25.01.2022 Scott Street Physiotherapy will be operating reduced hours over the Christmas to New Year week. We will be open Thursday 27/12/18 morning and Friday 28/12/18 in the afternoon. To assist in reducing the spread of COVID-19 disease Scott Street Physiotherapy is now able to provide telehealth consultations using COVIU. On April 14 health funds will provide a rebate for these services supported until 30/9/20.... The fee associated with this service will be $50.00 for private consultations or as appropriate for you Insurance provider (DVA, EPC Medicare referral, workers Compensation Claim or Motor Vehicle injury). 21.01.2022 Are shoes foot coffins? There is and has been an anti shoe movement lurking. This has been present for poorly fitting shoes for decades with a Tsunami of bunions and bunionettes turning up for treatment. The anti-shoe brigade likes to demonise shoes as fungus and bacteria incubators, stimulators of detraining for the little muscles in your feet and take issue with companies profiting making big bulky shoes for big bulky profits. Part academic peer review, part hysteria an...d part tribal. Fun facts Shoes should fit your foot - if you have the foot of a Hobbit then a pointy elf shoe will not do Spend some time of the day in bare feet to make your foot muscles do some work and get all the joints in your foot moving. 21.01.2022 Post event nutrition is the beginning of recovery after an event. Let's take Rudolf - circumnavigating the world in 24 hours - 40 075 Km in a straight line at ground level with no diversions averaging 1670kph. That requires a lot of energy along with some left over energy to make the nose glow red. So if you think you indulged too much over the last few days have a thought for Rudolf who has earned a large meal and a long rest. Good luck with the New Year...



20.01.2022 Here at Scott Street Physiotherapy we regularly challenge ideas that are routinely said to be good for us eg the "You must bend your knees and keep your back straight" dud. Other than eat good food, exercise regularly, sleep well, brush your teeth twice a day and flossing regularly (sorry Wendy I'm not so good at the flossing regularity) many other commonly held rules are flawed. 20.01.2022 History of Physiotherapy -is the saying "I am a tool, I work with my hands" appropriate or a forgotten part of Physiotherapy? Physiotherapy has been through a number of Renaissances. From the beginning Physiotherapists started out life as principally massage therapists. The observed effects of trauma and rest led to Exercise Therapy as a primary role of Physiotherapy, though yet to be referred to as Physiotherapy. Then came the use of fancy machines like infrared ray lamp...s, ice packs, galvanism, HVG, ultrasound, TENS, Laser, Stereodynamic Interferential and other various reincarnations of heat, cold and electricity. Do they work? - not at all convinced... The restoration of Movement and function remains the role of Physiotherapy. Knowledge, experience, detailed assessment, interpretation of symptoms, recognising limitations of various assessments and treatments plus expert understanding of anatomy are pre-requisites for Physiotherapy treatment.

19.01.2022 Here is a question often asked at the office - What mattress should I buy? I think I need a new pillow/mattress - my neck/back hurts when I wake up in the morning, or bigger problem I bought a new mattress for $$$$ and I still feel the same. Sounds logical but sometimes the issue can be simpler than the mattress or pillow. Consider these possibilities: Sleeping on your stomach. Clenching your teeth in your sleep.... Pillow too high or too low. Sleeping in a position like you fell out of a building. Adopting the same asymmetrical position sleeping, driving, standing, sitting. Often singular, repeated habitual movement patterns drive the pain you experience.

18.01.2022 There are various degrees of injury to the ACL. Normally described as a pivoting or hyperextension injury many other mechanisms of injury are possible. Over the past couple of months we have seen reconstructions for 1. Complete ACL reconstruction using hamstring graft 2 Complete ACL reconstruction using patella tendon graft for a hyper mobile knee... 3 Partial tear of 1/2 the ACL repaired with hamstring graft allowing accelerated rehabilitation as the knee is pretty stable due to the remaining ACL> 4 Avulsion of ACL attachment off tibia pushed into place with K-wire and bound down with string and a button. This is treated along the lines of fracture healing and again allowing accelerated rehabilitation as the knee is stable once the bone has re-attached. 3 and 4 should be back to normal activity at about 6 months post operatively whereas the usual 9-12 months rehab will be required for the others. general rules for ACL rehabilitation - get it straight, control the swelling, get your muscles going especially hams and quads.



