Foot orthotics alter the biomechanics of your feet to correct, offload and/or support your feet to reduce pain, heal an injury and generally increase your quality of life. Custom foot orthotics are bespoke devices made specifically for your foot and your condition. This means they will be tailored exactly to you and not created based on a generic template.... At Orthotics Plus we are able to customise the foot orthotic to suit your footwear, lifestyle and presentation depending on the outcome of the assessment we provide. Foot orthotics can be broken into 3 sections and the options available at each segment can be used in combination. The design and features used will depend on your assessment and what we are aiming to achieve. Here are some common features of a custom foot orthotic: Hindfoot Heel cupping, selective offloading (ie for heel spurs), wedging, height increase (to alter ankle pitch or accommodate for a leg length discrepancy). India has joined us part time at our Malvern clinic in addition to her secretarial role at a busy Rheumatology clinic just up the road at Cabrini. India says she enjoys working both roles simultaneously, as it has been a great opportunity to pick up a wealth of knowledge from both medical specialities. Outside of her clinical duties, India’s major passion is MUSIC. She has been singing as... long as she can remember and has performed professionally for 7 years. In addition to her busy schedule, India also runs private vocal classes for both children and adults in the evenings. 23.01.2022 Conservative Management for Patello-Femoral Osteo Arthritis: Wearing of the underside of the patella against the trochlear groove of the femur can cause inflammation and eventually osteo arthritis of the patello-femoral joint. Tracking of the patella is clinically difficult to define as there is significant variation in patella shape, height relative to the trochlear groove and movement of the patella. The patella is only in contact with the femur from about 20 degr...ees of flexion through to full flexion. In knee extension it can be palpated and moved quite a lot. In the PFOA knee, preventing normal ROM (range of motion) is not the goal, but reducing extraneous movement can offer some reduction of pain. Normal pronation of the foot during weight bearing will create a small amount of rotation through the ankle joint which is reflected somewhat to the alignment of the knee. Orthotic management of the PFOA knee can include foot orthotics to address the pronation of the foot or internal shoe wedges to adjust ankle alignment. There is recent clinical evidence to suggest that arch support does assist PFOA pain. Another orthotic approach can be knee bracing with a lateral buttress for patella support. More often used for patella instability or subluxation, the lateral buttress and compressive knee sleeve has shown to be beneficial in some patients in relieving pain. 22.01.2022 Poliomyelitis (polio) is a neurological disease caused by infection with the poliovirus. It can damage the nerves which control muscles, causing weakness and even paralysis most often affecting the legs, but also arms, chest and back. A vaccine was introduced to Australia in 1956, and since 2000 Australia has been declared polio-free. 25-50% of polio survivors will experience a return of physical symptoms 15-30 years after initial recovery. This is known as Post-Polio Sy...ndrome. Symptoms can include weakness, muscle atrophy, fatigue, pain in the muscles and/or joints, joint deformity, changes in gait pattern (i.e. limping) and an increased tendency to fall. Many experiencing the residual effects of polio will require the use of an orthosis to help provide stability and improve mobility. Orthoses can compensate for changes in muscle length and function, realign skeletal joints, redistribute pressure and optimise gait pattern. They can also reduce the risk of a trip or fall and improve energy efficiency. Orthoses are most frequently applied for lower limb muscle weakness and joint deformity, however they can also be used for spinal, upper limb and abdominal muscle weakness and deformity. 20.01.2022 Functional Electrical Stimulation (FES) devices are technology which generate muscle movement at a desired area by directly transmitting a low-level electrical impulse to the nerve to activate a weakened, damaged or paralysed muscle. This in turn helps to improve capability and mobility. These devices are wearable, battery powered and specifically target a muscle or muscle group at a specific time to increase function. An example of this is an FES device for foot d...rop. The device will sense when the patient is lifting their foot from the ground and stimulate the common peroneal nerve to activate the muscles which generate lift (dorsiflexion) at the ankle. Depending on position and tuning of the device we can also adjust the amount of eversion (moving the foot away from body at the ankle joint). 