24.01.2022 Everyday battler tip #1... Don't throw out that pillow... yet! One of the most common issues we are seeing now in patients is lower neck pain that radiates across the shoulders-- often leading to headaches!! These pains often originate from poor sleeping postures and we hear patients blaming this on an incorrect pillow selection for their body shape (we'll deal with sleeping positions and pillows next week).... The first question however I'll always ask is: "what were you doing BEFORE you went to bed".... not before you went to sleep but when you were; - watching TV slumped on a recliner - reading that book on your lap for 2 hours - knitting/sewing? - and of course playing on your phone The fact is, there are very few pre-bed activities that involve our poor necks sitting in their perfect alignment-- ie. chin tucked in, ears in line with shoulders. Once we are looking down at anything for a prolonged period the weight of our heads (which is upwards of 5kg) is being held by our upper shoulder and neck muscles.... these muscles are not designed for this purpose. The weight of our heads should be evenly distributed on our bones-- between our 7 vertebrae with muscles only acting as stabilisers. Prolonged overuse of these muscles creates thickening and tightness that turns into pain and spasms when you are resting through the night..... the result, a long, uncomfortable day the next day. SO, before you head out to replace your pillow for the 4th time, or return for your 17th treatment to the local massage parlour, try breaking up the pre-bed routine. Ideally spending no more than 20-30 mins at a time looking down, broken up with some time looking up to the roof, or at least walk a few laps of the house. The best neck stretch before bed I find is to lie flat, with no pillows at all, gently tuck your chin into the mattress and feel the stretch right up the back of your neck into the base of your skull-- hold a few of these for 30 seconds and you'll ease a lot of the day's tension! If you've had a crack at the above techniques and still find yourself waking up stiff and sore, physio can be a great way to relieve the pain and stiffness, correct postures, stretch the tight bits and strengthen the weak bits! Massage, joint mobilisation and desensitisation techniques and dry needling are a few of the treatments that compliment the advice above Feel free to share with your phone addict partners/friends/teenagers too!! 22.01.2022 G’day all... Quick Update on After Hours Physio... It’s been 18 months since we sold our home and attached Physio clinic in Matilda St Lennox Head.... Our DA was finally approved last week for a new build in Elevation Drive. After Hours Physio will hopefully be back in Lennox by early next year! Yay! We will be very excited to welcome new and current clients to our new attached clinic in Elevation Drive. In the meantime, Andrew is continuing to treat patients in Cumbalum. 21.01.2022 North Coast After Hours Physio is relocating to Alstonville! Andrew and I have sold our property in Lennox Head and will be residing in Alstonville while we rebuild in Lennox during 2019. From Tuesday 13th March the Physio clinic will be available in Alstonville with an evening appointment at 6:30pm and weekends on call. 13.01.2022 Battler tip #3... My knee is doomed for surgery... Too often as a Physio we’ll hear our more mature aged patients talking about a worn out knee. A knee that is bone on bone and buying time til surgery... ... Surgical options to assist with an arthritic knee are advancing rapidly however there is also a growing level of evidence for avoiding knee surgery through conservative management measures. Osteoarthritis comes with a whole range of co-morbidities that can be managed through consultation with a Physiotherapist... - weakness of knee muscles will increase instability leading to increased pain - stiffness of the knee joint will exacerbate inflammation - poor stability of the feet and inappropriate footwear will increase joint forces in the knee - Poor mechanics around the hip or ankle joints will increase pressure through your knee These issues are just a snapshot of what a comprehensive Physio review will assess with a worn out knee. The success of conservatively managing knee arthritis will often lead to significant pain relief, increased strength and endurance with daily activities, with the ideal outcome being postponement of surgical intervention or the possibility of avoiding the knife all together!!! 05.01.2022 Everyday battler tip #2 So how am I supposed to sleep...? This question inevitably comes up every time I start talking to a patient about neck/shoulder pain and associated headaches and muscle tightness. ... The correct answer?? Each person’s perfect position is different depending on numerous factors that a Physiotherapist will assess by looking at your natural spinal curvature and posture, muscle bulk, shoulder width and flexibility. Here are a few simple tips worth considering; *Is your preference side lying, or on your back. These are the only 2 positions I encourage as any version of stomach sleeping generally leads to excessive rotation of your neck for prolonged periods through the night. - if you’re lying on your side you’ll need a pillow supportive enough to fill the space between you neck and the tip of your shoulder (allowing your spine to remain straight through the night). - if you’re a back sleeper ideally you’ll only use a very thin pillow, as the perfect neck position in supine lying is actually NO pillow at all! - if you switch between the 2 positions then my advice is to use a thin pillow under a slightly thicker pillow so you can easily remove one should you want to roll onto your back through the night *The firmness of your mattress If you have a softer mattress you’ll need a firmer or higher pillow to accommodate your body weight sinking into the mattress. A firmer mattress usually allows for a thinner or softer pillow to keep your spine straight. *Get your partner to assess your sleeping position It doesn’t take a university degree to pick up a crooked spine. Make sure your head is in line with the centre of your spine and you’re not twisting by having one leg crossed over the other. Don’t forget about the last battler tip pain in the morning more often than not comes from your pre-bed activities not your sleeping position. Hopefully combining these 2 posts will help you wake up pain free and ready for the day! If not, your Physiotherapist has the skills to assess and guide you back to a happy night’s sleep again!
