A normal Power of Attorney (called a General Power of Attorney) is often inadequate as it loses effect if you lose mental capacity. Accordingly, you need what is called an Enduring Power of Attorney. An Enduring Power of Attorney will continue to be effective even when you lose mental capacity to carry out your affairs.
