13.01.2022 | Fentanyl | Emergency Indications Pain Induction to / maintenance of anesthesia ... Sedation for pacing / cardioversion Administration IN, IM, IV, SC, transdermal, IO Pharmacodynamics Fentanyl activates opioid receptors in the nervous system (particularly Mu receptors) to produce pain relief. Fentanyl is an opioid agonist which can slow the conduction velocity of the AV node (negative dromotopic effect) & cause depression of the respiratory controls in the brain. Pharmacokinetics Fentanyl is lipid soluble, meaning that it is rapidly absorbed (this is why it can be administered via IN route). Fentanyl is metabolised by the liver & excreted mostly via the kidneys. Adverse Effects CNS: Decreased GCS, pupil constriction CV: Bradycardia, hypotension Resp: Respiratory depression (decreased respiratory rate, decreased tidal volume, decreased response to hypercapnea) GI: Nausea, vomiting Skin: Pruritis, urticaria associated with histamine release Clinical Pearls Fentanyl is considered the analgesic agent of choice in the following settings; severe headache, hypotensive pt, nauseated or vomiting pt, contraindication to morphine Fentanyl has a serotonergic action. Consider this in the context of overdose where serotonergic agent(s) are suspected (SSRI, amphetamine, MDMA etc) Fentanyl is the preferred opioid analgesic for any patient with moderate to severe renal failure (acute or chronic) as it is mainly metabolised in the liver to inactive metabolites The presence of other CNS depressants (benzodiazepines, alcohol, some hypnotic/sedative agents) can increase the risk or severity of adverse effects Wooden Chest Syndrome (sudden severe rigidity in key respiratory muscles of the chest wall, diaphragm, and upper airway) is associated with rapid IV administration of fentanyl References https://go.drugbank.com/drugs/DB00813 https://litfl.com/pharm-101-fentanyl/ https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6863461/ https://pubmed.ncbi.nlm.nih.gov/25860200/

11.01.2022 A quick search can yield upwards of 50 different types of cognitive bias attributed to the medical field. Let's discuss a few specifics instances that lead to m...edical diagnostic error and thus detrimental patient treatment and outcome. COGNITIVE BIASES--> generally speaking, "systematic errors in thinking that influence decision making and judgment" . TYPES OF COGNITIVE BIAS 1. ANCHORING: rely too heavily on one piece of information or idea usually the first. Example-> patient has abdominal pain and must be biliary because I saw a stone but fail to consider the aorti aneurysm. "I made up my mind that this is the diagnosis or treatment no matter what" 2. CONFIRMATION: tendency to look for information that confirms one's preconceptions, often while dismissing information that may challenge them. Example: the radiologist read said no appendicitis despite me not looking at the film and considering the exam/labs/HPI. Its late in my shift and I don't want to make the consult. CT read confirmed. Done. 3. WISHFUL THINKING: idea that people believe in what they want to be true. This bias could cause someone to overestimate the rewards while underestimating the risks of certain decisions. 4. AVAILABILITY HEURISTIC: tendency to overestimate the likelihood of events that are more readily available in one's memories 5. IMPLICIT BIAS: refers to the attitudes or stereotypes that affect our understanding, actions, and decisions in an unconscious manner. NOTE: this is more clinical/direct patient care bias being discussed. There is also bias directly associated with research. There are also many other types of bias I did not fully cover that are more subtle. TEAM: it's an interesting topic to think and talk about. Even more crucial for us to have personal insight into our own bias while offering patient care.
