04.01.2022 Im yet to be convinced of the efficacy of stem cell therapy in the treatment of knee osteoarthritis. Im particularly concerned by doctors advocating treatment...s that they have a strong financial interest in and very slick purpose-built clinics that are developed around a single unsubstantiated treatment modality. Patients are easily misled by flash-looking clinics and websites and feel that if a treatment is expensive and cutting-edge, it must be effective. The BJSM have published a review of 6 trials and concluded that all 6 were heavily biased. The conclusion states "In the absence of high-level evidence, we do not recommend stem cell therapy for knee osteoarthritis". http://bjsm.bmj.com/conte//2017/03/03/bjsports-2016-096793 The ACSEP Australasian College of Sport and Exercise Physicians has a position statement that only advocates stem cell use "As part of a rigorous clinical research trial." http://www.acsep.org.au//Stem%20Cells%20Position%20BJSM.PDF There have been reported deaths in Australia associated with the stem cell harvesting process and cases where tumors have developed following treatment (with uncertain association). Osteoarthritis is a large-scale problem resulting in widespread morbidity. If a safe effective treatment emerges that helps with joint regeneration I will eagerly adopt its use (and be one if its greatest advocates). But being an early-adopter in medicine has historically been fraught with danger.

01.01.2022 MANIPULATION AND MOBILIZATION VS. ACTIVE COMPARATORS (INCL. EXERCISE AND PHYSICAL THERAPY) IN CHRONIC LOW BACK PAIN (FREE FULL-TEXT SYSTEMATIC REVIEW AND META-...ANALYSIS) This brand-new systematic review and meta-analyis of Coulter et al. (2018) compared the effects of manipulation and mobilization versus active comparators including exercise therapy and physical therapy in patients with chronic (> 3 months) low back pain patients. They found a small-moderate effect in favor of manipulation and mobilization in terms of pain (SMD = -0.28) and disability (SMD= -0.33). This effect seems to increase over time at 3 (SMD=0.68) and 6 months (SMD= 0.72) follow-up! Subgroup analyses showed that manipulation alone significantly reduced pain(SMD=0.43) and disability (SMD=0.86), compared with other active comparators including exercise and physical therapy. The body of evidence was moderate overall. Studies remain heterogenous in terms of dose, duration, techniques involved. Clinical heterogeneity between study groups remains a confounder. In studies that defined chronic pain as 12 months or more, there is insufficient evidence to draw any conclusions from the current literature. Evidence to support mobilization intervention does not seem to be as strong as evidence to support manipulation intervention for chronic low back pain. What are your thoughts on this systematic review? We find the results pretty surprising given that earlier evidence suggests that the effect of manual therapy - especially in the chronic low back pain group - is low! Think we have to dive deeper into the inlcuded RCTs here... Very curious about your input as this is always a very heated discussion!!!
