16.01.2022 Aberfan Mine Disaster 1966 - Wales - killed 116 children and 28 adults. The Aberfan disaster was the catastrophic collapse of a colliery spoil tip at around 9:15 am on 21 October 1966. The tip had been created on a mountain slope above the Welsh village of Aberfan, near Merthyr Tydfil, and overlaid a natural spring. A period of heavy rain led to a build-up of water within the tip which caused it to suddenly slide downhill as a slurry, killing 116 children and 28 adults as it ...engulfed the local junior school and other buildings. The tip was the responsibility of the National Coal Board (NCB), and the subsequent inquiry placed the blame for the disaster on the organisation and nine named employees. There were seven spoil tips on the slopes above Aberfan; Tip 7the one that slipped onto the villagewas begun in 1958 and, at the time of the disaster, was 111 feet (34 m) high. In contravention of the NCBs official procedures, the tip was partly based on ground from which water springs emerged. After three weeks of heavy rain the tip was saturated and approximately 140,000 cubic yards (110,000 m3) of spoil slipped down the side of the hill and onto the Pantglas area of the village. The main building hit was Pantglas Junior School, where lessons had just begun; 5 teachers and 109 children were killed in the school. See more

12.01.2022 Queensland Police have negotiated with a group of anti-mining protestors to unblock a public road six kilometres from our Carmichael mine site, which had been b...locked since Monday. We understand the protestors who were onsite are said to be from various anti-fossil fuel protest groups including, Extinction Rebellion, who is known for their Brisbane CBD street closure protests, Frontline Action on Coal, some of whose associates were recently charged with alleged assault of one of our contractors during a protest, and a minority faction of the Wangan & Jagalingou (W&J) People who have been involved in a series of failed legal challenges. The protest has had no impact on our project this week, with workers able to access the mine and rail sites via alternative access points. We have been working closely with all of the Traditional Owner groups on our project for many years, including with legitimate native title claimants from the W & J native title party who voted 294 to 1 in support of the Indigenous Land Use agreement with Adani in 2016.

11.01.2022 ICAM Lead Investigator Course | 2 Days | $1,195!! Regular Courses Australia Wide. The OHSA ICAM Incident Investigation Lead Investigator Course has been developed based on the Incident Cause Analysis Method developed by Professor James T Reason in his research on Human Error and its contribution to incidents. It is now globally recognised as a peak incident investigation methodology and is widely used in industry for safety related incidents. Many concepts such as the Swiss C...heese Model, Active and Latent Failures etc have now become part of the safety vocabulary of incident investigation. Numerous industries and many major construction and resources clients require their supervisors, managers and safety officers to be competent in using this methodology for incident investigations. OHSA has been teaching industry this methodology for over 20 years. Our facilitators of this course have conducted 100s of incident investigations for aviation, energy, rail, road, mining, construction and emergency services. We have been commissioned by industry, government, transport regulators, coroners and courts to either lead or contribute to these incident investigations. OHSA has developed this course so that you can easily understand the concepts such as the 5 Whys, Timeline, the PEEPO Data Collection Matrix (People, Equipment, Environment, Process & Organisation) and the four occurrence causal factor elements including Absent / Failed Defences, Individual / Team Actions, Task / Environment Factors and Organisational Factors and thereby practically apply them to your workplace incident investigation. Our course facilitator has investigated fatalities, mine explosions, near miss rail disasters, serious transport incidents and hundreds of common law personal injury cases. He holds a Masters Degree, has developed curriculum for University, lectured at University, advised Ministers and Commissioners, consulted around the globe and has been invited to many conferences over the past 20 years and was recently requested to convene a safety conference for serious plant incidents for the forestry industry (as recently as 24&25th July 2017 at Marysville Victoria)!

01.01.2022 Another tragic death. Seven (7) Qld Mining fatalities in 18months.

01.01.2022 OHSA recently spent the week conducting safety training at Osborne Mine QLD. Snakes in the pool? Yep. Apparently death adders! Love the colours of the landscape in North West Qld. Shift handovers at the airport due to plane turnaround requirements means you have to be organised communicators. Our consultants number come up for both breathalyser & urine drug test! Perhaps because he ate too much Rum & Raisin ice cream! He was though a recipient of the pineapple award. Great week. Thanks to the Osborne team!
