25.01.2022 At the end of week 2 stage 4 restrictions...thanks to all those being patient, continuing with programs and waiting until COVID under control again before follow up We will be ready at the end to take up where we left off! We are really glad to be able to have helped with new problems and urgent follow up. We will continue to be available to prevent issues escalating to something worse during stage 4.....it often just takes a simple explanation and some understanding to kickstart the road to improvement and recovery. 25.01.2022 PERINEAL MASSAGE PRIOR TO VAGINAL DELIVERY What is perineal massage? ... Perineal massage is a type of massage done to the muscles at the entrance to the vagina during the late stages of a pregnancy. It is done to try and prevent tears or the need for episiotomy during a vaginal delivery. The muscles at the entrance of the vagina are called the superficial pelvic floor muscles meaning that that are a little closer to the surface than the deeper pelvic floor. Just like anywhere else in the body, muscles tend to form layers, each muscle working closely with another muscle nearby, deeper or nearer the surface. The superficial pelvic floor, works with the deeper pelvic floor when doing well known pelvic floor exercises. The superficial pelvic floor muscles extend from the area around the bladder outlet, the vagina and to the back passage. There are also bands of muscle that extend out towards the ischial tuberosities, commonly known as the sit bones. Yesterday we had our own health promotion activity where by we were going to do pelvic floor exercises every time we washed hands...how did we go? 18.01.2022 Musculoskeletal pelvic pain Coccyx pain Coccyx pain can be very persistent simply because sitting makes it worse and we have to sit in life! Lots of regular Physios will offer help for this problem, but sometimes the problem is just too deep for these physios to get to. We are able to assess and treat the coccyx directly. Pubic bone... This is the bone at the front of the pelvis. The bone is actually a joint at the front between the two halves of the pelvis. The pelvic floor muscles attach to the back of this bone so any problem in this area may need a detailed look at pelvic floor. Sacro-iliac joints These are the two joints at the back of the pelvis, under your glute muscles. Without going into too much detail, if this is the area of your persistent pain, we can assess and help guide you. Problems in this area may cause pelvic floor issues and vice versa. Vaginal pain We see people with all sorts of strange pains that just feel like they’re in the vagina! 16.01.2022 CANDICE GRAETZ, APAM BPHTY, PG CERT PHYSIO (PELVIC FLOOR REHAB) I graduated from Monash University with a Bachelor of Physiotherapy in 2011, and have been lucky enough to work on the Mornington Peninsula since. ... In 2016 I completed post-graduate studies in Pelvic Floor Rehabilitation and Musculoskeletal Conditions in Women at the University of Melbourne. My motivation was to gain a greater understanding of the pelvic floor and pelvis so that I could help my clients more effectively. After graduation, I gained great experience working within a Continence Clinic, working with patients experiencing issues with bladder and bowel, as well as pelvic pain and pelvic floor dysfunction. Today it is confirmed via the Australian Physiotherapy Association after consultation with DHHS, that you do not need a Doctor referral to attend Physiotherapy during stage 4. Some children may be dry at certain age and bed wetting restarts later in life or some children have never been able to achieve dryness. During this time, I discovered my passion for Womens Health and Continence Physiotherapy and started further training in 2014, undertaking training with the Royal Womens Hospital which culminated in completing a Masters Degree in Womens Health and Continence Physiot...herapy in 2016 from Curtin University, Perth. 15.01.2022 FASCIA OF THE PELVIC FLOOR A little history Fascia has long been considered the forgotten part of our musculoskeletal system. As Physiotherapists, we learn in minute detail every part of the bone and every ligament that attaches two bones together. We learn every part of each muscle and how it attaches to the bones via tendons. We learn how each tendon attaches and transfers load of a muscle to produce human movement. We learn the details of every nerve and how the...y travel from brain, via spinal cord, out to the periphery of our body and bring messages back from the world to be processed by the brain to enable us to function. We learn about the organs of the body: the brain, the heart, the liver, the kidneys, the lungs, the reproductive organs and so on and on. Admittedly I trained as a Physiotherapist a while ago, graduating in 1993, but I have spent every year of my career attending professional development which is a prerequisite of being a Registered Physiotherapist. Every year we attend conferences, workshops, lectures to keep learning as new things about the human body are found out and researched. We have to keep up to date. But rarely have I learnt updates on FASCIA!! It took me until now to find the first Australian Fascia Symposium which I attended recently. Years ago, we were taught it existed, it was there almost as a rather inert structure around muscles: it was almost described like the packaging of the muscles: just surrounding them and holding them all together. 14.01.2022 At 30 weeks, Cee demonstrates some simple exercise to tone up your leg muscles to help anyone struggling with the limitations of lockdown. Its worth having some good strength in preparation for an active labour ...or whatever happens with the delivery, you need some good strength in quads and gluts with the lifting and carrying of your new baby . If you have pelvic girdle pain, its best to have an assessment of whats best for you first, but for most uncomplicated pregnancies, these should help. Have fun!!

13.01.2022 Stage 4 update: with new restrictions, like everyone else, we are scrambling to read through guidelines and be able to continue in line with new rules. At the moment we are able to stay open and we ARE OPEN in Dromana and Frankston. It is looking likely that Candice will be available for consultation in Dromana and Caroline and Cee at Frankston. This is due to restriction on work across multiple sites. We think a G.P. referral will be necessary to be able to attend if you havent attended before. The aim is to push for women to have access to better, more consistent, pelvic health Physio after the delivery of their baby. In some countries, women get government assistance to these services. Lack of finance or lack of knowledge should NOT be a barrier to Women receiving services.

As we move to week 2 it has been made clear to us that we are permitted to have face to face consultations for essential treatment. Essential treatment is treatment that prevents someone from getting worse and therefore needing increased care elsewhere. Do remember that we can offer Telehealth for anyone in need of care but not able or eligible to attend in person. It is embarrassing to talk about. But our job is to be comfortable helping you sort this problem out. Men can experience testicular pain, anal pain, perineal pain, penile pain. All of these pains might have their source in tight pelvic floor. Many people are familiar with trigger points in tight muscles e...lsewhere in the body. The pelvic floor is just muscle like any other muscle in the body. If the muscles hold too much tension, over time, painful trigger points develop and cause pain. See prostatitis above. 03.01.2022 The papers are full of ideas for simple exercise to help get through restrictions and isolation. As weve said before, when doing any other exercise, pelvic floor can be combined. With these 3 whole body stretches, those needing to relax their pelvic floor can focus on the pelvic floor release between each "whole body" stretch. You may ask "who needs to relax their pelvic floor? Well it tends to be those whose muscles are tight and so cause pelvic pain OR those who have tendency to tension everywhere in the body and may just find pelvic floor tightening is easier after relaxing first....its a complex thing! But suffice to say, a general stretch might feel good for us all, and why not pay attention to pelvic floor at the same time! #pelvicfloorexercises #simpleexercise #pelvicfloorphysio
