This study provides new and important information on the effects of resuscitation with an intact umbilical cord. The findings of improved SpO2 and higher Apgar score, and the absence of negative consequences encourages further studies with longer follow-up. Delayed cord clamping should be the norm now for all babies. Nature is very clever This is what the Australian study found... We found that it was possible to do CPR while the umbilical cord was still intact, and if cord clamping was delayed, it may reduce injury to the brain, he said. Our main finding was that performing CPR with the umbilical cord intact, and then delaying cord clamping for up to 10 minutes, prevented a large overshoot in blood pressure and flow to the brain, which likely reduces bleeding. 16.01.2022 For so long Obstetricians have blamed vaginal birth for pelvic floor dysfunction but may be it is the way we have been managing vaginal birth for so long that is the problem Whilst the ’hands off’ approach in this study isn’t really hands off it is interesting that the women were told to exhale and relax during the contraction rather than the usual ’push,push’. Although I still think it would be better to just trust women to instinctively protect their own pelvic floor th...is is a start. This bit is worth knowing too. 14.01.2022 Happy World Breastfeeding Week . This is a photo of my first feed with my last baby . I was lucky to have no problems breastfeeding my 4 babies however I do think it was the words and actions of my very first midwife which set me up beautifully for my successful breastfeeding journey.... . She told me I was a natural and kept her hands off with my very first breastfeed. . That gave me enormous confidence and self-efficacy. . Today I am off to help 2 mums who didnt get such a positive start as they left hospital confused and unsure of themselves because of all the different advise they got from different midwives . 13.01.2022 A great read. Written by Dr Kirsten Small who is an academic and an obstetrician. #1 and #2 are good too. Wrapping up Non-maleficence (often illustrated by the Latin phrase primum non nocere, or first do no harm Smith, 2005) is a core ethical principle in health care, requiring clinicians to ensure that the benefits clearly outweigh the harms of any planned intervention. There is no doubt at all that there is limited benefit from the use of intrapartum CTG monitoring and clear evidence of harm. As clinicians in maternity care our discussions with women need to go well beyond lets just pop the monitor on to be on the safe side. In order to begin having honest conversations with birthing women, we first need to start with honest conversations within our professions about the failure of CTG monitoring to delivery on its promises.

08.01.2022 We were discussing what happens if a baby passes meconium before it is born in class on Sunday. Here is the article I referred to. https://midwifethinking.com//the-curse-of-meconium-st/amp/... So you would think that the sensible thing to do if a baby has passed meconium (for whatever reason) is to create conditions that are least likely to result in asphyxia and MAS. This is where I get confused because common practice is to do things that are known to cause hypoxia, for example: Inducing labour if the waters have broken (with meconium present) and there are no contractions or if labour is slow in an attempt to get the baby out of the uterus quickly. Performing an ARM (breaking the waters) to see if there is meconium in the waters when there are concerns about the fetal heart rate. Creating concern and stress in the mother which can reduce the blood flow to the placenta. Directed pushing to speed up the birth. Having extra people in the room (paediatricians), bright lights and medical resus equipment which may stress the mother and reduce oxytocin release. Cutting the umbilical cord before the placenta has finished supporting the transition to breathing in order to hand the baby to the paediatrician.

I love teaching this training day because not only will my Hypnobirthing Australia clients get amazing support during their births but I know these midwives of the future will be able to help all birthing women find their birthing power. . I am also a huge fan of Nicky Leaps and we discussed her fantastic paper Working with pai...n in labour. . I know people think hypnobirthing teaches no pain but that is not what the Positive Birth Program by Hypnobirthing Australia is all about. It is about help couples (and midwives) understand the physiology of labour and birth and how we can work with pain (and therefore release endorphins) for the benefit of our bodies and our babies. . Hi all...we have our doula training practice day on Monday 14th September and if you are 38 weeks or more and would be able to donate your time to helping out with doula practice that would be great. ... You would need to be willing to spend 2 and a half hours having a combination of massage, acupressure, a variety of doula skills including working with a rebozo, pelvic pathways positioning / stretching and receiving labour and birth information. We need 8 women for the morning session from 9:30am till 12pm, then another 8 women for the afternoon session from 2pm till 4:30pm (we cannot change these times so you must be available these times). 02.01.2022 I discuss the ‘perineal bundle’ which has been introduced in public hospitals in Perth in my classes. This is important research from th UK to know about in giving informed consent. "The prevention of severe perineal trauma for 3 in 1000 women who gave birth vaginally in hospitals after the bundle was introduced isn’t really a stunning reduction in incidence. And I wonder if birthing women would accept having their midwife or doctor handling their perineum during birth and la...ter performing a rectal examination as an appropriate cost to pay for this reduction."
