24.01.2022 What is TCP? TCP is the Transition Care Program, a federally funded, short-term program that is designed to help older people who are about to be discharged from hospital. TCP provides up to 12 weeks of care to allow older people to regain their confidence and get back on their feet in a non-hospital environment, and to improve or maintain their level of independence, following a hospital stay. Depending on individual circumstances, the TCP may be extended if needed.... State and Territory governments partner with service providers who provide the TCP services. The service providers may request fees from the care recipient if they are financially able to contribute to the cost of their care. If a person is not ready to go directly home from hospital, TCP can be started in an aged care home, giving the recipient additional support and then the recipient may go home to complete their TCP program. If a recipient is able to go directly home from hospital, TCP will be delivered in the client's home. The sorts of services that are available under TCP are: Nursing support Low-intensity therapy such as physiotherapy and occupational therapy Podiatry Social work Personal care The older person must be assessed in hospital by a TCP clinician to establish eligibility. Patients in either public or private hospitals can access this program. The hospital staff can arrange the referral.



22.01.2022 What is the dementia and cognition supplement? This is a supplement (an extra amount of money) that is allocated to the client’s home care package if the client meets the eligibility criteria for the supplement. However, the dementia and cognition supplement is not automatically allocated if someone has a diagnosis of dementia.... The amount of the supplement depends on which level of Home Care Package (HCP) a person is on. Home Care Package Level Daily Supplement Amount Level 1 $2.81 Level 2 $4.95 Level 3 $10.77 Level 4 $16.32 Eligibility is determined by undergoing the appropriate assessment. There are 3 different assessments. 1. RUDAS Rowland Universal Dementia Assessment Scale. For those from a culturally and linguistically diverse background. 2. KICA-Cog Kimberley Indigenous Cognitive Assessment. For First Nations people living in a rural or remote area. 3. PAS Psychogeriatric Assessment Scales. For all other care recipients. These assessments can be performed by: A registered nurse A clinical nurse consultant A nurse practitioner A clinical psychologist (PAS or RUDAS only) A medical practitioner Other appropriately trained health practitioners (KICA-Cog only) The home care provider can organise the assessment and complete the application form. Once a care recipient is determined to be eligible for the supplement, there is no need for reassessment, even if their package level or provider changes. If you believe your loved one may be eligible for the dementia and cognition supplement, ask your home care provider.

10.01.2022 Senior veterans or war widows/widowers may be able to access services from both My Aged Care (MAC) and the Department of Veterans' Affairs (DVA) at the same time, as long as they are not getting the same service from both.
