25.01.2022 I was taught and have taught that chest grippers are External Oblique (EO) dominant. But I’ve been thinking about it...bear with me here... 1. EO attaches from ribs 5-12 to the semilunaris... 2. Therefore, EO can only pull on that oblique angle only so far 3. If that 80-90deg infrasternal angle is to be narrowed, can external oblique actually cause that to happen? 4. Urquhart et al 2005 reported upper fibres of EO are about 49deg +/- 7deg from the horizontal in 24 subjects. 5. It gets complicated with the angles that the bony ribs sit vs the cartilage that forms the rest of the ribs and the infrasternal angle. Also, it’s important to remember that the rib cage is a 3D structure. That being said, if the line of pull of EO can only bring the ribs towards the semilunaris, how much can it approximate the infrasternal angle? 6. If the obliques (EO and IO), thoracolumbar abd thoracopelvic erector spinae (ES), Quadratus Lumborum (QL) are the main rib cage depressing muscles, then the intercostal muscles can also join in. But why does EO get such a bad rap then!? 8. EO can only pull to its attachments - so the fibres that attach to the ribs 5-9 attach to tissue. Only ribs 10-12 attach to the iliac crest. So even that 10th rib to iliac crest can only pull medially in a limited way 9. Upper fibres of TrAb are in prime position to contribute to narrowing of the infrasternal angle. Why don’t we say this muscle is the culprit? Anyway, I’m just musing. Do you have any thoughts? Let me know below. Feel free to explain to me why I might be wrong. 09.01.2022 Have you considered the effect of your advice and cues? I’ve been going through the videos for #thefemalesthletemembership and have been finding that so much of our well-meaning advice leads to unintended consequences like people fearful to move or lift with bad technique. How to you test for your cues and advice?... By what standard are we guaging what is helpful? How do we test? This week alone has had some new clients really concerned about things like a one finger width #diastasisrecti and with so much conflicting, super-conservative advice out there with devil-may-care advice, it’s confusing. So let’s keep moving towards what we want to see. In early postpartum pelvic health, from a purely physical point of view, that means we look for hard, stretched out anterior abdominal walls and bearing down in the pelvic floor during activities. Yes there are more things to think about like ALL the other things that go with being in the early postpartum period - feel free to let me know what you think is crucial to add! But from an exercise and advice point of view, testing is important. Test your advice. Use the meaningful tasks for that person. As I get older, I realise more and more that isolating TrAb and even the pelvic floor () is waaaay less important than I used to think. Super important for a small % of people but not everyone needs to cue their PF and core before they do something or deliberately breathe in a certain way. Anyway, I see so many strong, capable, adaptable, resilient people being held back by fears and theories. What do you think? Have I gone too far? Where’s the balance? #endrant See more
08.01.2022 What if the main problem with the way we communicate the issues that people can face CAUSE the very problems we are hoping to avoid? Hear me out on this one... 1. We cannot possibly tease out the biological/structural, the psychological (our conscious thoughts) and the sociological (culture, context, history, upbringing etc). Each issue will affect each of the other domains and whilst it seems like there may have been a sequence of events leading up to something, how fa...r back do we go until we say *that* was where it all started...we can’t because we then go into what your mother was doing and epigenetics and then it continues back into generations and environmental impacts etc. So let’s just say we can’t tease it all out but accept they are all there as ways to interact with the system and then ensure we monitor and address the factors that seem to make the most impact first using #testretest and being mindful of confirmation bias and post hoc fallacy. 2. Right, so if we provide information about the concerns we have, how are we presenting the information? How are we checking for understanding and meaning? How are we checking to see how the person sees what that information looks like in their life? 3. Because if fear and caution is what they take away, maybe, just maybe, they will increase their protective behaviours, their tension, #overcookthecues, brace, activate their core, go slowly and carefully - all of these things can raise Intraabdominal pressure (IAP) 4. This can then become a self-fulfilling prophecy - sustained IAP may bring on the symptoms we were hoping to avoid with our advice. 5. You may be checking for this with internal pelvic floor exams and providing feedback - but what do they do when they go home? What if they bear down to get the same feeling because they don’t have your fingers inside for feedback? So, all this to say... ...please continue to be mindful of the effect of our language - they provide clues of what we believe and our attitudes to what happens.
