25.01.2022 Heel pain/plantar fasciitis: One of the most common causes for pain at the base of the heel is plantar fasciitis. So what is your plantar fascia and what is plantar fasciitis? ... Your plantar fascia is a broad band of fibrous tissue that runs along the bottom surface of your foot, connecting your heel to your toes. When mircotrauma begins to occur due to the structure being overloaded, it is typical for sharp pain occur first thing in the morning (upon getting out of bed) or when standing up after a long period of rest. After a short period of time, the pain normally decreases, but it may return after prolonged standing or walking. The prognosis for plantar fasciitis is usually very positive and treatment often includes: inflammation relief to reduce pain, stretches, strengthening and orthotics (if a structural/biomechanical cause is noted) to reduce the load on the degenerative structure and prevent further issues and reoccurrence. It is important to note that "heel spurs", even if they are present, are not usually related to this condition and therefore surgical removal of the spurring is NOT advised.

To reduce the likelihood of issues arising, be sure to:... -Stretch: Stretching regularly, including before and after exercise, can help minimise the risk of injury. -Slowly increase activity: for your New Years resolution (getting fit) to be a long term success, it is imperative that you slowly increase your exercise levels. This will assist in preventing overuse injuries and keep you exercising for longer. -Wear the right gear: not all running shoes suit each athlete or weekend warrior. Make sure you get fitted properly and are wearing the right shoes to reduce the risk or unnecessary injuries. -Seek advice: if any issues arise, whether they are pain, numbness or any other symptom that raises concern, book in to see us for a thorough assessment from the feet up. This is due to the fact that it is very difficult for tall sprinters to maintain the angles and positions required in order to produce the power and speed needed during the inital 30m. ... Tall sprinters in the past (Carl Lewis, Linford Christie) have been required to use their extra stride length to make up for their poor starts. Gaining an advantage in the back end of the race. But Bolt is different. He manages not only to take advantage of his long stride but he also manages to cycle a faster stride length, like a shorter athlete would. This keeps him up in line with the shorter sprinters at the 30m mark and allows him to pull away in the back end of the race. Giving him the time to stop and smiler for the camera.

14.01.2022 How do orthotics work? Whilst walking barefoot, forces are applied from the ground onto the parts of the feet that are in contact with it. These forces are known as ground reaction forces (GRF). Whilst wearing an orthotic, these forces can be dispersed and shifted, reducing loads on injured tissues and improving the functionality of how you walk and run. The forces applied from the orthotic onto the foot are orthotic reaction forces (ORF).... Therefore, a well designed orthotic which is fit by a well-informed podiatrist, can be very effective in treating foot, ankle and even knee and lower back issues. Some examples of conditions that orthotics can treat or manage include: ball of foot pain, pain along the top of the foot or around the ankle, plantar fasciitis, achilles issues, knee arthritis and even posture related lower back pain. The diagram in this post demonstrated where forces are applied through arrows and the strength of those forces through the length of the arrows. Medial knee osteoarthritis (OA), which is easier understood as wear and tear on the inside of the knee, is one of the leading causes of disability in Australia. It can lead to knee pain, swelling, impaired function, morning stiffness and joint crepitus. ... The leading causative factor of this condition is the higher compression loads on the inside of the knee, which is associated with the positioning of the foot and knee. Diagram 1 demonstrates a normal knee to foot relation, while diagram 2 demonstrated a higher risk (genu varum) position, with an increased load on the inside of the knee. As the condition progresses there is a gradual thinning of the cartilage on the inside of the knee, leading to a larger (genu varum) deformity and an increasing load on the inside of the knee. The use of a well-designed (valgus) wedged-orthotic, as shown in diagram 3, can redirect the loads from the inside of the knee outwards, rebalancing the loads more evenly across the entire knee. This allows not only for a reduction of symptoms, but a reduction in damage and progression of the condition. If left untreated and managed, diabetes can result in a numerous complications, a number of which can be first seen in the feet. The two most common complications which we see and the two main things we assess for in relation to diabetes are; the damage of sensory nerve tissue (located in the feet) and d...amage to the vascular (blood) system, which can also be caused by smoking. Nerve tissue damage results in loss of feeling and therefore masks the feeling of pain when you injury yourself, while damage to the vascular system reduces the supply of blood to the feet, resulting in delayed healing. Both of these complications lead to the increased risk of complications, including foot ulceration and amputation. Yearly assessment from your podiatrist can reduce the risk of these complications, by early intervention and education. 06.01.2022 How do orthotics work? Whilst walking without orthotics, forces are applied from the ground onto the parts of the feet that are in contact with it, which are generally the forefoot (front) and rearfoot (back). These forces are known as ground reaction forces (GRF). Throughout normal standing and walking, the medial longitudinal arch (MLA) which also known as the instep, doesnt come into contact with the ground and therefore there is no GRF.... Whilst using an orthotic, GRF are applied to the device which then applies its own forces on to the feet. These forces are known as orthotic reaction forces (ORF). A properly fit (non-custom) or customised orthotic disperses these ORF across the feet, including the MLA. Due to the orthotics ability to apply an external force to the MLA, it decreases the tension stress on the load bearing structures of the foot, such as the plantar fascia as well as plantar intrinsic muscles and ligaments. Therefore a key component of how orthotics work are their ability to relieve loads on injured tissues in the foot and leg, whilst improving gait function (the way you walk and run). Hence a well designed orthotic, fit by a well-informed podiatrist can be very effective in treating conditions such as plantar fasciitis, plantar intrinsic muscle fatigue, plantar ligament strain, or dorsal midfoot interosseous compression syndrome.
