19.01.2022 Low back pain management - Part C Postural awareness and regular back stretches are important to assist in controlling your acute low back pain. In order to prevent recurrence of your low back pain for the long term, solid core muscles are needed. Just imaging you are going camping and you need to set up a tent. The poles to support the tent need ropes to be tightened to hold the pole strong and steady to stop it from falling sideways. Those ropes are just like our core m...uscles holding our spine in a good alignment to stop abnormal stresses through the spine. Therefore, your lower back shouldn’t get as stiff and tight as quickly and then that greatly reduces the chance for your low back pain to recur. Many people think that in order to build up core muscles you have to do a lot of sit-ups. Our core muscles are deep muscles around our waist and they do not just perform general gross movements. They are more of a control unit for subtle spinal movements. For example, when you are doing a sit up, some people would often perform a quick and full sit-up but in this action, you tend to use more of your hip flexor muscles (at the front of your hips) and therefore put strain on your lower back. However, doing a half sit-up without your whole back lifting up from the ground at a slow pace will strengthen your front abdominal muscles (6 pack muscles and obliques) and activate your core muscles with much less strain on your back. There are many other exercises people commonly do (such as bridging, lifting arms and legs while on their hands and knees), which can assist in strengthening your core muscles by modifying the pace and the movement pattern. Your Physiotherapist can review your exercise routine and adjust them as needed to accommodate your low back issues, allowing you to make sure you have a good daily routine to help you maintain good low back flexibility and stability. 11.01.2022 Lower back pain management - Part A Many people have the impression that when you start getting a sore low back while doing housework/gardening, then you should sit down and rest. Furthermore, when you see the Doctor about your low back pain, they might often suggest you to rest more and so you end up sitting more! Many research studies have shown that the disc between your vertebrae at the low back region starts to build up pressure after you have been sitting for at leas...t 15 minutes! With our abdominal contents at the front of the disc, the disc can get pushed backward when the pressure starts to build up resulting in direct touching of our adjacent nerves. Since the nerves control your muscles, a build up of pressure on the nerves may lead to increased tension and soreness in the back and lower limb muscles! If your sitting posture is poor, especially if you tend to slouch forward, then the backward squeezing force on the disc is even greater. Therefore, prolonged sitting is definitely not a good solution for low back pain. Ideally for office workers, it is recommended to get up to walk around after every 30 minutes of sitting. If this is not possible, then it is fine to have a quick stand up, followed by a quick stretch and then sit back down. That is why many offices nowadays are equipped with stand-up desks. (to be continued) Is your sleep disrupted by lower back pain? Sore back with driving? walking? Call us for a physiotherapy appointment for assessment and treatment. 06.01.2022 Low back pain management - Part B Many people wonder whether you should surgically remove a bony spur that has developed on your spine. Some people would even consider taking medication to reduce the bony spur. The cause of a bony spur is the result of long term degeneration. As your cartilage wears out with the ageing process, bones start to rub against each other and subsequently the bony spur forms in order to reinforce the area. ... The composition of your bone and the bony spur are exactly the same. Therefore, if there were medication able to reduce bony spurs, your normal bone would probably be adversely affected as well. People who present with acute low back pain, often have low back X-ray images that tend to look the same compared with imaging one year previously. During this same period, the back condition does not necessarily worsen and nor does the bony spur necessarily become larger. Most acute low back pain can get relief with Physiotherapy intervention, control of inflammation, correct exercises and improved posture despite the presence of a bony spur. Many research studies have shown that out of a hundred people with low back pain, only less than five actually need surgical intervention. Therefore, in the majority of cases, seeing a Physiotherapist to analyse your general posture, work tasks and exercise routines can assist in identifying the contributing factors to your acute low back pain. Therefore, it will be significantly easier to keep your symptoms under control and fast track the rehabilitation phase of getting your back stronger to minimise recurrence.
