19.01.2022 Ive really enjoyed planning this case with 2 surgeons. This 15 year old with a glenoid the size of a 5c coin has a flail arm. The goal is to give him some function back by custom making a constrained reverse shoulder prosthesis. Its round 1 of the planning stage. These 3D printed models allowed us to evaluate and further refine the design to accommodate his soft tissues. This unique situation requires a bespoke solution. 17.01.2022 This was a great case to plan as I was sent a few xrays via sms and asked if my prosthesis would fit without removal of the gamma nail proximally. Knowing the length of the gamma nail (180mm) and by looking at the right side femur so see how much length has been lost due to the fracture. I was able to visualise that my minimum length implant would fit and recommend the surgeon we could use this implant without removal of the nail. It was very rewarding when it was millimeter perfect. 14.01.2022 Planning this case was hard. A failed hinge case that required a bone conserving distal femur. Knowing that the joint space in extension would jack out when we unlocked the hinge. The best option was to plan the tibial reconstruction to replicate anatomy. Then telescope the joint distally with traction while placing a trial over the patients anatomy while measuring the femoral cuts. This was my theoretical Plan A. Knowing that the femoral metaphysis would remain intac...t. A distal femoral cone and cable would bolster up the bony scaffold. I decided 2 more plans, just in case the bone was weaker and therefore I needed to prepare the team and the surgeon with immediate options and Plan B and C would be more stressful. Especially when we needed to avoid the total hip prosthesis and create a stress riser unnecessarily. Then after studying this pre op X-ray for a while and devising said plans. I jumped on a plane to meet the surgeon and do the case 4 hours flight away. It all went to plan A and everyone was happy. The old poly liner had worn through to metal and caused the bodys natural defences to attack the knee. 11.01.2022 Def scroll through these 9 pics. When planing this case I had a standard A/P and LAT X-ray and I felt I wasnt getting enough information. So asked the surgeon to get a full ling leg X-ray and when they did, I came back to replan. This was invaluable as it gave me so much more information to formulate 4 different plans dependant on the remaining bones health. So a bone scan was ordered, however I didnt know this test result until the day of surgery. So I had to play for... multiple outcomes and react on the day for the best plan for the patient. This became a really rewarding case as my Plan A did eventuate. Keeping more of the patients bone is tougher as there is more anatomy to respect. Metal backed tibias and thick femoral components may require augments. Uncemented components with complex fins and pegs may take good bone upon extraction. ... Having an instrument set that allows for primary jigs to reference existing bony landmarks in the presence of medial loss is important for component positioning and sizing. Having crossover cutting blocks for the provision of augments in the presence of bone loss makes for a seamless and efficient next step. When things get serious and require the use of additional cues within the knee joint because bony landmarks are unreliable due to considerable loss. Quickly we can flip between a gap referencing approach to double check femoral component rotation, sizing and A/P position.