20.01.2022 This patient had abdominal surgery in 2008 and since then has experienced muscular weakness, unsteadiness and regular falls. He presented to us with bilateral foot drop (worse on right side) and unable to walk without assistance. This shock absorbing material is much more comfortable for the foot whilst still allowing excellent support and control to be built into the orthotic. The density of the foam will depend on the patient and what we are trying to achieve. The properties of EVA allow us the flexibility to make small modifications during your fitting appointm...ents to completely tailor the foot orthotics to your footwear and your feet. 18.01.2022 FOOTWEAR SELECTION FOR DIABETICS People with diabetes are prone to foot problems due to peripheral neuropathy (nerve damage) and poor circulation. When left untreated, complications such as developing ulcers and/or charcot neuropathy can develop (see previous 2 posts!). Correct conservative management is therefore a must, and wearing specially designed footwear can help reduce risk, promote healthy circulation and allow the foot to maintain its shape, structure and sha...pe. Management includes: Extra width/depth footwear or custom made footwear (seamless) Custom made foot orthotics When fitting footwear to a diabetic patient, it is essential that there is no risk of pressure or rubbing that could cause skin breakdown and ulcers to develop. Ulceration can occur in as little as just a few hours! Therefore its important to make sure the shoes are seamless on the inside and that the width and depth is appropriate for the patient so as not to increase pressure. Making sure the shoes are made from an appropriate material is also important. Sometimes a lower risk stretchy material is needed as leather may not be forgiving enough. For diabetic patients with a significant foot deformity, custom shoes may be necessary. This is particularly so if the patient requires an orthotic to offload a large area as the extra width/depth footwear may not actually be wide or deep enough to accommodate both an orthotic and deformed foot appropriately. 18.01.2022 Sesamoiditis is a chronic stress injury caused by frequent pressure resulting in irritation and painful inflammation in the tissues surrounding the tibial and fibular sesamoids (the tiny bones embedded in the tendon of the big toe). It is generally felt as a gradual building of pain in the ball of the foot. Individuals may experience difficulty and pain when trying to bend or straighten the big toe. Swelling, redness and bruising may also be present. Sesamoiditis co...mmonly affects runners, basketballers and dancers and those frequently participating in activities that require a lot of pressure to be placed on the ball of the foot (i.e. crouching and heavy lifting). Wearing high heels and hard-soled business shoes can also increase pressure on the ball of the foot. Individuals with prominent foot bones, high arches and flat feet may also be more prone to sesamoiditis. It’s important that patients experiencing Sesamoiditis seek early treatment to prevent the problem and pain from worsening. Treatment is generally non-operative. Orthotics Plus can assist with a selection of comfortable and supportive footwear and custom foot orthotics to alleviate pressure from the sesamoid bones. Shoes should have a wide toe box so that toes aren’t cramped, and should also be soft-soled and low-heeled. Most patients will experience immediate improvement in symptoms once the pressure on the sesamoids has been offloaded using custom orthotics. If there are no complications, complete resolution can be achieved within months. 18.01.2022 Orthotic intervention is a proven and effective conservative treatment option for people suffering from osteoarthritis and rheumatoid arthritis to help reduce pain and improve mobility. It may also delay the need for surgical intervention. For example, Osteoarthritis sufferers experiencing pain and knee malalignment may benefit from the use of foot orthotics which alter knee alignment by reducing the knee abduction and adduction moment, thereby reducing pain. Foot orthoti...cs are more effective when treatment is started early. Knee orthoses such as the Unloader One Lite (mild to moderate arthritis) and Unloader One X (moderate to severe arthritis) are also great options for offloading one side of the knee joint to reduce malalignment and pain. Softer circumferential knee sleeves such as the GenuTrain Knee Brace may also assist to reduce persistent knee pain and swelling by providing compression and a feeling of stability. 18.01.2022 Do I need foot orthotics? The answer to this question is generally based on these factors: Pain... If you have a high level of lower limb and foot pain, your pain is worsening, or your foot pain has been persisting for a longer time, we may be more inclined to suggest foot orthotics. If your foot pain is low and reducing then we may suggest other options first. Age If you are younger and growing, or older and not healing as well, or the pain is limiting your mobility, foot orthotics may be required. Biomechanics An assessment of the structure and function of your foot will assist this decision. If during the assessment and gait analysis we note issues we can immediately address, foot orthotics may be a good option for you. (e.g. hyper mobility, flat feet, very high arched feet, pronated foot posture, bony prominences, painful rigid joints, osteoarthritis, foot structures which cause torsional forces in the foot). History What have you done previously? Have you tried other options or changed your footwear recently? Have you recently started training for a marathon? Are you noticing your feet change shape? The answers to these questions will assist us to determine a course of action. If there are things we can modify in your life such as activity level we will advise accordingly. 