22.01.2022 The blood from the placenta is not EXTRA blood. It is 30-50% of the babys normal blood volume. Premature cutting of the umbilical cord deprives the baby of it...s full blood volume. There is no such thing as cord blood. The cord is just what the babys placenta uses to send and receive blood from the baby. Why is this so difficult for most people including doctors, to understand? 18.01.2022 Are you curious about Waterbirth. A recent Aus study has found women who gave birth in water were more likely to experience shorter first and second stages of l...abour. However a common question about waterbirth is, "What keeps the baby from breathing under water?" Here are some great words from @waterbirthint website. "There are four main factors that prevent the baby from inhaling water at the time of birth: 1. The foetus moves the muscles of the chest wall during pregnancy about 40% of the time. Close to the time of labor, the Prostaglandin E2 levels from the placenta rise, which cause a slowing down or stopping of those foetal breathing movements. As the baby is born, the Prostaglandin levels remain high, disabling the babys muscles for breathing. The muscles simply dont work, thus engaging the first inhibitory response. 2. All babies are born experiencing mild hypoxia or low oxygen levels. Hypoxia causes apnea (absence of breathing) and swallowing, not breathing or gasping. The first reflex after a baby is born is to swallow, not breath. The swallowing will allow the fluids that are in the mouth to enter to stomach. 3. Foetal lungs are already filled with fluid. That fluid is there to protect the lungs, and keep the spaces open that will eventually exchange carbon dioxide and oxygen. It is very difficult, if not improbable, for fluids from the birth tub to pass into those spaces that are already filled with fluid. One physiologist states that the viscosity of the fluid naturally occurring in the lungs is so thick that it would be nearly impossible for any other fluids to enter. The blood supply to the lungs is also very low during pregnancy and birth. This causes a high pressure within the lungs, thus keeping everything out. 4. The mammalian diving reflex is an inhibitory factor that is present at birth in all humans as well as all mammals. It lasts in humans up to six to eight months. When the face comes into contact with water, the glottis at the back of the throat automatically closes and prevents water from entering the lungs. Any solution that enters the throat is swallowed, not inhaled. Image by @alana_farmer

11.01.2022 Lets take a trip back to the 1940s, when women were given chloroform or morphine during labor that caused unconsciousness and no memory of giving birth. Obvio...usly back then, anesthesia was much less safe, and aspiration was more common than it is today. Aspiration is when an unconscious person inhales their stomach contents during an operation. The reason you are told you shouldnt eat during labor is because you might have to have a c-section, and if you have a c-section, you might be at risk for aspiration. But back in the 1940s, anesthesiologists were using very primitive tools to keep the mothers airway open while under general anesthesia, and some didnt use tools to keep the airway open at all. Today, our medical tools and anesthesia are much more advanced, and aspiration during birth is rare. But even though there have obviously been great medical advancements since the 1940s, the policy of not letting a laboring woman eat or drink has only slightly lifted. ACOG now suggests women be allowed clear liquids during birth. The American Society of Anesthesiologists (ASA) said in their position statement, that aspiration has become so rare that randomized trials have been unable to calculate an incidence of it in recent years. Their conclusion was: There is insufficient evidence to draw conclusions about the relationship between fasting times for clear liquids or solids and the risk of aspiration during delivery. What we do know without a doubt though, is that the uterus is mostly made of muscles, and muscles need fuel to work efficiently. 06.01.2022 ENTONOX - is it as harmless as we think? This months issue of TPM is focused on climate change - and our responsibilities as midwives to play our part ...in halting the process. One brilliant and insightful article (SWIPE) by two Public Health Speciality Registrars in England lays out the facts clearly - Entonox is contributing to global warming. Climate change is one of the biggest threats to the health of a child born today. Yet most babies in the UK are born into a room full of greenhouse gases, with nearly 80% of women using nitrous oxide (mixed with 50% oxygen, also known as Entonox) during labour. The carbon footprint of an average birth using Entonox as analgesia is 400kg CO2e3 this roughly equates to driving an average-sized car 1,400 miles. Unfortunately, many healthcare professionals, and labouring women, remain unaware that the nitrous oxide in Entonox and other anaesthetic gases are major greenhouse gases. 03.01.2022 Why do almost all mums focus on a set date for their baby to arrive by? Because society teaches these things because our medical system has instilled fear and t...he mistrust of our own bodies abilities and natural processes. According to an article written by Rebecca Dekker from Evidence Based Birth "For many years, a baby was defined as being born at term if it was born between 37 weeks 0 days and 41 weeks 6 days. Anything before that 5-week period was considered preterm, and anything after those five weeks was post-term. But today we see our OB, Doctor or medwife and we are told that if the baby doesn't come by a certain date we need to consider induction, membrane sweep, you need to start considering things to get baby moving along or even maybe a c section! We use to have more available care providers who knew and understood the natural normal process of labour and birth, understanding that there is no set 'arrival' time for any baby but that each gestation is different and unique to the baby. We are brought up being told these things now because our mothers were told this, all our friends were told these things. We are taught to believe that our baby is naughty or being stubborn if they don’t come when we want or when our doctor wants. When a mother chooses to become educated in her choices if she educates herself and says different to what her care provider likes than she is bullied and called crazy or a no-it-all. A mother who goes past 40wks or *gasp* 41 or 42wks is given so many scenarios, fear-mongering and is bullied to make her think she is endangering her child by wanting to follow her own intuition and natural pregnancy process. Less than 5% of babies are born on their estimated due date while the average gestation of a first time mum is 41+3wks gestation. Your body or baby isn’t broken and you’re not crazy for listening to your body and baby. We need to make a change in our language, in our society so that our daughters grow up to understand, value and respect the whole process of pregnancy and birth, not to fear but to trust their body and baby.
