18.01.2022 Isothiazolinone (Iso) is a corrosive biocide that causes sensitization in some people. When I was in #watertreatment and a #productmanager I heard many ugly stories from people hurt using Iso. Some went like this: Bob’s job was to prevent #legionella outbreaks in small #coolingtowers by to dosing them with Iso. Using a jug was normal, and so were spills. One morning Bob spilt Iso into his boot and did not realize until he removed his sock after work and found horrible blister...s covering his foot. He could not walk for days. Iso causes little burning sensation, so Bob thought it was only water. After years of repeated exposure, Bob became sensitive to low concentrations. He would get a rash if he went near cooling water treated with Iso. He even started to get a rash when he washed his hair because Iso was an ingredient in his shampoo. Some individuals are susceptible to sensitization to some chemicals, just like Bob was to Iso, and it is not always easy to predict. 12.01.2022 Chopping the hooves off wild goat helped pay my way through University. It was an easy job to learn until #safety changes made it difficult. Hock cutters are designed to be used with two hands, to prevent you cutting off one hand. Hold a button with one hand then pull the trigger with the other. Safe enough, right? Seems not. Tape down one button and you can easily cut off your hand.... New cutters appeared that only worked if you pushed both buttons together. Timing was crucial but the safety problem was solved, right? Nope, you could still cut off someone else's hand. The next #innovation had a delay after you pressed the buttons, to allow time to abort. This change made the job tricky because it delayed feedback when you mistimed the buttons and because the cutters could not move horizontally To solve this, a roller was installed at the top of the cable holding the cutters. This gave more time but had the effect of pulling the shears across as you went to make a cut. 11.01.2022 Chopping necks off sheep helped pay my way through University. It was the easiest job, ever. Hold a button, push down, rinse and repeat. I could do this forever I was wrong. There was a big problem and it took me a month to notice. Doing the same thing every 12 seconds for 8 hours was taxing a single spot in my back that controlled the movement of my right arm. This spot gradually got very, very sore.... I noticed it was getting sore at the end of the week but did not worry because the pain was gone after the weekend. I simply adjusted my posture and tried to unloaded the muscle. It finally got so bad it was still sore after the weekend and I asked the foreman to move me to a new task. After much insistence he gave me a job with a different action and left me there, without rotation, for a month. This experience taught me to seek out issues that might only be apparent to people who have done the job many times themselves. 07.01.2022 The human error rate is 1 in 10, except when it’s not, and only when it matters! I was at a James Morrison concert with my very musical, trumpet playing, wife It was surprisingly good. From start to finish James was the consummate performer. He strode in, playing the first note for what felt like 3 hours circular breathing or a hidden compressor? He backed it up with an intricate piece that he played so fast my brain could not keep up. He combined top notch Jazz with engag...ing stories and even stopped mid- piece to explain the music and tell jokes. Not-so-musical me was enthralled. Afterwards, I could not recall any mistake in the whole performance. It got me thinking: What was his error rate? Saying one in a million would have been unkind he was just that good. What might James say though? Maybe 10% of his performance did not meet his own high standards I don’t know. What would have been an error that actually mattered? I’m still not sure. The slide flying off James’ trombone and hitting me in the head would not have ruined it for me. I might even have enjoyed it more. 06.01.2022 Why do scary dangerous things happen at work? Is that a ladder in a scissor lift? the auditor exclaimed as we entered the warehouse. We took photos of the 2 metre A frame ladder and removed it from the scissor lift. I then went off to have a chat with the two electricians who had last used the lift... It was necessary they explained. Their job was to connect the warehouse office to the internet and were running the LAN cable via the ceiling. The scissor lift was 2 meters to...o short so they needed a ladder to make up the difference. One electrician stood on the top rung of the ladder, in a scissor lift that was 12 metres above the ground, while the other electrician held onto the ladder. The office had to be connected by tomorrow and this was the only way they had to do it. It is easy to get people to do scary dangerous things at work. Just demand that they get the internet connected by close of business that day and give them a scissor lift that is 2 metres too short. They’ll be sure to find the right sized ladder, and if they can’t, maybe a pallet and an office chair will do. #safetystory #stafetystories #safetystorytelling #workingatheight #riskperception #auditing