17.01.2022 CAM walkers have been walking out the door lately! You might know them as CAM Boots or Moon Boots . Their primary purpose is to prevent or limit ankle and foot movement after a foot or ankle fracture, ligament sprain or to help progress out of a cast. They are often used instead of a cast and can be removed for showering and small range of motion exercises if instructed by the treating doctor. But beware the cheaper versions available online!... If CAM walkers are not made from the right materials and professionally measured and fitted, they will not be effective and can lead to further damage . This can also create a multitude of other issues including lower back pain, knee issues, hip issues, contralateral (opposite) foot pain or even pressure injuries. It is not uncommon for a patient to wear a CAM walker for over 8 weeks a long time to be wearing a poorly fitted or uncomfortable device! 16.01.2022 Ankle sprains can be the bane of an athletes season or even for the every-day person whos stepped on a crack in the foothpath! Painful and often reoccurring , orthotic management is an effective strategy to reduce the risk of sustaining a follow up injury or series of sprains. Braces such as the ASO or Formfit Ankle brace (pictured) use figure 8 strapping to allow the foot to move in a natural way but prevent excessive supination (rolling outwards) which is ...the movement commonly associated with an ankle injury. The British Journal of Sports Medicine @bjsm_bmj published a study showing that bracing was more effective than neuromuscular training to prevent lateral ankle sprains reoccurring, however we would suggest both effective rehabilitation and orthotic management for a patient looking to return to full function. When bracing its important to have the correct size fit with no excess space around the heel, and the figure-8 straps to be done firmly. We suggest tightening the pronatory strap last and doing this very firm to prevent as much supination as possible. 16.01.2022 How do foot orthotics work? The human foot is made up of 26 bones, 33 joints and over 100 tendons, ligaments and muscles. This structure is remarkable in its function, however things do go wrong. The result may be acute pain, inefficient biomechanics, stress injuries, chronic issues or bad posture of the foot leading to ankle, knee, hip and back pain amongst other problems. If we provide support and correction to the foot, we can address many issues by protecting or redu...cing load on the stressed tissues. By supporting the bony structures of the foot we can offload tired overworked muscles and encourage the activation of other muscles which may not be as active due to the malalignment. Foot orthotics will not magically heal your issue, only your body can do that, but we will encourage an environment which will make this possible. Most patients do notice immediate pain relief when wearing their foot orthotics. Everything is connected! For example, a flat foot which overly pronates (rolls inwards) may suffer from aching arches and lateral (outside) ankle impingements locally, but the flow on effect internally rotates the tibia, causes the knees to dip inwards (valgus malalignment) and the pelvis to drop. The gluteal muscles are then required to work harder to prevent internal rotation and adduction at the femur which could aggravate lower back pain. Some patients with knee osteoarthritis find foot orthotics and suitable footwear will reduce some pain in their knees as the ground reaction force is shifted to the less damaged region of their knee. Foot orthotics are best used in conjunction with suitable footwear, which your orthotist can advise you on. Foot orthotics can be custom made or pre-fabricated. 16.01.2022 Paul suffered a stroke 2 years ago, which left him with right-sided hemiplegia. He presented to us with medial-lateral ankle instability, foot drop and non-symmetrical gait. Our Senior Orthotist, Lynne, trialled him with a Bioness FES device (refer to the ON video ) and was able to achieve a much faster and more confident gait with heel strike, no slap, no ankle instability and reduced asymmetry - a very good outcome! Whilst it may not look like much to the untra...ined eye, notice in the video where the device has been turned OFF, how Pauls right foot is wobbly when he strikes the ground. The light device seen in the video is a high intensity portable blue light which rapidly cures the FlexiOH making the internal structure rigid, whilst the external structure remains cushioned and soft against the skin. This whole process takes about 3-10 min. The perforated design allows for skin ventilation, reducing cast itchiness and preventing skin infections. It’s also l...ightweight, waterproof and washable making it a good option for summer. The zip/button/strap system of these casts make for easy application. 14.01.2022 Diabetic foot ulcers are a common condition, occurring in approximately 15% of people with diabetes. There are 1.7million people with diabetes in Australia, so thats about 255,000 individuals who will experience a diabetic foot ulcer (rates of diabetes are increasing, so this number may end up higher!) Diabetic foot ulcers have lengthy healing times and a high rate of reoccurrence that may lead to lower limb amputation. Therefore, prevention and early treatment of ini...tial ulceration is imperative. Orthotists are the only allied health professionals qualified to provide a full range of lower limb orthoses for people with diabetic foot ulcers. In our arsenal we have total contact casting (TCC), CROWs, special offloading CAM walkers, custom foot orthotics and specialised footwear. This case required a non-removable TCC. (SWIPE ) 2 months after initial treatment the wound had healed enough so we transitioned the patient to a more functional device and then to custom foot orthotics with orthopaedic footwear. If managed poorly amputation was a very real possibility for this patient. Our diabetic ulcer patients are managed in collaboration with a GP and podiatrist to manage debridement, nail care and to monitor any infections. 