20.01.2022 We are learning as much as we can this month about tele-rehabilitation, including how to introduce technology/Zoom successfully, conduct analysis from a distance, lead a novice stroke survivor through the process of setting up their device and any equipment for quality practice that is safe, and coaching/progressing their practice. While we have done many Zoom meetings and consultations with stroke survivors over the years online, its time to learn from the research and clin...icians who know more than we do. Thanks Kate Scrivener and Laura McCredie at Concentric Rehab for sharing your resources and letting us join a tele-rehab group session. Other followers - please share your tips below. Karl and Annie are preparing to run the 3-day UL workshop fully online with 8 stroke survivors in early Feb 2021 (watch this space...!) . Unfortunately two face-to-face workshops in Melbourne had to be cancelled (again) due to COVID travel restrictions between NSW and Victoria. So we have decided to trial the event online (thanks Tash Lannin & Steve Woollard), which will make future international workshops feasible without travel. We know that the lectures and other practical sessions can work well having recently run a 1-day workshop with therapists on the Orkney Islands, Isle of Wight, Singapore, Spain, NZ and Canada. What we have not yet trialled online is analysis and training with 8 new stroke survivors / 8 groups of therapists. All therapists and stroke participants will be in different locations. We plan to spend time on the phone with individuals, then via Zoom preparing them for the workshop. We know that not everyone uses computers or tablets (or has wifi), and success in the first session is critical. This special issue in Frontiers in Neurology about telerehab is one of many journals we've been reading - we already know many of the barriers and disincentives which need to be addressed for success. But there are positives. Tele-rehab can help broaden your social network - when family and friends help set-up and practice - other stroke survivors also meet and talk online and socialise before or after group sessions - they get to see what other people are doing in their practice, how exercises are set up, and how much home practice is being done (much like the sessions during our workshops, where we collate practice reps and measure change across the 8 stroke participants). https://www.frontiersin.org//127/tele-neurorehabilitation
20.01.2022 Big day today. Our first face-to-face workshop since March 2020. Feeling a bit rusty but enjoying delivering this coaching skills workshop with Karl Schurr after so many months. Including videos of Karl’s rehab in our workshops, such as the feedback he found valuable back in the days when he could barely sit on the edge of the bed due to severe breathlessness.

"We are OT students currently on placement at Toowoomba hospital in QLD, and part of our project is to contact OTs in rehabilitation centres around Australia in order to gather information about which UL assessments or outcome measures are being used in their setting. 10.01.2022 Our final workshop for 2020 was held this week (1000 reps online) with 30 therapists from Australia, NZ, Denmark, Isle of Wight, Spain and Canada (some keen therapists stayed up till 4am ......). We will be taking a break until late January, when online workshops commence again. We are hopeful that our 2-day and 3-day workshops with clinical sessions will recommence in Feb/March in Melbourne and Sydney (see link below to calendar). 10.01.2022 Post-COVID 19 rehab, with a specific bushwalking goal: walking from Mt Kuringai Station to Apple tree Bay and back. Extensive uphill walking on the way back. 8,5km round trip. Goal achieved before the self- determined deadline of 1 Sept. But this is what recovery from COVID looks like when lungs are badly damaged.....

Weekly outdoor bootcamp on Sat with Simone Dorsch in Ashfield while Karl walks around the oval with 10kg in his backpack, up / down hills Enjoying this varied range of enforced exercise and ‘hands on’ case management to check the patient’ meets his goals. Weekly physiotherapy tele-rehab session this morning. Nothing like a meeting with a therapist (or supervisor) to focus the mind.

01.01.2022 Listen to, and participate in journal club activities related to CIMT and the large EXCITE randomised controlled trial. The PEDro Worldwide Journal Club provides a structure for clinicians to follow, which aims to encourage practice change. Resources provided by PEDro include: a research article that addresses an important evidence-practice gap; a video summary of the article; a panel discussion about the article that explores applying the results into practice; and, links t...o resources to support ongoing discussion and implementation. PEDro recently posted two recordings as part of a journal club about CIMT. The first recording is a short 3 minute appraisal of the EXCITE trial. The second recording (40 mins) includes a really interesting and informative panel discussion directed by Simone Dorsch (from StrokeEd), and included a patient participant, recent graduates, clinicians and researchers who have implemented CIMT in Sydney. The CIMT implementation was led by PhD candidate and occupational therapist Lauren Christie, a member of the panel. I highly recommend that you take the full 40 minutes to listen to the recording.
