23.01.2022 Another case of arrogant response to having failed a patient. Today, a very happy family came to see me. The girl had virtually stopped wetting ... which had b...een twice each day ... at the age of 10. She had an anatomical anomaly that was the main cause of her wetting - her anatomy has been EASILY corrected, recently. 4 years ago she was seen by a high profile surgeon in Melbourne, but, despite several infections, she did not have a repeat ultrasound, she did not have a complete physical examination and he mother was told the girl had a behavioral problem. The family have lived with the lack of easy resolution in belief they had seen "the expert". When the hospital and surgeon involved were contacted about the error in diagnosis, there was no response from the surgeon, the medical director, the CEO, nor the complaints department. The girl wrote a card of thanks stating, "I have a whole new life". We had met two weeks ago. Errors in medicine are not well managed! Thus, this message is on facebook ... hoping to encourage AHPRA to protect the public. And, for families with girls with wetting and infection - there may be a solution; there should be thorough review and repeat investigation.
20.01.2022 A positive outcome of staying at home ... a common problem for girls from after the age of 3 years is that they development bladder spasms, resulting in wetting... and urine infections. Again this week I have seen two 10 year old girls with 7 years of unhappiness because of the bladder problem ... not being diagnosed. Being at home makes it easy for girls to go to the toilet more regularly, which is part of the treatment. If a girl has had a urine infection, she should have an ultrasound (even if only one proven infection); if a girl has more than one infection and a normal ultrasound previously, she should have a repeat ultrasound. Wetting in young girls can be improved, usually with simple measures, and mainly by understanding the bladder spasm phenomenon, but ... not always. 14.01.2022 Wait until he is 3 or 4 to unroll his foreskin, as advice for the care of a boy doesn't sound right does it? Today I saw a 2.5 yo boy who had had multiple episodes of urine infection and blood from his non-retracting foreskin. The advice to the family had been - it will roll back when he is older. His foreskin was obviously abnormal on examination. Steroid ointment might enable his foreskin to retract, but a repeat kidney ultrasound should, and will, be done. And, if medical treatment fails, surgical care would be appropriate. Boys should be reviewed if they have a urine infection and the foreskin narrowing should be managed, which steroid ointment usually fixes.
08.01.2022 Let's make medicine safer. Again, in the last week, I have seen two boys who have had significant problems with their foreskin because the parents have not been given the confidence to gently but thoroughly care for their boy's uncircumcised penis - just one of the many issues that indicates that Australian healthcare needs to improve; much of the improvement will come from prevention, not treatment.
07.01.2022 Hydronephrosis ... what does it mean? To an 11 yo boy it meant three operations within one month ... but two functioning kidneys and a happy family as a result ...of the surgery - he presented with no symptoms, except for a painless abdominal lump the size of a rock melon. Hydronephrosis is urine seen on ultrasound in the system that collects urine (calyces and renal pelvis) from the kidney, then directs the urine to the bladder, via the ureter (the tube between the kidney and bladder). The 11 yo boy, without surgery, would have ended up in renal failure. Hydronephrosis is poorly managed at present - patients who require little care get over-investigated; some patients with on-going damage to their kidney are not actively treated ... all because the investigation and management is too often not tailored to the specific findings for the patient. All of medicine deserved the attention to detail (but not the hysteria) that is being directed to the the current viral pandemic.

05.01.2022 Paediatric Surgical update: A hydrocele is a collection of fluid around the testicle. In boys who have the fluid from birth, and the scrotal distension is minimal, no surgery is needed. Careful evaluation is needed to ensure there is not a groin hernia! A boy who presents with a hydrocele after the age of 2 years, most likely will need an minor operation that is like fixing a hernia, which is a minimal operation in a child.
01.01.2022 Another community health update. News ..... every boy is different! As a Paediatric surgeon, you come to understand that the appearance of the scrotum and tes...tes of boys varies greatly. And, that an ultrasound looking for the testicles often is misleading, because like "balls" they are able to "bounce" into the superficial inguinal pouch. This "bouncing" is normal - and it can be stimulated by touching the thigh; and is a normal reflex. So, if you are wondering if your son's testicles are normal, look for them, rather than feel for them; if you need medical advice, seek an expert, not an ultrasound! I have seem many families who have become worried about the ultrasound result of their boy's testicles, when there is no cause for concern.
