25.01.2022 **Gait Analysis** A key part of my podiatry assessment is looking at you moving. There are lots of ways to do this, each with their uses and limitations. First up, visual assessment ... This is the simplest form of gait analysis and with a trained eye can be a very useful tool. Our eyes can pick up abnormalities and injuries, particularly in those people we regularly see. I find that visual assessment can pick up the rhythm of a persons movement and how well the different parts of the body move in unison. I can easily and quickly get a person to change the type of movement, speed or surface and get a idea of how this affects their movement pattern. If necessary then we can use further tools to assess movement in more detail. It can also form part of gait retraining when we use our eyes as well as client feedback to tweak movement. However there are some obvious limitations with visual assessment. It’s highly subjective and prone to error, particularly when trying to assess faster movements (like running) in fine detail. It doesn’t give us any objective data and lends itself to pseudoscience particularly when we use language like you have good running form. Feedback can differ greatly depending on the assessors speciality - for example a running coach may focus on arm swing whereas I might be more drawn to what the feet are doing. 23.01.2022 *Running Self Care - Ankle* This is a great exercise to improve the quality of movement around your ankle joint and reduce stiffness. I do 10 of these on each side as part of my warm up - it helps to prepare me for the run. Grab a resistance band, and step and bring the knee over the foot whilst the band is pulling back on the top of the foot - experiment with the knee alignment to find any areas of stiffness. Dont push too hard - just enough to feel a little resistance from... the ankle. The ankle should feel good afterwards! #runningpodiatristclinic #runningpodiatrist #runready

20.01.2022 Thanks Kevin A. Kirby, DPM for sharing this insight. Elite runners in the same race at a relatively similar speed (around 2:50/km) have a wide variety of footstrikes. This adds weight to the case that no best footsrike pattern exists for all. What works best for each athlete is very person specific!

18.01.2022 **Cadence** Cadence or stride rate is the number of individual steps per minute and is a very hot topic in running! A number of recent studies (Willy et al 2015, Bradley et al 2018) have shown that increasing cadence by 7.5% reduces hip and knee loads and pain in those with Patellar Femoral Pain. Most smart watches measure cadence and if yours is on the low side (less than 160 for a steady run) then increasing your cadence could be a quick and easy way to improve your technique and reduce peak loads on your hips and knees! Its something I look at with my clients and should definitely be considered when assessing running form.

17.01.2022 Load Management and Training Intensity Have you considered how much of your training is done at a moderate or high intensity? (Above a 5/10 effort) There is a growing amount of data that shows that elite distance runners do around 80% of their training at a low intensity and very little at actual race pace.... I believe that this concept of polarisation is key to building a strong aerobic base and preventing injuries caused by under or over loading the body. High Intensity Training is a key part of any running programme but only when done in small doses on top of a low intensity base. This approach has certainly helped me to avoid injury and I would recommend it when you are planning your next event to work towards! They differ in a number of shape characteristics, particularly at the arch, the lateral side of the foot, the first toe, and the ball of the foot. So when thinking about shoes, even if you have a wide or long foot, you should still try to wear footwear specifically designed for a womans foot shape. 08.01.2022 This week I'm talking all about running shoes! First up some quick tips when it comes to shoe wear. The life of a shoe is very variable depending on the shoe and person. However looking at an old pair of shoes can help you to understand some interesting things - these are some Hoka One One Clayton's I've worn since 2017. Lasting pretty well id say!... We can see the areas of highest wear which give us clues as to where load is occurring during running. We can also see differences from shoe to shoe - such as my left heel wear which is much greater than my right. So yes have a look at your old runners and always bring them when you go to see a podiatrist. 07.01.2022 **The Basis of Running What does the body need to be able to able to do?** Understanding whats involved with running is the first step to ensuring that you will cope with and enjoy it! Running differs from walking in that there is a period between each step where neither foot is in contact with the ground (called the float phase). This means that our body alternates between periods of absorbing and generating force (decelerating and accelerating our speed). Our tendons and... muscles store and return energy during this movement think of it like being on a pogo stick! So what do tasks do we need to be able to perform during the running motion? 1. Shock absorption and control when the leg finishes swinging and lands on the ground 2. Ability to generate energy to move us forward and upwards as we push off from the ground 3. Control of balance and posture of the upper part of the body 4. Control of direction changes of the center of mass of the body The simple task of running is to primarily move forwards quickly and easily whilst limiting net rotational and side to side movements, thats why doing exercises like single leg squats are a good place to start for all runners! So next time to are running think about how well your body is able to do these things. How we improve them is the next step! 07.01.2022 **Lateral heel wedge modification** In this case using @dolaortho wedge strips on @birkenstockau insoles to help improve stability in a client with an ankle deformity. Wedging can help to alter ground reaction forces thus changing loads of structures like joints around the ankle, and the peroneal tendons. Its a relatively simple solution which can be easily modified as required. I love playing around with support and seeing the impact on movement with my clients.... Its important to remember that insole modifications may cause a number if effects throughout the body so regular review is vital! #runningpodiatristclinic #southyarraspinesportmedicine #sumofusstudio

02.01.2022 **Ankle Ligament Rupture and Instability** Not often I see this in my clinic! This client ruptured his ATFL ankle ligament when aged 18 and didnt get any treatment for it. Now 35, the ligament has never healed and he compensates by turning his leg so the ankle never rolls out when walking. Now we are finally working on some ankle rehab to reduce the demand on his achillies tendon which is inflammed and taking all of the load. 17 years late is better than never!

02.01.2022 **ASICS Running Shoe Updates** Great to catch up with Jason Mansfield from ASICS at South Yarra Spine & Sports Medicine yesterday. Of note: - unlike some other brands ASICS are continuing to use medial posting in their shoes (support along the inside) and they have a new model the GT 4000 which is similar to the old Brooks Adrenaline - the Dynaflyte is a great option for runners who dont need much biomechanical correction and/or who are running at a faster pace - the DS Trai...ner has become much stiffer and is ideally suited to faster runners with a higher cadence - of their childrens shoes, the Kayano is the only shoe that has equivalent technology to the adults version, so dont be fooled into buying other models if you are a distance runner with small feet! Always good to be across the latest shoe tech! See more

A study in 2017 found significant improvements in pain and toe angles after 8 weeks.
