12.01.2022 Lets discuss why oxygen therapy causes hypercarbia in some patients with COPD. Spoiler alert: it has NOTHING to do with suppressing the hypoxic drive to breat...h. Think about it how many times do your patients STOP BREATHING when receiving O2 via facemark or nasal cannula? This is a dogmatic teaching with no physiologic backing! The REAL reasons are: V/Q MISMATCH and the HALDANE EFFECT. . Our lungs do a phenomenal job of matching ventilation (V) to perfusion (Q) at the alveolar level. The pulmonary arterioles supplying poorly ventilated alveolar units undergo hypoxic pulmonary vasoconstriction (HPV) to shunt blood to better ventilated areas thereby optimizing V/Q matching. COPD creates changes in lung architecture. Patients compensate for this with HPV to optimize gas exchange; however, supplemental O2 is a powerful pulmonary arteriole dilator. Vessels which were previously constricted are now dilated stealing blood from ventilated areas. In a sense, this REVERSES HPV and creates V/Q MISMATCH with resulting gas exchange impairment (ie, hypercarbia). . The second reason is the Haldane effect! Hemoglobin not only carries O2 (HbO2) but serves as a physiologic buffer by carrying protons (H+Hb). When oxygen is added, the proton is kicked off as shown in the equilibrium: [H+Hb] + [O2] [H+] + [HbO2]. . This free proton then combines with bicarbonate (HCO3-) to create carbonic acid which then splits into water and CO2! By adding more H+, we shift the following equilibrium to the right to produce more CO2 (aka, hypercarbia). [H+] + [HCO3] [H2CO3] [CO2] + [H2O] . 06.01.2022 On Tuesdays CoronaCast, John Steenhuisen was joined by Paediatric Pulmonologist, Dr Fiona Kritzinger, from the Christiaan Barnard Memorial Hospitals Chest and... Allergy Centre. She highlighted the following key information for parents, in support of calls for children to be allowed to go safely back to school: 1 Less than 3% of Covid-19 infections are in children under the age of 18. Children are infected at a far lower rate than the adult community, and are far less likely to have serious symptoms. 2 Children rarely infect other people. Studies show in only 10% of households, a child was responsible for spreading the virus, in 90% of cases it was spread by adults. 3 There is no data to say that closing schools in particular is extremely helpful on a population level to stop the transmission of the virus. 4 Last year, of the 11 000 influenza deaths in the country, about 550, or 5%, were school going children, far higher than the death rate we are currently seeing with Covid-19, yet last winter schools did not enforce mask wearing, flu vaccines, symptom screening, temperature taking or hand washing. Given the screening and safety measures now put in place by schools, the relative risk to children currently is no greater than the same time last year. 5 The benefits for children of returning to school, including for their education, safety, nutrition and mental health, far outweighs the risk of infection.

05.01.2022 Following Healthy Lifestyle Recommendations May Reduce ADHD Risk Children who meet a greater number of established recommendations for a healthy lifestyle at ag...e 10 or 11 are less likely to receive a diagnosis of attention-deficit/hyperactivity disorder (ADHD) by age 14, suggests a study published in Psychosomatic Medicine. The incidence of ADHD among children and youth is high, and temporal increases have been paralleled by deteriorating lifestyles. Poor diet quality, physical inactivity, poor sleep habits, and sedentary behaviors have all been associated with ADHD, researchers wrote. However, no earlier prospective study has examined the independent and combined importance of meeting established lifestyle recommendations in childhood for ADHD in adolescence. Researchers collected survey information on lifestyle for 3436 children ages 10 and 11 as well as linked health data until age 14. They examined the data for associations between adherence to 9 specific lifestyle recommendations and ADHD diagnosis and physician visits.
