20.01.2022 Evidence Based Practice EBP is the integration of best research evidence with clinical expertise and patient values (Sackett, Straus, Richardson, Rosenberg, & Haynes, 2000, p.1). This means that evidence based practice does not rely on appraisal of the literature alone. As well, clinical experience (i.e., the clinicians expertise, clinical reasoning, and past experience) and the patients preferences and life-goals must also be considered (Hoffman, 2008). This is a great definition of evidence based practice. Rather that just relying on the literature alone, clinical experience and the patient's goals must be considered. If something doesn't work, you reevaluate and work out a new plan. But if something works consistently in clinical practice, then it is reasonable to keep doing it.

13.01.2022 Understanding the Shoulder I recently took an online course called "The Shoulder Complex" by Marilyn M. Pink PT, M.B.A., Ph.D. through Educata - an online continuing education resource. I attended a course by Marilyn when working in the US and was most impressed! The talk of the town at that time was "Scaption". This online course thoroughly examined the shoulder complex and reviewed mechanics, muscle function and exercise. Marilyn talks about the 4P's of scapulohumeral muscl...es: Glenohumeral PROTECTORS: Rotator Cuff Arm PLACERS: Deltoids Scapular PIVOTERS: Scapular muscles POWER Drivers: Lats and Pecs Through her research and other research quoted, she clarifies the function in detail of muscles, their directional pull, force couples and strengthening of muscles in athletes. The course gave me a good review and a deeper understanding of the 4P's. Don't be turned away by the pre course exam and the American accent! Hang in there! - It's worth listening to and great value for money! Course rating: 4.5 / 5.



13.01.2022 Is there a plumbing problem? The plunger is a great therapy tool! It's simple, cheap and extremely versatile. This simple tool allows you to facilitate multi directional arm movements in standing, sitting and in supine. The plunger can be suctioned to the floor or wall to keep it in position and a noodle grip added to facilitate a better grasp. Add weights to increase resistance. In the picture below, the plunger replicates a gear stick. By moving the "gear stick" into first ...gear and then into second gear, we facilitate elbow extension and flexion in a functional task. Now take that plunger one step further! Add on an arm trough made from Manosplint thermoplastic to support a flaccid or weak upper arm whilst doing reach activities. By having the support to the arm, you can increase the number of repetitions and do functional reach in both sitting and standing.

12.01.2022 The more you know, the less you know! As you develop more experience, you become more analytical and think about things in greater depth. You then start to have more questions and less answers! What happens to ligaments and tendons when the wrist is positioned in extreme flexion? Do ligaments contract like muscles? Can we use what we know in Orthopaedics and apply it in Neuro?... What do we know about ligaments? Taken from M. Solomonow / Journal of Electromyography and Kinesiology 14 (2004) 4960 Ligaments consist of closely packed, parallel collagen fibers which appear to have various degrees of undulation (or helical) form along the axis of each fiber at a resting length. There are also short cross fibrils which connect the axial fibers to each other. Ligaments, mostly collagen (75%), elastin and other substances structure provide various degrees of stiffness at various loads and at various ranges of motion. When a constant load is applied to a ligament, it first elongates to a given length. If left at the same constant load, it will continue to elongate over time in a exponential fashion up to a finite maximum.This elongation over time is termed creep. When ligaments are subjected to a stretch and hold over time (or constant elongation) the tension relaxation phenomena is observed. The tension in the ligament increases immediately upon the elongation to a given value. As time elapses, the tension decreases exponentially to a finite minimum while the length does not change. Is full recovery of the "creep" possible when the ligaments have been loaded for a prolonged period of time? What happens to ligaments after prolonged immobilisation? Solomonow reports that immobilisation results in increased osteoclastic activity, resorption of bone and disruption of the pattern of diffusion of the ligament fibers into the bone. Immobilisation or decreased physical activity results not only in weaker and thinner ligaments, but also in weaker attachment to the bones of the respective joint. So, in the neuro hand/ wrist that is positioned in prolonged flexion; can we assume that the ligaments get weaker and thinner? Is this the reason why we see wrist instability and "clunking" when attempting to range a wrist from flexion into extension? Can we also assume that this also happens in the extensor expansion with flexed digits which then cause joint deformities such as the swan neck and boutonniere deformities? http://www.ucdenver.edu//Musculo/8LBP-18-Ligaments-JEK.pdf
