20.01.2022 My dentist wants me to see an ENT. Whats that all about? Many patients are sent to us by dentists. That kind of makes sense on a basic level because dentists a...nd ENTs look inside the mouth. So it is no surprise they may find something wrong. In fact they will often find things well before medical doctors do because of the attention to detail when it comes to looking inside the mouth. Before we go into the long list of what dentists are worried about, think about this for a moment- you would not come to see us expecting us to know much about diabetes (we do know a bit). Its not what we focus on. Likewise, most medically trained doctors know next to nothing about teeth. If you were to ask a medical doctor what the causes of teeth grinding are, what the effect of mouth breathing on gingival health is, what molar incisor hypomineralisation is, and so forth then we are certain youll get a blank look, just like you would get from us if you asked us about diabetes. So we should all be comfortable knowing nobody can know everything about everything. Quite simply, medical doctors have next to no education about teeth. Even our principal Associate Professor David McIntosh will admit to that! Thats why we have dentists! But what you may not appreciate is just how much dentists know about general health conditions. Dentists will often spot things that a medial doctor would not even know to look for but nonetheless relates to some form of medical condition. So with no further delay here is a comprehensive but not totally inclusive list of conditions that dentists have identified and referred to us over the past 12 months alone: Snoring Allergic rhinitis (hay fever) Tongue tie Deviated nasal septum Silent sinus syndrome (a condition that if missed can lead to the eyeball falling down into your cheek sinus) Large adenoids Large tonsils Throat cancer Oral papilloma (warts) Choanal stenosis- blockage at the back of the nose Sleep disordered breathing Inverting papilloma (a sinus condition that can turn cancerous) Reflux Adenoiditis Tonsillitis Sinusitis caused by dental infection Teeth grinding (from low oxygen levels) Sleep talking (from low oxygen levels) Night terrors (from low oxygen levels) Sleep walking (from low oxygen levels) Obstructive sleep apnoea Mandibular retrognathia (short lower jaw) Maxillary constriction (narrow upper jaw) Opinions for treatment planning in kids that need braces Eustachian tube dysfunction Otitis media Vertigo Tonsil stones Mandibular tori (bony growth on the lower jaw) Maxillary tori (bone growth on the upper jaw) Lip tie Fungal sinusitis Dry mouth Neck lumps Bad breath Vertigo And there is still even more!!! Wow! So as you can see, your dentist is a valuable person. And it is not just dentists of course- it is dental therapists and hygienists who also pick these problems and raise the alarm that things need assessing. So dont be surprised in the slightest if your dentist advocates seeing an ENT. In fact many parents are very grateful a problem was found as they have struggled to know why their child had something going on but it was not quite right.

17.01.2022 Dear loyal Now Dental patients, With the increasingly widespread situation of COVID-19, we would like to advise you that everyone in the Now Dental team (receptionists dentists and dental assistants) will be wearing surgical masks at all times. Here at Now Dental we are doing our very best to ensure that our infection control is not compromised in any way.... All patients entering the surgery will be required to use hand sanitiser, rinse with mouthwash with antiseptic and have their temperature checked. There will be no handshake during this time. We want you to know we are doing everything possible to ensure our practice is a safe environment for all patients and staff. Also we ask that you please inform our receptionist if you have recently travelled to any overseas destination or if you have been in contact with anyone that has been unwell. We are screening all patients, and any at risk patients will be asked to stay home for at least 2 weeks. In addition to our normal infection control methods, we have removed all books, magazines, leaflets and toys from our waiting room to minimise the spread of infection through touch. Waiting room chairs have been placed wide apart and if the waiting room is full, accompanying persons will be asked to wait outside or in their cars. We are routinely disinfecting all clinical areas, as well as our waiting room. Thank you for your understanding during this time.

15.01.2022 Sometimes I just want to cry. I had a mum see me and she brought in this photo. What you are seeing is a young child (11 months) who is so obstructed by their t...onsils that to try and breathe their chest wall is being sucked in and collapsing on itself due to excessive muscular activity. This child was dismissed by a GP as being OK because they had not had tonsillitis. This is so very wrong and is a common and repeated experience. I am all for collegiality and mutual respect but this is just not right. This child is in extremis. Please take note: 1. Tonsillectomy is most commonly done due to obstruction. 2. Paediatric ENTs could not care less if a child has never had tonsillitis if they are obstructed. 3. They dont need to have sleep apnoea to warrant surgery 4. They dont grow into their tonsils 5. This child is at risk of heart failure and brain damage 6. They are not too young for surgery. 7. They dont need a sleep study. 8. It is a category 1 or 2 condition needing urgent attention within a few weeks- even 6 months is too long 9. See a paediatric ENT that focuses on airway problems and can quote the literature when providing an opinion. 10. Dont ignore snoring or mouth breathing in kids. Im going to spend the next month posting on this. Enough is enough when cases like this are being dismissed. This post needs to go viral. Please make it so. Kids need your help.

09.01.2022 These are some of the problems that we look out for at every child's dental visit. Facial bone structures are almost 80% formed by the age of 10 years old. To ensure normal development and optimum facial aesthetics, we evaluate habits such as tongue thrust, tongue ties, mouth breathing and lip trap habits. Airway and sleep issues in children can lead to impaired cognitive development and behavioural issues which can often be misdiagnosed as other conditions. Kids should n...ot snore or have noisy breathing at night. It's normal and an ENT assessment is something we can organise for you. We work closely with Oral Myologists, Allergy specialists and ENTs to ensure normal facial development and breathing is on track and intervene with early orthodontics when required. We can always move teeth but within the bones guided by correct facial development. Early visits to us can really make a difference to people's lives.

Its a method of stratification of likely risk based on 8 criteria, as follows: Snore Lou...dly enough to be heard outside of the room or your getting thumped by your bed partner to quieten down. Tired often, sleepy during the day Observed periods of stopping breathing or choking or gasping for breath Pressure - High Blood Pressure Body Mass Index more than 35 (your weight in kilograms dived by your height in centimetres and then divided again by your height in centimetres) Age is over 50 Neck size measurement around your Adams apple is 43cm or more in men, and 41cm or more in women Gender is male. Score one point for each question that is a yes. If you score 0-2, its a low risk, 3-4 is an middle ground risk. You are high risk if you scored 5-8 points. You are also high risk if you scored yes to 2 or more STOP questions and yes to any one of the following: male, BMI more than 35, or large neck circumference. Lots of this is covered in more detail in the book!

04.01.2022 Dr Erich is away again this weekend. This time learning how to take blood, spin it in our centrifuge and extract the cream of the blood called A-PRF and i-PRF. These products can be extracted from your blood to assist in accelerated healing in sockets, promote bone growth & soft tissue growth and even be used as natural dermal filler for facial cosmetic injectables. In TMJs, elbows and knees, it also has been shown to regenerate cartilage and speed healing of tennis elbow. But at Now Dental we will stick to what we know i.e. teeth and faces!
