24.01.2022 This year marks the 75th anniversary of the end of WWII. After four years of fighting, thousands of brave men and women returned home to their loved ones, hopeful for a more peaceful world. Sadly, there were also thousands who never came home. Their sacrifice will never be forgotten. Now, and every day, we will remember them. #WeRememberThem #RemembranceDay #LestWeForget



23.01.2022 Our clients say that they love being cared for in their own surroundings and we are keeping them safe from exposure to COVID-19, said Sue Meteyard, Director, Clinical Operations. Our Hospital in the Home team on the Sunshine Coast has been looking after acutely unwell people at home, as well as both suspect and confirmed COVID-19 clients who need to be monitored while in home isolation. This week, for Nursing in the Community Week, we celebrate the expertise of our home hospital teams across Australia and honour our nurses who enable clients to stay safe at home and get the vital care they need.

23.01.2022 During NAIDOC Week we are proud to launch our second Reconciliation Action Plan. Our RAP 2020-2022 is an important milestone in our journey to help bring about a just, equitable and reconciled Australia. With our teams across the country, we will work towards meaningful change in the way we embrace our First Nation’s people in the communities we serve through the care we provide and importantly, in our own hearts and minds. With our RAP, we will honour and reflect on Aboriginal and Torres Strait Islander culture and their story that began more than 65,000 years ago. The painting is ‘Seed Pod Dreaming’ by artist Micky Barlow. Micky is a Kokatha woman, whose people are the traditional owners of a region in northern SA. The painting is proudly displayed in our office in Collinswood, SA.

In 2019, our team of 750 highly trained registered and enrolled nurses managed more than 40,000 wounds on 18,000 people, healing 79% of them. We have the largest wound care service in Australia and are a national leader in wound care research and innovation. ... Our Clinical Nurse Consultant, Prof Keryln Carville is an international expert on wounds and her research has been integral in building our service and expertise. Keryln wrote The Wound Care Manual, a comprehensive resource for nurses, doctors and health professionals used around the world.

17.01.2022 Meet Matthew Fantoni, our new community pharmacist who works with our frontline teams across Australia on ways to improve medication management for our clients. Safe medication management is essential for everyone and for many older Australians, the ability to remain independent at home can depend on being able to manage a complicated medication regimen. With more than 17 years’ experience as a community pharmacist, Matthew has joined Silver Chain to create new innovative models to further benefit our clients, while educating our staff.

17.01.2022 Our Western HealthLinks service manages more than 7,500 Melburnians with chronic and complex health conditions in their homes avoiding unnecessary hospital admissions. Jennifer Collins, Director Clinical Operations, said her team visits clients at home and connects them with the support and care they need in the community. When a client is concerned and/or unwell, they provide an urgent rapid assessment and nursing response that keeps the majority of our clients safe at home... rather than present to hospital. #nursinginthecommunityweek

17.01.2022 A huge congratulations to our team who have picked up four national LearnX Awards for their COVID-19 online learning model! As we moved to new ways of working during the start of the pandemic, we rapidly transitioned face-to-face workshops to online learning models to support our front-line workers. The most crucial of these online learning models - 'COVID-19 ‘Protecting yourself, your clients and your community’ supported frontline teams to safely use PPE and prevent COVID-19 infection.

16.01.2022 For 88-year-old Ernest, regaining his balance and coordination with our in-home occupational therapy service has enabled his return to gardening. Cate Maguire is investigating the impact of targeted skin tear education in improving wound outcomes in a high-risk community population. My very first research project was in determining the effectiveness of a pH-friendly moisturiser on preventing skin tears. I provided education to nurses and care staff in skin tear prevention and management. I have been interested in what it takes to provide good ou...tcomes to our clients ever since, said Cate, a Research Nurse studying a Master of Philosophy (Nursing). For more than ten years Cate has researched best practice wound care, and now contributes to our WA Wound Interest Group and the National Wound Service Network to support evidence-based best practice.

14.01.2022 This #WorldAlzheimersDay, we are excited to relaunch our dementia-friendly programme with Dôme Café that promotes social connections. Our clients are now out in the community, but in a supported environment. Their families are at ease because their loved ones are safe and enjoying themselves, said Caitlin Burnett, Dementia Change Champion. Social connections have proven benefits for people with dementia - reducing loneliness and cognitive decline, avoiding mental health co...nditions, and maintaining independence. Our WA clients with dementia can visit Dôme Café with confidence knowing the programme is staffed by our Dementia Change Champions and specially trained café workers. We have 37 trained Dementia Change Champions, including Caitlin Burnett and Angela Hill, who started the initiative to help clients to maintain vital social connections.

11.01.2022 Kirsty Tilden is working with our research team to understand ways to identify when our respiratory clients start to deteriorate, so we can respond quickly. For some of our clients, their respiratory condition is progressive and can lead to their health deteriorating. We are keen to understand the triggers that lead to a change in well-being so that our care can be targeted and focused on their needs and preferences, said Kirsty. Our in-home respiratory service provides oxygen therapy to more than 1,000 clients in Perth, and is supported by telehealth technology which allows the team to monitor each client’s condition. For Nursing in the Community Week, we thank the 18 respiratory nurses who make this possible.

