20.01.2022 A review of 41 research studies has shown that "suspected LGA" (or being told that your baby is bigger than average), is only weakly predictive of the risk of s...houlder dystocia and that routine ultrasound to check the size of a baby in the third trimester could do more harm than good. Researchers looked at these studies becuase there is a growing trend towards recommending induction of labour for a variety of reasons, even though there isn't always good evidence to support this. Induction also carries risks and side effects of its own, and many women who experience induction of labour are not happy with the experience. This is especially the case at the moment, when many are still being told that they cannot have partners with them until they are in established labour. With induction, it can take up to three days for labour to become established, especially in first-time mums. This review looked at whether identifying larger than average babies by ultrasound was an effective way of predicting problems at birth, notably shoulder dystocia. They found that, although universal third-trimester ultrasound screening will result in more women being told that their babies are larger-than-average, it will not have a clinically significant effect at predicting shoulder dystocia. The authors of the review, "recommend caution prior to introducing universal third-trimester screening for macrosomia, as it would increase the rates of intervention, with potential iatrogenic harm, without clear evidence that it would reduce neonatal morbidity. Newborn head molding is an (temporary + safe) abnormal head shape that results from pressure on the baby's head during childbirth. The bones of a newborn baby's skull are soft and flexible, with gaps between the plates of bone. The spaces between the bony plates of the skull are called cranial sutures. The front (anterior) and back (posterior) fontanelles are 2 gaps that are particularly large. These are the soft spots you can feel when you touch the top of your baby's head. When a baby is born in a head-first position, pressure on the head in the birth canal may mold the head into an oblong shape. These spaces between the bones allow the baby's head to change shape. Depending on the amount and length of pressure, the skull bones may even overlap. These spaces also allow the brain to grow inside the skull bones. They will close as the brain reaches its full size. Fluid may also collect in the baby's scalp (caput succedaneum), or blood may collect beneath the scalp (cephalohematoma). This may further distort the shape and appearance of the baby's head. 08.01.2022 Perth Hills Bushfire It has been a tiring couple of days with the start of these catastrophic fires on Monday. All focus has been on updates and emergency apps with choppers flying overhead frequently and no power. We are still located within the red emergency zone, but very blessed to be in a little pocket that appears will remain untouched. Whilst the wind has provided us with protection is has seen many others lose their homes. And this is not over for many who are only ...beginning their evacuation process My thoughts and prayers go out to those we know, and don't know who have lost everything. Much appreciation to all of the emergency service staff and those working in the background to hopefully get this under control soon

04.01.2022 I think a CTG is just another tool use to control and manipulate women in labour and birth... AND as protection against litigation. Continuous CTG (cardiotocogr...aphy) is when a CTG machine - with two receiver discs, 2 waist straps and 2 cords that feed back to the machine- are attached to a labouring woman for the duration of her labour and birth in order to get a print out of her babies heart rate. It’s happening all the time as a result of birthplace culture and reduces the woman’s comfort, access to water during labour, limits movement and most importantly makes very little difference to the outcomes for the baby, whilst increasing the chances that the woman will undergo major intervention. @who doesn’t recommend it, the research is against it... even @birthsmalltalk who has a PhD on this can’t confidently say that CTG’s have a beneficial place in labour care at this time. What we know is that we don’t know the benefits of the use of CTG, research hasn’t found anything of significance, except that they are bad for women without creating a significant improvement for the well-being if babies. What are the alternatives? The use of intermittent auscultation (listening in) with a hand held Doppler is another option and is recommended by @who as a suitable alternative... the world health organisation states, ‘in settings where continuous CTG is used defensively to protect against litigation, all stakeholders should be made aware that this practice is not evidence-based and does not improve birth outcomes’ @lifeandlens_photography @lifeandlens_kin #cardiotocography #midwife #obstetrics #undisturbedbirth #homebirth #hospitalbirth See more