13.01.2022 What was missed?! On occasion, Orthotics Plus will intervene in cases of Lisfranc/midfoot fractures. These can be misdiagnosed as a severe ankle sprain, mismanaged or even just missed. If a Lisfranc injury is not treated appropriately, damage can occur to the cartilage. This then increases stress on joints of the midfoot, resulting in severe flat foot and midfoot arthritis without resolution. Not a great patient outcome! Lisfranc injuries occur when the bones of the m...idfoot are separated from the bones of the forefoot (metatarsals), particularly around the 1st and 2nd metatarsals. This injury can range from simple to complex, depending on how many joints of the foot are involved. This 31 year old patient fell on her right foot whilst playing badminton. She presented to her local doctor 5 days post injury, however a standard X-ray showed no fracture. She was then referred to us for a CAM walker for pain relief. The patient presented to us with pain, moderate swelling, some plantar bruising and difficulty weight-bearing. When taking into consideration the mechanism of injury (extreme plantar flexion under axial load) and presentation of symptoms, we suggested a Lisfranc injury needed to be considered and recommended that she seek an orthopaedic opinion for further management. After a weight-bearing CT and x-ray showed widening at the Lisfranc complex, the patient underwent surgical fixation and was advised to be non weight-bearing for 6 weeks. 6 weeks post-op, we then fitted her with an Aircast boot in which she was able to weight-bear for another 6 weeks. At 12 weeks post-op we transitioned her to appropriate shoes and foot orthotics, and at 6 months her metalware was removed. www.orthoticsplus.com.au

13.01.2022 ACL CONSERVATIVE MANAGEMENT At Orthotics Plus we work with multiple Orthopaedic knee specialists, Sports Doctors and Physiotherapists to provide conservative management for ACL injuries. ACL surgery is considered on the basis of age, level of instability and lifestyle/level of activity (and Covid-19 elective surgery restrictions!)... Surgery can be avoided by ensuring strong surrounding musculature around the knee joint, however if the patient does not maintain their strength, the instability and pain will return. Symptoms would include pain, swelling and instability - a feeling of the knee giving way. The degree of injury can be measured with knee laxity tests or confirmed with MRI. Mild instability can be managed with a soft sleeve hinged knee brace for media/lateral stability and proprioceptive feedback. Greater instability including rotational instability requires a rigid frame Knee Control Orthosis (KCO) which can stabilise medio-lateral instability and rotational instability by nature of its frame design, for example, the ssur CTi or Flex. 12.01.2022 Charcot foot is a serious complication that can develop as a result of peripheral neuropathy (nerve dysfunction) and decreased circulation. It is commonly seen in those with Diabetes Mellitus. Over time, elevated blood sugar levels may cause the tissues of the body to become stiff (glycation). This includes muscles, tendons, nerves and blood vessels in the foot and ankle and even the lower leg. This combined with a reduced blood supply, which prevents healing, means the... foot is unable to maintain its normal structure, and the small bones of the foot collapse and fracture. The loss of protective sensation due to Peripheral Neuropathy means the patient may continue to walk not knowing their feet are experiencing damage. If the fractures and injury go unnoticed and are not correctly treated, severe deformity and disability can occur. The hallmark deformity associated with this condition is a rocker-bottom appearance of the foot caused by mid-foot collapse. A deformed foot is at high risk of ulceration due to high pressure areas. This can lead to breakdown, infection and even amputation. Early diagnosis of Charcot foot is extremely important. It’s imperative that diabetic patients take preventative measures and seek immediate care if the following signs or symptoms of Charcot foot are present: Foot is warm to touch Redness Swelling Pain or discomfort (may not always be present) For patients with a Charcot foot, there are several phases of pathology which require different treatments. These include: Total Contact Cast (TCC) CROW (Charcot Restraint Orthotic Walker) Custom made footwear or extra width/depth footwear Custom made foot orthoses Correct management of an active Charcot foot will allow the foot to maintain its shape, structure and function. 