Caitlin’s dedication and leadership are evident through her dementia awareness and advocacy, where she is determined to create an inclusive environment for anyone living with dementia and their carers. 09.01.2022 We support our older clients to maintain their strength, coordination, balance and flexibility, to remain active with a team of physios who specialise in gerontology. This month, our highly-skilled allied health leaders, Frances Wallwork and Kathryn Devereux, lectured at the Australian Physiotherapy Association's Gerontology course. Pictured is our physiotherapist Lara who attended the course: "Factors like mobility, balance, continence, and medications, can combine to cause huge impacts for older people. I'm looking forward to using my new tools to support our clients maintain their wellbeing and independence."

09.01.2022 Gylsie Antoni is a case coordinator in our multidisciplinary Wounds Interest Group. This national group brings together our expertise in wounds, is a forum to share best practice, trial new products and approaches and develop new policies and procedures. Since we started in 2016, the Wound Interest Group has continued to grow. Our training modules for our teams have expanded. We have advanced our knowledge and skills and developed our wound care plans to offer Best Care to our clients. This also enables our nurses to work more efficiently, said Gylsie.

08.01.2022 Over the last month, Jennifer and Joanna have visited 75% of aged care facilities in the west of Melbourne to teach staff about infection prevention and control and how to use Personal Protective Equipment correctly. 07.01.2022 In South Australia, we lead the state-wide HIV service. For Nursing in the Community Week, we spoke to Caroline Amato, Director Clinical Operations, who manages this important service. Our care is underpinned by confidentiality, acceptance, care, trust, empathy, respect, empowerment and partnership, said Caroline. Our clients are empowered to self-manage their health at home to avoid hospital admissions. We're privileged to serve the community by tailoring care packages for people who are either newly diagnosed with HIV; living with complex health needs; or new to treatment and care in SA.

07.01.2022 Our team is on the front-line protecting Victoria’s most vulnerable aged care residents from COVID-19. Our highly skilled community nurses, Jennifer Zerafa and Joanna Georgiou, are teaching staff in residential aged care about infection prevention and control and how to use Personal Protective Equipment correctly. 06.01.2022 With my weekly physio sessions, I can climb the stairs no problem! said our 79-year old client, Keith. Siobhan and Isabelle from our physio team designed tailored exercises so Keith could strengthen his arms and legs following an emergency bypass operation in March. Using simple household items to exercise, Keith has improved his balance and enjoys the independence of walking around Fremantle. 06.01.2022 They say there’s no place like home. For hundreds of South Australians, it is the place where they receive our nursing care, including blood transfusions essential to their health. Kelly Fraser, South Australian Director Clinical Operations, says our clients can be elderly and frail; or young but with low immunity, so the blood transfusion service is vital in limiting visits to the hospital. Understandably, the demand has continued to grow. "In 2019, we completed a total of 291 blood transfusions, and the past six months we have already done more than 300, said Kelly.

03.01.2022 Today for Wounds Australia's Wound Awareness Week, we recognise the expertise and dedication of our highly trained wound Nurse Practitioners. Margaret Edmondson, Pam Morey and Gordana Petkovska in WA, and Sue Templeton in SA, treat the hardest-to-heal wounds in our clinics with their advanced knowledge and skillsets. On each visit, we collect information on the assessment and management of our client’s wound. This data is entered into our specialised wound module, which all...ows us to use data to monitor healing progress and benchmark our wound healing outcomes, said Pam Morey. Our nurses undergo evidence-based wound management education and clinical competency evaluation regularly. We are proud of our nurses with postgraduate qualifications in the field, which enables advanced wound practice and specialisation.

02.01.2022 Sharon Boxall’s research is improving the healing time of our client’s wounds. Sharon’s latest study is looking at the effectiveness of compression wraps compared to compression bandages in treating leg ulcers. She is passionate about finding the most effective and comfortable wound products for our clients and has been researching compression therapy since 2018. ... Wound research changes the way we address problems as we encourage the exploration of best practice and we look beyond the immediately obvious to seek solutions, said Sharon. Sharon has also been involved in research for Stomal Therapy Nurses for rural patients, a diabetes telehealth pilot in collaboration with Wirraka Maya Aboriginal Health Service and a variety of different projects looking at the relative effectiveness of different wound dressings.

01.01.2022 I have stayed with Silver Chain for many years because it means I can keep living at home and I don’t have to worry about looking after the house. For 84-year-old grandmother Delys Nielsen, being able to access care in her community means a lot. Delys receives in-home care in the form of cleaning, podiatry services and social support. She also has a personal alarm for around the clock support. ... Her care aide, Christine Hulme says she feels privileged to provide vital social support to Delys and other community members.
