The care can give patients up to 5 visits for allied health services, including podiatry services. Chronic medical conditions are those that have been or are likely to be, present for at least six months. A patient is considered to have complex care need...s if they require ongoing care from a multidisciplinary team consisting of their GP or medical practitioner and at least two other health or care providers. Examples include: asthma cancer cardiovascular disease diabetes kidney disease musculoskeletal conditions stroke.