12.01.2022 Conservative Management of Ankle Osteo-Arthritis (OA) Ankle arthritis can significantly reduce quality of life when walking becomes painful and activities of daily life are affected by ankle pain. The joint of the ankle is a hinge in 2 parts - between the tibia, fibula and talus and further down the chain, the talus and the calcaneus. There are multiple articular surfaces within the ankle joint and ankle arthritis can be in either section of the hinge, both sections or ...focal to an area within the ankle joint complex. Osteo-Arthritis can be the result of a post traumatic change in ankle alignment or damage to the chondral surfaces, it may be wear and tear due to overuse or compensation for another injury or it could be related to postural alignment where a person has a pronation or supination of the ankle that causes uneven distribution of forces through the ankle. Before surgery is considered, it is worth trying conservative management of the ankle joint. There are levels of orthotic management that can relieve OA pain significantly, waylaying the need for surgery. Least intrusive would be appropriate, well-structured footwear with a rocker sole. There are brands of shoes and runners on the market that have these features built in. Good runners will often help with ankle and foot OA. More designs of casual shoes with these features are available now than ever before. Ankle stabilising braces will restrict ankle ROM down to a level that can be sufficient to control ankle OA pain. Foot orthotics can help with re-alignment of the structures of the foot which can offload the area affected by OA to produce a great pain free outcome. An Ankle Foot Orthosis (AFO) is often described as an ankle fusion without the surgery and is sometimes used to trial the concept of an ankle fusion and how it would affect gait by walking with a locked ankle. Although more cumbersome than the restrictive orthoses, an immobilising orthosis can be taken off at the end of the day! 12.01.2022 FES System Comparison - A Clinician’s Perspective Orthotics Plus’ senior orthotists have reviewed the currently available Functional Electrical Stimulation (FES) lower-limb gait devices in Australia. This is to assist in your research and decision-making. Our opinions and findings are not meant to be a replacement for a consultation and assessment. There are aspects of every individual presentation that will always require professional consultation.... FES uses electrical impulses to stimulate nerves to cause a muscular contraction. In a foot drop device, it focuses on the muscles to lift the foot (dorsi-flexors) and the muscles that turn the foot out (evertors) to create a natural walking pattern. Aside from the technology replacing the traditional static Ankle Foot Orthosis (AFO), the other benefits include strengthening and lengthening of disused and contracted muscles, improved blood flow, improved proprioception, improved gait symmetry and reduced energy expenditure. It also means not having to wear shoes all the time as you have to with a traditional AFO. Device cost aligns to options of fine-tuning for specific individual requirements. The lower cost FES units are single channel and have limited options in adjustable settings. 11.01.2022 Did you know that Orthotics Plus regularly provides treatment for upper limb fractures? Wrist Hand Orthoses (WHO) are commonly used to treat wrist fractures, scaphoid fractures or midshaft radius/ulna fractures. They can be fabricated to immobilise the wrist or both the wrist and thumb. WHO can either be custom made from a low temperature plastic or a high temperature plastic. A low temperature plastic WHO can be moulded directly onto the skin and finished on site usin...g velcro straps. A high temperature plastic is a stronger material which requires moulding over a plaster cast. If a high temperature WHO is required, a plaster cast must be taken and the orthosis fabricated overnight. At Orthotics Plus we also use pre-fabricated WHO if there is a simple ligamentous injury which doesnt require complete contouring to the shape of the limb. 11.01.2022 De Quervain’s Tenosynovitis is a painful condition affecting the tendons along the thumb side of the wrist (Abductor Pollicis Longus and Extensor Pollicis Brevis). Symptoms can include pain and swelling near the base of the thumb and difficulty moving and gripping whilst using your thumb and wrist. Since this is an overuse injury, repeated movements of the wrist and thumb will worsen the symptoms. A great conservative treatment option is a custom made or off the shelf Wr...ist Hand Orthosis (WHO) to immobilise all wrist and thumb movements. This will reduce movement and pain levels. 10.01.2022 The scaphoid is one of the small bones in the wrist, positioned on the thumb side. It is the most commonly fractured carpal bone, accounting for 70-80% of all carpal bone injuries, and 8% of all sports related fractures. A scaphoid fracture is usually caused by falling on an outstretched hand with the weight going through the palm. Patients present with wrist pain (particularly over the anatomic snuff box ), swelling and often with limited motion. Approximately 10-20% ...of scaphoid fractures don’t show up on plain x-rays, so a Magnetic Resonance Imaging (MRI) is required. The major complication of scaphoid fractures is non-union or mal-union which can lead to instability and secondary osteoarthritic change. Proximal, oblique or distal fractures may be more susceptible to non-union due to poor vascularity, with non-union occurring in 5-15% of cases. Generally, stable (undisplaced or minimally displaced) scaphoid fractures are conservatively managed by immobilisation of the wrist in a below elbow fibreglass cast or custom moulded plastic wrist hand orthosis for up to 12 weeks depending on the location of the fracture. It is important to start immobilisation early, as the risk of non-union increases the longer immobilisation is delayed. There are generally two different types of pre-/post-operative knee braces: Range of motion knee brace... Locked knee brace Your knee specialist will specify which brace is required and the range of motion allowed. The braces we generally use are the GII Hinged Knee Brace , Formfit Hinged Knee Brace and the Zimmer Knee Immobiliser. These are pre-made braces, which need to be customised/shaped to correctly fit the contours of your knee. If this doesn’t occur the knee brace will not be comfortable and will not control the knee correctly. 