22.01.2022 I'm not sure you'd disagree with me that there is a groundswell of support for championing more funding into aged care. I can't say I am vocal proponent of this argument. However with scrutiny from the Royal Commission, I feel like the conversation might be turning towards the dialogue it needs to.... And that is "where does the money go?" $20 billion in funding each year. That's a LOT of funding. Thank you Prof Ibrahim and Ian Henschke for saying what needs to be said. 22.01.2022 Hi everyone, Thinking about navigating the aged care system, from first point of contact with My Aged Care, to assessments with RAS or ACAT assessors, to understanding the approvals and what to do next, then choosing and negotiating the most appropriate support within a home care package, understanding legal aspects such as EPOAs and how to access specialised advice, ie Geriatricians, what do you think would be an invaluable tool or service to assist you with this? Do you th...ink a suite of Road Maps or guides (how to's) would be helpful or an online platform that you could access and work through to help answer questions and suggest a 'next step' to answering your questions. I'd love your feedback. I'm looking at developing a toolkit for consumers to assist with understanding and navigating the aged care system. Cheers, Coral :)

20.01.2022 If your aged loved one receives a home care package and they have a cognitive impairment, perhaps they have a diagnosis of dementia...are they receiving the dementia and cognition supplement?

19.01.2022 What is the dementia and cognition supplement? This is a supplement (an extra amount of money) that is allocated to the client’s home care package if the client meets the eligibility criteria for the supplement. However, the dementia and cognition supplement is not automatically allocated if someone has a diagnosis of dementia.... The amount of the supplement depends on which level of Home Care Package (HCP) a person is on. Home Care Package Level Daily Supplement Amount Level 1 $2.81 Level 2 $4.95 Level 3 $10.77 Level 4 $16.32 Eligibility is determined by undergoing the appropriate assessment. There are 3 different assessments. 1. RUDAS Rowland Universal Dementia Assessment Scale. For those from a culturally and linguistically diverse background. 2. KICA-Cog Kimberley Indigenous Cognitive Assessment. For First Nations people living in a rural or remote area. 3. PAS Psychogeriatric Assessment Scales. For all other care recipients. These assessments can be performed by: A registered nurse A clinical nurse consultant A nurse practitioner A clinical psychologist (PAS or RUDAS only) A medical practitioner Other appropriately trained health practitioners (KICA-Cog only) The home care provider can organise the assessment and complete the application form. Once a care recipient is determined to be eligible for the supplement, there is no need for reassessment, even if their package level or provider changes. If you believe your loved one may be eligible for the dementia and cognition supplement, ask your home care provider.

19.01.2022 What is TCP? TCP is the Transition Care Program, a federally funded, short-term program that is designed to help older people who are about to be discharged from hospital. TCP provides up to 12 weeks of care to allow older people to regain their confidence and get back on their feet in a non-hospital environment, and to improve or maintain their level of independence, following a hospital stay. Depending on individual circumstances, the TCP may be extended if needed.... State and Territory governments partner with service providers who provide the TCP services. The service providers may request fees from the care recipient if they are financially able to contribute to the cost of their care. If a person is not ready to go directly home from hospital, TCP can be started in an aged care home, giving the recipient additional support and then the recipient may go home to complete their TCP program. If a recipient is able to go directly home from hospital, TCP will be delivered in the client's home. The sorts of services that are available under TCP are: Nursing support Low-intensity therapy such as physiotherapy and occupational therapy Podiatry Social work Personal care The older person must be assessed in hospital by a TCP clinician to establish eligibility. Patients in either public or private hospitals can access this program. The hospital staff can arrange the referral.

14.01.2022 I thought about the heading of this blog post a lot. I'm not trying to convey how to jump the queue to getting ahead with assignment of a home care package, but I want people to be aware there is a way to have the ACAT review your situation and possibly re approve you or your ageing loved one as a high priority for a home care package. Here's how I go about supporting desperate clients in this situation.

11.01.2022 Hi everyone, I'm watching the OPAN webinar about what you need to know if you want to take your loved one out of an aged care facility. It's very Victoria orientated but the information may be worth considering elsewhere in Australia too.... They are discussing the support available at home if you do take your loved one home from an aged care facility. There will be a emergency leave 'package', similar to a HCP 4 available to these residents returning to the family home. My questions were, will these HCP 4s impact on the people already in the national queue waiting assignment of a HCP 4 and will the ACAT/ACAS need to assess for these emergency packages. The answers were that these HCP 4s are additional funding, limited to 8 weeks and not required to be ACAT/ACAS assessed. That is, the people in the national queue waiting for a HCP 4 will not be disadvantaged by people exiting RACFs and accessing these additional HCP 4s. Coral :)

10.01.2022 There is a cohort of older Australians who are struggling with an emotional toll. There are also family carers who used to attend residential aged care facilties on a daily basis to provide comfort and support to their loved ones residing in those facilties. These are the stories we're not hearing about in the public dialogue of what is best for residents in aged care facilties.... I urge aged care facilties and their representative organisations to take a more compassionate, inclusive and balanced approach.

10.01.2022 Progressing conversations with ageing parents about accepting services and support can be really challenging. An ageing parent may see this kind of conversation as being a criticism of their ability and will be suspicious that their independence is threatened. Sometimes it takes a courageous conversation for a breakthrough to occur.... A skilled, compassionate and independent person should be able to pivot the discussion and engender trust, thereby allowing the older person to see the offer of support from another perspective.

08.01.2022 Did anyone watch 7.30 Report tonight, about home care packages? Know that your elderly loved one has choices...choice of provider...choice in what they can spend their package funding on...that they can ask for a support plan review if their needs have increased and they are still waiting for assignment of a higher level home care package...that there are other supplements they may be able to apply for or services to 'top up' their home care packages. Whilst upwards of 40% in... administration fees is questionable at best, be aware that home care package providers who charge significantly less, may be recouping their profits in higher costs for actual service delivery. Shop around and do your research. Draw up a comparative list to take to each package provider, so you're comparing 'apples with apples', this way you'll be clearer about how costs differ between providers. And if you don't get a good feeling or feel you are being coerced into signing up with a provider, walk away. If you or your ageing loved one can relate to any of these issues, please reach out and ask us how we can guide you in the right direction and get you closer to getting the right support within an appropriately financially managed package. 08.01.2022 Qld residents 60 years and older, I've just become aware of this additional funding through the home assist program. One off funding up to $5000 to complete larger home maintenance or modifications. Eligibility is being 60 years or older, holding a Commonwealth Pensioner Concession Card, being unable to complete the work yourself, not having family to complete the work for you and not being able to access CHSP or DVA funding to complete the work.... There are 7 simple questions you go through to check your eligibility. Then you register your interest on 13QGOV or 13 74 68.
