14.01.2022 Standard 2 Aged Care Quality Standards - Plans of Care A reminder during this important time to maintain communication with your aged care provider to ensure the plan of care is meeting your loved one's needs. This can be a simple as a phone call to the Registered Nurse, Clinical Manager or Case Manager to discuss what is in the care plan and collaborate to ensure the most relevant interventions for care are current for your loved one. If visiting in person you can request to read the care plan and also have a copy if you have enduring guardian and your loved one agrees to you having a copy of the plan of care.