09.01.2022 Tibial Plateau Fracture Orthotic Management: Tibial plateau fractures are often caused by a side impact, seen in pedestrianvs. car accidentsor when your excited 30+kg puppy runs into your legs They can be very significant injuries, often involving the knee joint which can have more serious implications in the long term and would require surgery.... The tibial plateau is one of the major load bearing surfaces of the body. The large surface of the top of the tibia makes up the base of the knee joint. A small fracture to the medial or lateral aspect of the tibial plateau can heal without surgery, however high energy injuries will often result in the stability of the tibial plateau being compromised with the cartilage and soft tissue structures of the knee joint damaged. Surgery will most likely be required for these injuries. A small, non-displaced tibial plateau fracture can be managed with a Range of Motion knee brace. Also a ROM knee brace may be introduced post operatively for rehabilitation and recovery. A brace instead of a cast has the benefits of being removable for wound reviews and hygiene. Also, the Range of Motion knee brace has a hinge design that can be locked in one position, limited to a controlled range of movement that can be increased over time or allowed to move through the full range of knee movement. Generally for tibial plateau fractures, the brace will be locked with the knee fully extended for several weeks. 08.01.2022 How much better does that look?! Mary suffered a stroke in 2011 which resulted in right-sided hemiplegia. She presented to us with right foot drop with audible foot slap, stiff knee gait, hip hiking and circumduction during gait. She was also uncomfortable walking without an assistive device or rail. ... Our Senior Orthotist, Lynne fitted Mary with a Functional Electrical Stimulation (FES) device (this one is the Bioness L300-GO) with very good outcomes. Whilst wearing the FES, Mary was able to walk with heel strike, decreased circumduction, some knee flexion and few gait deviations. There was also a significant increase in her walking speed (approximately 60% faster!) She also reported significantly less energy expenditure when wearing the FES device and is more comfortable to walk without assistance or rail. Together with her physiotherapist at Klint Intensive Neuro Therapies , we are continuing to review Mary, and are pleased to report that several months on she is making great progress with stamina and different terrains. 08.01.2022 Painful bunions are more common in women than they are in men, but they do occur for reasons other than wearing high-heeled shoes! Mostly, bunions are hereditary. Poor footwear can exacerbate the bunion prone foot to create painful, inflamed bunions. The term bunion mostly refers to the 1st Metatarsal Phalangeal Joint (MTPJ), but the 5th MTPJ can also develop a bunionette or Tailors Bunion. The Great Toe or Hallux shifting its alignment such that the 1st metatars...al abducts away from the midline of the foot is referred to Hallux Abducto Valgus (HAV) or Hallux Valgus. Foot structures that are low or flat in the arch, or pronated through the ankle and midfoot, are more likely to develop HAV. In severe cases, surgery is recommended for pain relief. Surgery will change the foot structure to relieve pressure, but often choices about appropriate footwear are still required after surgery. Conservative management of bunions can provide excellent pain management without the hospital stay and 6 weeks off your feet plus the risks of wound infection. Custom foot orthotics are designed to take some of the load of the body off the 1st MTPJ to relieve pain. Depth/width footwear that will accommodate the foot orthotics and allow the toes to sit without pressure or restriction are simple ways to relieve bunion pain. Runners are the most easily accessible and generally accepted footwear that is appropriate for relieving pressure on bunions, but fortunately there is a growing range of nicer shoe options that will allow for orthotics and good foot posture. There are some silicon products that help with sheering and rubbing over bunions that can be very helpful such as toe spacers, bunions shields or gel pads. But the jury is out on night splints generally they help reduce pain when worn at night time, but they cannot be used inside a shoe and the general view is that they do not provide a long term solution. 