A wall distance of 5 inches or 12,5cm is considered a normal range of motion. Just use one hand-width as a rule of thumb. If your patient feels an anterior pinching sensation, the limitation probably lies in the joint itself and mobilizations might help. Secondly, if the patient rather describes a tension posterioly soft tissue might be the cause of the problem and targeting the soleus might resolve the limiation. To assess progress using this technique a change of 1.9cm has been described as the smallest detectable change.

Sometimes, especially in the case of ACUTE pain, the cause is indeed relatively si...mple - your toe hurts because you stubbed it, or your wrist hurts because you twisted it. As your body heals itself, the pain goes away. Simple, right? BUT, more often than not, particularly when it has persisted for a long time, pain cannot be explained in terms of simple biological processes. If you've had chronic low-back or knee pain for many months, or even years, "it's complicated" may not feel like an adequate explanation, but at least it's an honest one. If someone tries to give you a simplistic explanation for your persistent pain (especially if they're also trying to sell you a simple 'cure') be VERY skeptical. Decades of scientific research has shown us that chronic pain is influenced by a WIDE range of factors in addition to biological ones, including emotions, beliefs, expectations, past experiences and cultural narratives. Indeed, the LONGER pain persists, the LESS it tends to be associated with biological factors. That's why a multi-disciplinary approach combining physical AND psychological therapies is now considered the gold-standard evidence-based treatment for chronic pain. Don't get me wrong - it would be AMAZING if there was a simple biological explanation for your persistent pain that could be solved with a simple biological treatment.
