25.01.2022 Great post again by @theconsciousphysio: "What really is acceptance? It's a term thrown around a lot but how do we really know if someone has reached that accep...tance stage? Well there may be several clues they give in terms of their outlook and attempt to move forward. One thing for sure is acceptance doesn't mean someone will always be positive and happy , there may still be frustration and anger at their situation but often this will not last long as they will have/find a way to manage this and the person will keep moving forward and focusing on what they can control. The person will also have a greater understanding of themselves (strengths and weaknesses). Eventually the person may not be emotionally bothered at all but remember acceptance is a JOURNEY ." . 23.01.2022 The windlass effect describes a biomechanical concept in which dorsiflexion of the big toe causes the plantar fascia to be put on tension. ...This tension then causes the arch to lift, the calcaneus to invert and the subtalar joint to move into supination. Why is this important? Supination and pronation are integral movements to normal foot function and serve different purposes. Supination locks the many joints of the rear foot and mid-foot and occurs when we first contact the ground at heel strike and again as we push off of the ground into our next step. In between these two supination periods, we find pronation, which is when the joints of the foot unlock, allowing the foot to absorb force and conform to uneven surfaces. Problems arise when big toe dorsiflexion is limited (60 degrees is normal) and normal foot mechanics are altered during the gait cycle. Over time, individuals with limited range of motion may be more prone to developing pain and/or various orthopedic foot conditioning, including bunions. 19.01.2022 Movement myth "You should start running 3 months post ACL surgery" A lot of practitioners believe that the best time to start running after ACLR is at... the 3 month mark. But is everyone at the same stage physical function wise after 3 months? Not always So when SHOULD you start running after ACLR? Research suggests that a combination of functional criteria should be used to determine readiness to return to run, including: Full or close to full knee ROM Pain < 2/10 VAS > 70% quadriceps LSI > 70% hamstring LSI > 70% single hop for distance LSI LSI = Leg symmetry index. [study: Rambaud 2019] If these criteria are met before running, this increases the chances of the patient making a successful return back to running, as their body is better prepared to run.

08.01.2022 Is it ok to experience some discomfort when exercising? The below chart outlines acceptable pain response with exercise 1)Pain during activity 2)Symptom response for 24 hours after activity 3)The trend in symptoms over time... Of these 3, the latter two are usually more important than pain during activity. Pain during activity simply tells us that a sensitive tissue is being loaded, which is expected following injury. Our aim with exercise is to ensure pain during activity is mild (0 to 3 on pain scale) and settles to baseline levels within 24 hours plus that the trend over time is an improvement in symptoms. If this is not being achieved a review of the activity levels and modification of them should be assured to create the desired response.