06.01.2022 How much better does this look?! Knee Ankle Foot Orthoses (KAFOs) are orthoses that encompass (yes, you guessed it!) the knee, ankle and joints of the feet. They are commonly prescribed to people that have muscle deficiencies which result in the inability to the keep the leg stable whilst standing. In the absence of a KAFO, an inefficient and unsafe walking pattern can ensue- or the patient may not be able to walk at all. Different styles and variations of KAFO...s include: Free-swinging (knee is unrestricted) Locked (knee is always locked) Stance control (locked when weight is put through the orthosis and unrestricted when the affected leg is swinging) Stance and swing control (leg is controlled when weight is put through the orthosis and also when the affected leg swings through) Historically the most commonly prescribed KAFOs were locked and free-swinging. This was in part due to a lack of technology available and funding limitations. Whilst they have their benefits, they are a far cry from producing a natural and safe walking pattern. In order to produce the safest and most functional walking pattern, a stance and swing control orthosis, such as the C-Brace, needs to be considered. The C-Brace has a computer controlled knee joint that can allow the user to move in a more natural way on all terrains (flat surfaces, uneven terrain and stairs and ramps). Importantly, it controls the leg when you put weight through it, but allows it to bend (as opposed to keeping it locked straight) so helps to preserve the joints in your body and allow more natural movement. It is for this reason, it has become the gold standard in KAFO management. 06.01.2022 Have you met Kate Perlstein, Senior Orthotist and Director of Orthotics Plus? Kate graduated with a Bachelor of Prosthetics and Orthotics from La Trobe University in 2004. After graduating, Kate was immediately offered a one year graduate position at St. Vincents Public Hospital. Throughout this year, Kate was exposed to complex musculoskeletal conditions (particularly Polio) and complex custom made orthotics. This included custom foot orthoses, custom ankle foot o...rthoses and custom made ground reaction ankle foot orthoses. St Vincents Hospital is one of the only Orthotic departments in Australia to use carbon fiber and fiberglass laminates almost exclusively to create their orthoses. In 2005, Kate commenced practicing at the Alfred Hospital . During this time, she was exposed to acute trauma patients (particularly spinal fractures), some of which were life threatening. The Alfred is one of only two hospitals in Melbourne specialising in the fitting of halo-thoracic orthoses for treatment of complex cervical (neck) injuries. The orthotics department was also in charge of the outpatient fracture clinic where she gained significant experience fitting fracture casts and fracture orthoses. In 2008, Kate started work in a private orthotic facility. The primary motivator for a move to private practice was the opportunity for greater exposure to foot and ankle conditions and interaction with Orthopaedic surgeons, Rheumatologists and other referring doctors . 06.01.2022 Bioness have 2 models of lower limb FES units currently available in Australia. The L100-GO and the recently released L300-GO. The L100-GO is the simplified model of the Bioness range and as such is cheaper. It is a single channel unit and has an optional footplate for greater refinement of gait. The L300-GO, is arguably the top of the line of all FES units currently available and also carries the highest price tag. It has multi-channel electrode options, does not requi...re a footplate to get finer gait adjustments due to its gyroscopic technology, and has an option (at an additional cost) for a knee stabilising thigh cuff attachment which can be used for quadriceps or hamstrings stimulation. This thigh cuff is functional without the lower leg unit if there is no footdrop presentation. Importantly, it offers the largest range of options for a customised positioning of electrodes and level of stimulus to offer the most flexibility for varying individual anatomies and presentation. We find the Bioness system to be the most responsive unit for people taking sidesteps, walking backwards and performing atypical steps that cheaper units may not be capable of correctly interpreting. This can be an important feature for safety and specific activities. The Bioness system is also able to be controlled by an app on the patient’s phone. Training modes are available with both Bioness devices which can build strength and endurance and improve blood flow in the affected muscle groups when at rest. 04.01.2022 An Olecranon Fracture is a fracture of the elbow that can occur as a result of direct impact or when falling on an outstretched arm, where the triceps tendon pulls the olecranon away from the ulna. An Olecranon Fracture can be very painful and make elbow motion difficult or impossible. Depending on severity of the injury and on direction from the referring surgeon, a hinged elbow brace may be required to reintroduce and gradually increase elbow range of motion. ... A ROM brace such as the Innovator X consists of a forearm and upper shell with a joint in between. This joint can be set in various positions depending on the stage of rehabilitation. Other conservative management treatments for an Olecranon Fracture may include a custom moulded backslab or shoulder immobiliser. In an acute patient, with the relevant history, this would likely be a sign of a PCL rupture and would indicate management with assistance of an orthopaedic surgeon and potentially an orthosis such as the Össur Rebound PCL brace to reduce strain on the healing PCL. ... In this patient’s case it is due to a PCL which never healed after a traumatic incident. It will be managed via a rigid carbon fibre device to provide more stability long term. The goal is to enable more stable gait with less pain and give the patient greater confidence in the knee.