18.01.2022 COVID-19 Firstly in these strange times I wish all our clients and staff to look to the future pursuing civic responsibility, courtesy and goodwill towards each other. At Scott Street Physiotherapy we are doing our very best to limit any exposure within our clinic as we will have to close for two weeks should we be exposed to person with positive COVID-19 test... Current protocol is: Do not enter until you have been met at the door and answered questions related to potential exposure. 1-2 people will be in waiting room at any time. 17.01.2022 Regular exercise can make you younger. If you are a little overindulged where fast foods, cakes, chips and beer have become a primary food group for you, your physiological age will be elevated. Obesity, diabetes, high cholesterol and elevated blood pressure contribute to estimated Physiological age. This is reversible. Get 30 minutes 5 days a week as a minimum. Not just a dawdle - your heart rate must go up so you can't hold a conversation or eat a croissant at the same time.

16.01.2022 How much water do we have to drink? Water has become an expensive commodity to buy as a drink. The price of 1000 Litres of water from the tap in Perth is in the range $1.82- $4.55. To fill a 45 000 litre pool can cost less than $100.00. On the flip side 1 Litre in a bottle is around $2.00 so to fill a pool from bottles would cost $90000.00. Basic rules around drinking are:... 1. It is really hard for your body to lose 2 litres of water in one hour. 2. First sign you are dehydrated is most often - THIRST. 3. As we get older our sense of thirst diminishes so check the clearness of your wee (orange is no good). There are a few variables around this especially if taking medications or supplements, chronic medical conditions and eating beetroot (wee goes pinkish). 4. Tee, coffee, veggies and fruit all contribute to fluid intake. 5. You can drink too much water and become very unwell. 14.01.2022 How does Physiotherapy treatment help people with dizziness? Not all Physiotherapy treatment is hands on. Determining the cause of dizziness is most important which involves an array of specific tests. These tests determine the initial treatment. ... When the primary cause of the vertigo is no longer present but unsteadiness persists we challenge some of your senses (a few examples include eye function eg gaze fixation and proprioception/balance) to bring on the wonky feeling. These become the exercises to recover from your episode of vertigo.

11.01.2022 Concussion Why does our brain react to a knock that causes momentary loss of consciousness, short term memory loss, headache and confusion? Basically the consistency of our brain is a bit like just set jelly. When the head suddenly accelerates or decelerates it wobbles and twists in the skull causing injury to the nerve cells that make up all the various parts of your brain - the balance bit, the seeing bit, the memory bit, the I am tired bit. Post concussion recovery is an... interesting process to observe. Following medical examination and maybe imaging the process takes time. Your brain can be damaged from a knock at sport and affect what are normally simple acts such as reading, watching TV, writing a letter or doing normal work duties for a week or two after. Overuse of the brain early on after concussion can lead to headache, nausea, vertigo, frustration, problem solving troubles and memory problems. Resting the brain along with manageable levels of activity is the only way to recover from the knock, just like resting after any other acute sports injuries. See more

10.01.2022 CORE EXERCISES - necessary or a waste of time? Many attending Scott Street Physio for their treatment refer to their core exercises or core strength. Often this is a reference to how long they can hold a plank position. Specifically, the CORE is a set of muscles that coordinate their contractions to support you spine/pelvis whilst arms and legs move at the same time preserving normal breathing and no leaky bladder (ie continence). It refers to the ability to coordinate ...these contractions rather than a straight up static bracing effort. There are plenty of cheats when you think you are doing a core programme. Two considerations - Consciously activating the core during exercise is not normal. Core exercises are not always a necessary part of an exercise programme for a good functioning spine. Often it just does its thing in the background.. Happy exercising...

All pain is real but some pain is more real than others. Sometimes if you take a pill or placebo or supplement and say things like" I don't need to know how it works it just does" it could be you pain volume control turning down.... Happy reading

09.01.2022 It's time to get rid of your rules about how bad back pain can be.... 80-90% of people get over back pain in 6-12 weeks often with no treatment It you are 20 you have the back that is yours not the back of an 80 year old BENDING YOUR BACK IS GOOD FOR YOU, SO IS SLUMPING.... Very few people end up having back surgery - it is rare. Your back and hips do not go out, slip or sublux. A straight back is not a good back - a mobile one is. Backs are strong and capable of absorbing tonnes and tonnes and tonnes of force. TONNES Wear and tear, arthritis, spondylosis, disc bulging are all NORMAL. Rarely bad things happen to spines so avoid falling into the trap of believing all back pain is bad. Ditch the rules and dud ideas about back pain and get on with it. Backs need muscles, movement and use NOT rest, braces and tensing up.

