25.01.2022 #Mythbusting round two! Low Back Pain (LBP) is the number one cause of disability worldwide, often associated with expensive, sometimes ineffective or even harm...ful care. But why does this persist? According to new research from Peter OSullivan, JP Caneiro, Kieran OSullivan, Ivan Lin, Samantha Bunzli, Kevin Wernli and Mary OKeeffe, unhelpful beliefs and pervasive myths though they are common and culturally endorsed are not backed by evidence and are associated with greater levels of reported pain, disability, work absenteeism, medication use and healthcare seeking. To begin to peel back some of the unhelpful beliefs outlined in the research, were launching another myth-busting exercise, this time focussed on LBP. Today, were busting the notion that scans will always show the cause of back pain. In fact, scans are only helpful for a small minority of people. While a lot of conditions can be reported with a scan bulging discs, protrusions, degeneration, arthritis and so on what cant be seen is the reality that many of these conditions are also common in those without any pain at all. Advice from the Federal Minister for Health, The Hon Greg Hunt The APA has received the following advice from the Federal Minister for Health, The Hon Greg Hunt: The Australian Minister for Health has today confirmed that ALL allied health businesses can continue working and are encouraged to do so. In addition he has indicated that they are now developing full allied health telehealth options within the week for allied health along with all other health and medical services. This will be part of a staged plan. Face to face services for many remain vital so where possible, we encourage you to continue. I thank you for your courage, dedication and care, the Minister has said. It is our priority as a clinic and health professionals to ensure the health and wellbeing of all of our valued patients and staff. These measures will commence from 8am Monday 30 March. Of relevance to physiotherapy is the inclusion of bulk-billed Medicare-subsided telehealth for chronic disease management items. Information about relevant Medicare item numbers can be found here: https://bit.ly/3as1Wui At this stage, it is our understanding that patient eligibility criteria remains unchanged. A patient must have: - a general practitioner management plan (GPMP) - item 229 or 721 - team care arrangements (TCAs) - item 230 or 723. We are seeking clarification regarding the proposed incentive payment to ensure practices stay open to provide face-to-face services where they are essential for patients with conditions that cant be treated through telehealth. This is completely reasonable reaction given the importance of the b...ack to overall function and well being. However, from multiple standpoints research indicates there is reason to be optimistic with such a diagnosis. . Firstly, many disc herniations will spontaneously regress (heal). In fact, worse herniations = faster and more substantial the regression . The rate of spontaneous regression was found to be 96% for disc sequestration, 70% for disc extrusion, 41% for disc protrusion, and 13% for disc bulging. The rate of complete resolution of disc herniation was 43% for sequestrated discs and 15% for extruded discs. . Second, many people have disc herniations and are asymptomatic. Given this, the diagnosis on its own is not necessarily a sentence for back pain and regression may not always be needed for an improvement in symptoms . Pain is in fact multifactorial! .