03.01.2022 Will foot orthotics weaken my feet? No. This is a common myth. When used correctly foot orthotics do not ‘take over’ the role of muscles in your feet, leaving them lazy and ineffective. They correct a biomechanical issue and rebalance and normalise the muscle activity in your feet. Many studies and even a systematic review have found a more normalised muscle activity in patients with a flat foot deformity when using foot orthotics.... When aligned correctly the foot will work more efficiently as the muscles and joints are not at the extreme end range of their possible range of motion. Muscles generally work more efficiently and can produce more strength during the middle of the range of motion of the respective joint they control. Additionally a foot which has a corrected posture will provide a better and efficient structure for human gait during running, walking or sports. 03.01.2022 Achilles injuries are often associated with sudden bursts of speed or changes in direction when playing sport , but they can also occur under less physical circumstances such as diabetes or use of some steroidal medications. The level or grade of injury will generally fall within 3 grades: Grade 1 a minor tear with pain and some muscle fibre damage but no loss of strength or ROM.... Grade 2 a partial tear causing pain and restricting strength and ROM. Grade 3 a full rupture whereby there is a distance between the ends of the tendon and can be detected with palpation and a visible depression in the region of the rupture and a loss of active plantar flexion. Treatment options depend on age, activity level and the severity of the injury. Younger, more active people are better candidates for surgery and older more sedentary people will often be managed conservatively. The benefit of conservative management being the removal of risks associated with surgery such as wound infection. There is great debate regarding surgical versus conservative management of Achilles injuries. It mostly comes down to the rate of re-rupture in the post injury population. High level athletes will lean toward surgery for the lowest chance of re-rupture, however there is growing support for conservative management as with functional rehabilitation the rates of re-rupture are similar to surgical outcomes. Severity of the rupture will be a deciding factor as with larger gaps between the ends of the tendon, it may be necessary to surgically intervene. Grafting is considered when the distance between the ends of the tendon is greater than 3cm. 02.01.2022 Who can wear an FES device and how do I know if I am suitable? Functional Electrical Stimulation (FES) devices are suitable for people with muscular weakness, loss of co-ordination or paralysis due to upper motor neurone issues. An upper motor neurone issue is an issue which is limited to the brain or spinal cord. A patient with a lower motor neurone lesion (such as nerve injury in the extremity) is not generally suitable for FES as the pathway the electrical impulse a...ct along is interrupted and can’t conduct the signal regardless of an internal or external generation. Interestingly, even if the nerve is severed beyond the spinal cord but above the site of stimulation the stimulation will generally not work. We require a complete circuit from muscle to spinal cord to complete the stimulation, and the nerve will generally cease to function due to the damage to the structures which provide the cells with the necessary resources for cellular maintenance. At Orthotics Plus we generally use functional electrical stimulation systems for patients with: Foot drop Quadriceps weakness Upper limb weakness Our goals will always align with those of the patient, but most people work with us seeking to function more effectively, gain strength, rehabilitate more effectively and gain independence. 01.01.2022 What was missed?! On occasion, Orthotics Plus will intervene in cases of Lisfranc/midfoot fractures. These can be misdiagnosed as a severe ankle sprain, mismanaged or even just missed. If a Lisfranc injury is not treated appropriately, damage can occur to the cartilage. This then increases stress on joints of the midfoot, resulting in severe flat foot and midfoot arthritis without resolution. Not a great patient outcome! Lisfranc injuries occur when the bones of the m...idfoot are separated from the bones of the forefoot (metatarsals), particularly around the 1st and 2nd metatarsals. This injury can range from simple to complex, depending on how many joints of the foot are involved. This 31 year old patient fell on her right foot whilst playing badminton. She presented to her local doctor 5 days post injury, however a standard X-ray showed no fracture. She was then referred to us for a CAM walker for pain relief. The patient presented to us with pain, moderate swelling, some plantar bruising and difficulty weight-bearing. When taking into consideration the mechanism of injury (extreme plantar flexion under axial load) and presentation of symptoms, we suggested a Lisfranc injury needed to be considered and recommended that she seek an orthopaedic opinion for further management. After a weight-bearing CT and x-ray showed widening at the Lisfranc complex, the patient underwent surgical fixation and was advised to be non weight-bearing for 6 weeks. 6 weeks post-op, we then fitted her with an Aircast boot in which she was able to weight-bear for another 6 weeks. At 12 weeks post-op we transitioned her to appropriate shoes and foot orthotics, and at 6 months her metalware was removed. www.orthoticsplus.com.au
