted. . WHAT CAN CAUSE DIRECT TRAUMA TO OUR NAILS? Shoes are the most common cause of nail growth disruption. When the big toenails hit the top.../front of the shoe, it causes direct injury to the nail matrix which is located at the bottom of the nail plate. As a result, the nail stops growing for a short period of time. When the nail eventually starts growing once again, a Beaus line is formed. . Presence of multiple Beaus lines in one toenail is possible and, in fact, very common. It just means as the nail continues to grow, there have been constant disruptions to the nail matrix. . Beaus lines associated with pressure damage are harmless. The problem with labelling a sporting activity as exercise is that it makes it difficult for us to see why we need to TRAIN for the sport before PLAYING the sport. . We all know that professional athletes train very hard. If we look at what their training involves, we will see that their training isn't just playing tennis (let's take tennis players as an example) the whole time. . They do cardio training, weigh training, muscle ...conditioning and stretching as part of their training routine. Why? By doing cardio, their body and muscles can gain endurance; by doing weigh training, their muscles can generate more power; by doing conditioning and stretching; their muscles can stay in an optimal condition and prevent injuries. Then, they go out to play tennis. . If tennis players only play tennis, it is likely that they don't have the endurance to last a 3-hour match, the strength to generate powerful shots, and reflexes required for effective play. More importantly, they are likely to tear a muscle and get injured. . While we are not elite athletes, the same concept applies to us. If our muscle capacity is only at 30%, we can't expect our body to function at 70%. Pains are often a signal our body uses to tell us that it is not quite ready for what we try to do yet. . Therefore, we could not emphasise enough how important it is to do specific muscle exercises to train our muscles, and just to have a more well-rounded approach in general. When we view "exercise" as any umbrella term for all those various trainings (cardio, strengthening, conditioning, stretching), it becomes easier for us to understand why we can't blame our body for not putting up with one hour of social tennis. .
