25.01.2022 I thought it may be a better idea rather than us post information that may or may not be of interest to the public, that instead, we ask you to post any questions pertaining to exercise and health to which we will respond with the latest evidence based advice... So feel free to post a question and we can start a forum.



21.01.2022 So now all jokes aside about my recent surgery, the true reasons behind the posts are to demonstrate that one of the most important aspects of surgery is prehabilitation; it is what an individual does prior to surgery that has the greatest impact, not necessarily what you do after (albeit that is important also). Make no mistake, I spent many hours with an ice pack on or laying down with leg elevated, but its about getting the right balance between movement and rest. Everyone responds differently, but if you have a pending surgery, don't leave it until after the surgery to commence rehabilitation...

17.01.2022 It is with a heavy heart that on Friday we farewelled Hayls (Hayley) who has decided to take some time for herself and family. Hayls started in 2018 as a practicum student which resulted in full time employment. Hayley was the consumate professional and was highly regarded by all she treated; we wish Hayley and her family all the best for the future.

13.01.2022 7 days post-surgery and surgeon follow up consult... - nil crutches required - 90 deg. Knee flexion - back to riding quad bike and ride on mower - stationary bike... - upper body resistance - blower vac and pressure cleaning ...ahead of schedule

11.01.2022 I have been a bit slack and haven't posted any rehab progresses for a while and several inquires have been forthcoming, so.... Latest consult with surgeon went extremely well (5 weeks): 1) screws haven't moved and exactly where he put them 2) bone growth (re-bonding) is on track 3) Patella re-alignment is perfect.... 4) Nil need for leg brace anymore 5) See him again in 6 weeks; extremely satisfied with progress/rehab Routine up to 4 weeks consisted of: 1) mobilisation w/- foam roller 2) 20 minutes on Recumbent/stationary bike 3) 4 sets x 12 reps Leg Press (seated or 45 deg.) minimal weight i.e. 30kg 4) 3 sets x 15 reps Calf Presses (approx. 30kg) w/- manual knee extension x 20 sec 5) 3 sets x 12 reps x Cable machine/Powerband Terminal Knee Extensions 6) 3 sets x 20 secs x Stability ball Isometric Glute Bridge (single leg) 7) 45-50 minutes NMES (Neuromuscular Electrical Stimulation Commenced the following sequence of exercises approximately 1 week ago (5 weeks rehab)... 1) 5-10 minutes warm-up bike 2) 4 sets x 15 reps Leg Press x 59kg 3) 3 sets x 12 reps/leg x unilateral step-ups (FFSU's) 3 risers 4) 3 sets x 12 reps/leg x unilateral step-downs (FFSD's) 1 riser 5) 3 sets x 12 reps Decline Squats 6) 3 sets x 12 reps Leg Curls (80lbs) 7) 3 sets x 15 reps x Terminal Knee Extensions 8) 3 sets x 12 reps Stability ball Leg Curls w/- single leg glute bridge x 30 sec hold 9) 3 sets x 15 reps x Calf Presses x 59kg

01.01.2022 FYI: for those suffering from T2DM... A strong link has been observed between glucose control and COVID-19 outcomes among hospitalized patients with preexisting T2D. Among 952 patients with COVID-19 and T2D, 282 individuals had "well-controlled" blood glucose, ranging from 3.9 to 10 mmol/L (median, 6.4 mmol/L [115 mg/dL]) and an A1c level of 7.3%. Those with "poorly controlled" blood glucose, defined as the lowest fasting glucose level 3.9 mmol/L or above and the highest 2-...hour postprandial glucose > 10 mmol/L (median, 10.9 mmol/L [196 mg/dL]) and A1c level of 8.1%, had a higher mortality rate. More than 1 in 10 people with diabetes hospitalized with COVID-19 die within 1 week, and nearly one third require mechanical ventilation, new research shows. Data from the French Coronavirus SARS-CoV-2 and Diabetes Outcomes (CORONADO) study also reveal that body mass index (BMI) was independently associated with death or intubation at 7 days, whereas A1c level and use of renin-angiotensin-aldosterone system blockers and dipeptidyl peptidase 4 (DPP-4) inhibitors were not. A retrospective study by Chen and colleagues of 136 patients with COVID-19 and T2D found that older age, elevated C-reactive protein level, and insulin use were risk factors for mortality. The adjusted odds ratio for mortality in insulin use was 3.58. Whether insulin itself is a risk factor or whether the increased mortality reflects the characteristics of the patients taking it is unclear. Women taking metformin may be at lower risk for fatal COVID-19, according to a recent study. Among more than 6200 adults with diabetes or obesity who were hospitalized for COVID-19, fewer deaths were reported among women who had filled their 90-day metformin prescriptions. After adjustment for other risk factors, these women were 21%-24% less likely to die of COVID-19. This link was not observed among men. Although the risk for death from COVID-19 is significantly increased in people with both main types of diabetes, early findings suggest that the highest risk is associated with T1D.