07.01.2022 Telehealth success... This week we did our first half dozen telehealth sessions using COVIU. There was a small glitch getting audio from client to Physio. To correct this when accepting the appointment it is important to select the appropriate audio input. That is, rather than just pressing allow camera and audio, please check the audio list and select internal microphone as the source of audio (not a bluetooth speaker). Covid-19. We continue to follow self isolating out...side of work and maintain cleanliness of the surgery. There is much anxiety around COVID-19 and its prevalence in the community. Currently * it is highly unlikely you will get it in WA. There are under 500 cases, 180 have recovered, 50-60 are in hospital and most are from holiday makers coming off cruise ships or aeroplanes, many in quarantine. * The Chief Medical Officer of Australia recommendations around use of face masks is you do not need one unless you are in contact with large numbers of people in a health setting environment (eg hospital, medical practice) or if you are unwell. The wearing of a facemark when you are not unwell under these guideline suggests you are unwell. 05.01.2022 Be sceptical of your thinking around back pain. Beliefs and thinking biases are simplistic ways to ease through each day. Thinking takes time, energy and understanding at the same time as challenging your biases. With regards to low back pain - the ridiculous proposition "Keep your back straight, shoulders back". The idea would appear to come from a number of areas. Firstly, observing the way people with acute spinal pain move when they have pain and concluding that this ...is the way we should move even when we are good. Insurance companies have Mission Statements and defined Worlds Best Practises around preventing low back injury continuing to promote this furphy. Thankfully an unintended consequence is it supports a printing industry providing posters for every lunch room and shop floor at worksites. Secondly the idea that a straight spine is good, something to be proud of whereas observed bad posture is frowned upon, a lack of self care and maybe you deserve pain when you get low back pain (which 80-90% of us will get in a lifetime). No other part of the body has a rule around continuing to perform a similar motion in perpetuity. Ohh I sprained my ankle. It feels better if I hop on the other leg or use crutches - what a good idea I will continue to hop and use crutches. Just as ridiculous. Rule 1. Keep moving, SLUMP, bend your back in every direction. It is good for you. Rule 2 Logic is often floored. Check your biases. 05.01.2022 The argument against "Pilates is for everyone" Lots of people tell me they do Core stability, TA contractions, Clinical Pilates like it is mandatory to do for any activity or exercise or movement they do. The assumption here is that it is good and normal to consciously contract the pelvic floor and core muscles before every movement to prevent injury. This is an incorrect assumption. The thing is ... 1.Preparatory TA and pelvic floor contraction is a stabilising effort - probably great if you are loose around the pelvis, terrible if you are stiff and stable in the first place. 2. the effort of the contraction has to be balanced by the effort of the activity and modulated depending on whether you are breathing in or out and maybe if your bladder is full. 3. The same people advocating increase the tension in your TA's and pelvic floor may also be the people advocating drop your shoulders to ease the tension in your neck. Go figure.. Rules about muscles that should be on or off is a bit confusing. In the end - Right diagnosis, right exercise

04.01.2022 Kenny Rogers recorded The Gambler song with the famous line "You got to know when to hold 'em, know when to fold 'em, Know when to walk away and know when to run."... Scott Street Physiotherapy abides by these ideas not because we like country and (the other type) western music nor associated with gambling. Rather there are times when patients need treatment, don't need treatment and need a rehabilitation progression to get them from walking and back to their sport. These words could also be applied to choice of treatment modality - When do I push (lots), when do I crack (rarely), when do I trigger point (sometimes), when do I strengthen (nearly everyone), when do I dry needle (occasionally), why I shouldn't bye those machines (TENS, ultrasound and the like). Bring your own hand sanitiser so you don't have to touch our bottle.... If you have to wait then you will be asked to sit outside or in your car if it is raining. In the future we will be offering appointments online through TELEHEALTH or home visits. Until this happens it is business slightly modified from usual. 02.01.2022 Which shoes should I buy? Brand, Colour, pronator, supinator, neutral foot, over strider, barefoot technology, minimalist shoes, big bulky shoes, bare feet, thongs, Crocs, Ugg boots. All have their place (maybe not Ugg boots). Very few of us need a full biomechanical assessment of walking or running technique. High level athletes with injury certainly have separate requirements. Most important points are 1. Exercise 2. comfortable shoes. 3. Good training progression.... 01.01.2022 Question - When is stretching not OK for some of your aches and pains? Answer 1 - When it's a tendon problem especially if the problem is at the attachment to the bone. Leave it alone. This includes hamstring origin, achilles insertion and gluteus medius insertion. There are more including golfers elbow and tennis elbow and ... and maybe if you can do this (see photo).
