23.01.2022 A prospective cohort study comparing labour and birth outcomes between women having their first baby who used epidural analgesia (EA) and women having their fir...st baby who didn't have an epidural shows significant associations between epidural use and several intrapartum outcomes. "Women using epidural analgesia were more likely to require a vacuum-assisted birth (RRR 3.35, p < 0.01) or forceps-assisted birth (RRR 11.69, p < 0.01). Exposure to EA was associated with significantly greater risk of 10 h first (OR 6.72, p = 0.01) and 2 h second (OR 2.25, p < 0.01) stage labour, increased likelihood of receiving IV syntocinon (OR 9.38, p < 0.01), antibiotics (OR 2.97, p < 0.01) and a greater probability of pyrexia (OR 10.26, p < 0.01). Women who used EA were half as likely to be breastfeeding at three months postpartum (OR 0.53, p < 0.01). 17.01.2022 Today on the show I speak to Dr Clare Davison. Clare is a privately practising midwife who attends births at home as well as in hospital. She is also an academi...c who holds a PhD and she is a lecturer in midwifery at Edith Cowan University here in Perth, Western Australia. Clare and I chat all things birth and feminism. We start by chatting about her own births - always my favourite way to start an interview. We then chat about working inside the system, working outside the system, changing the system or maybe creating a new one? Ultimately a birth revolution! Clare is the woman who is changing the face of birth by educating the next generation of midwives and I am very grateful for her work, knowledge and passion. Dr Clare Davison is a midwife, academic and feminist. Clare is passionate about promoting and supporting physiological birth and working with women so that they can be empowered in their birth choices. 10.01.2022 All screening tests have downsides. They may lead to overuse of intervention. And every time we use technology like this, we are undermining women’s own knowled...ge. The constant use of technology implies that our bodies aren’t capable of growing, birthing and feeding babies without a shed load of expensive machinery. And that’s not just true. I spent last week chatting with a wonderful, passionate group of midwives, obstetricians, doulas, childbirth educators, hypnobirthing teachers and other birth workers from around the world, and this came up - again - as a key theme that so many of us are experiencing. It's so important to keep sharing positive messages about the awesomeness of the female body. And to remind people that we did this for millions of years before the machines came along. 07.01.2022 We have our doula training practice day on Monday 14th September and if you are 38 weeks or more and would be able to donate your time to helping out with doula... practice that would be great. You would need to be willing to spend 2 and a half hours having a combination of massage, acupressure, a variety of doula skills including working with a rebozo, pelvic pathways positioning / stretching and receiving labour and birth information. We need 8 women for the morning session from 9:30am till 12pm, then another 8 women for the afternoon session from 2pm till 4:30pm (we cannot change these times so you must be available these times). I’m sorry that we can’t respond to all the comments and questions that we get sent, but... if you’re looking for information on induction (or other birth-related topics, come to that), there’s loads on www.sarawickham.com I have also written a book about induction of labour if you would like to better understand the different aspects of this process and the evidence relating to it. You can see that at www.sarawickham.com/iol I also want to re-share this important thought. When it comes to making decisions about things like induction of labour, it’s not that people are trying to persuade women to say no. It’s about trying to make sure that women have all the information they need to decide whether a particular path is right for them. As recent research has again confirmed, there are also advantages to waiting. The research that suggests that induction confers benefits isn't as robust as some people make out. And many women who have induction and similar interventions during their births later wish that they had been told more about what it had involved and about what it would mean for them and their family. Some are very happy with their decision but others are not.
